You, Doris Smith, Administrator, assisted me with the visit. Your program currently operates with Notice of Compliance issued June 14, 2018. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-nine percent as of January28, 2026. The NC Secretary of State website was reviewed on January 28, 2026, and Community Christian Child Care Center, Inc. was listed as current- active. The notice of Compliance was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children actively engaged in activities in the indoor learning environments. Compliance with supervision and staff/child ratios requirements were met. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were completed, recorded, and maintained as required. Lunch was also observed and consisted of chicken with gravy and rice, string beans, pineapples, and milk. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed and compliance was met. A fire has not occurred since the last visit and remains current. An updated sanitation inspection was completed on November 13, 2025, and a superior classification was earned. You gave me a copy of the current sanitation inspection report today. Four violations were observed today. The violations are as follows: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Bolts were protruding from the black border enclosing the surfacing on the playgrounds used by children. An open drink can was on a cubby shelf accessible to children in Space #3. The top of the small storage shed on the back playground and accessible to children was cracked exposing sharp edges. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A can of air freshener in an aerosol dispensed container and a container of Clorox wipes were observed in an unlocked cabinet in Space #3. A staff member’s purse was observed in an unlocked closet in Space #7. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Two tubes of diaper cream kept onsite for children were expired in Spaces #1 and #4. .0803(12) 9995 A violation was found for which there is no item number. The diaper pad on the diaper changing table in Space #4 was torn. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. On or before February 11, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 TECHNICAL ASSISTANCE/GUIDANCE: SAFE ENVIRONMENT: We discussed the potential hazards identified during the observation. I explained that all hazards must always be made inaccessible to children. During the observation, unlocked cabinets containing aerosol cans of air freshener, a staff members purse, and cleaning products were noted. I shared that if these items are kept in spaces used by children, they must be secured in locked storage and made inaccessible to children. We also talked about beverages brought into the classroom by staff children are present. I shared that staff must model appropriate eating and drinking behaviors in the presence of children and remain mindful of safety concerns, including risks posed by open cans which could potentially cause injury to children is obtained. SANITATION REQUIREMENTS: I reminded you that diapering surfaces shall be made of smooth, intact, non-absorbent material and shall be kept clean and in good repair. The diaper changing pad on the diaper changing table in the infant classroom was torn. We talked about ensuring diaper changing pads remain in good repair so they can be easily cleaned after usage. You inquired about other options available instead of a diapering pad. I advised you to speak with the Environmental Health Specialist regarding guidance. I also encouraged you to replace the crib mattress that was torn and exposing the interior. This poses a risk to the child/children utilizing the crib. I explained, although the mattress will be covered with a sheet, there could be a time it becomes accessible to children. SEX OFFENDER REGISTRY: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. (§14-208.19) To register for the e-mail notification, go to http://sexoffender.ncsbi.gov. If you have any questions, please contact your local sheriff's department. D. Smith, Administrator, was present and assisted with the visit. This center currently operates with a Notice of Compliance issued on June 14, 2018. The last annual compliance visit was completed on September 9, 2024. The last sanitation inspection was completed on June 2, 2024, earning a “Superior” classification. The last fire inspection was completed on March 18, 2025, and the center was approved for daytime care only. Forty-four (44) children ranging from ages zero (0) to five (5) years old were observed in care today. Children were engaged in free-choice play, outdoor play, eating breakfast and lunch, completing hand washing diapering, and toileting routines, and interacting with caregivers. Breakfast consisted of pancakes, bananas and water. Lunch consisted of ravioli, peas, mixed fruit and milk. The following violations were documented. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Departure and arrival times were not recorded for approximately ten children between the dates of July 3, 2025, to August 15, 2025. 10A NCAC 09 .0302(d)(4) 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. A staff/child ratio chart was not posted in Space #3. .0713(a)(10), (c) & (f)(3); .2818(e) 807 A safe indoor and outdoor environment was not provided for the children. Bolts were protruding from the carport shed and a bottle of hand sanitizer was in a space accessible to children on the toddler playground, a bottle of hand sanitizer hanging on a string attached to the wall was accessible to children in Space #4. 10A NCAC 09 .0601(a) 843 A drug or medicine was administered after its expiration date. Sunscreen and diaper cream onsite for four enrolled children had expired. 10A NCAC 09 .0803(1)(d) 1757 A valid qualification letter was not on file and available to review at the facility. Prior to making any changes in the ownership status contact me to discuss the change of ownership procedures. Failure to comply could impact the program license status. FACILITY PROFILE INFORMATION: You stated the mailing address, phone number and email address listed for your program are correct. If changes in your facility information occurs, please contact me at 252-214-2709 or email keshia.hayward@dhhs.nc.gov to discuss the changes and ensure the information is updated in the system. QUALIFICATION LETTER: A printout from the ABCMS portal was on file to verify completion and qualification of criminal background requirements for one employee hired on August 4, 2025. I informed you that a qualification letter must be available and on file for all staff; therefore, the printout was not valid. You stated the employee could not retrieve the qualification letter from the criminal background check portal to provide. I explained an email was sent to the employee with a link to retrieve the qualification letter. I recommended contacting the Division’s criminal background check unit to request assistance with retrieving the emailed qualification letter. SAFE INDOOR/OUTDOOR ENVIRONMENT: Bolts were protruding from the carport shed on the toddler playground. Also, hand sanitizers were in spaces accessible to children both indoors and outdoors. I suggested moving the hand sanitizer attached to the wall because the children could reach it. You said the hand sanitizer on the playground would be removed and staff will be required to monitor it to ensure safety of children. ARRIVAL/DEPARTURE TIMES: As part of continued efforts to maintain accurate and compliant records, encourage staff to consistently check sign in/out logs each day. It is important that times of drop-off and pick-ups are recorded as required. It parents forget to sign children in/out, staff may document the times and add their initials to indicate who recorded the information. Maintaining accurate records for sign in/out helps your program stay aligned with licensing standards and accountability of all children in care. MEDICATION REQUIREMENTS: During the visit, we discussed reviewing the permission forms submitted by parents to ensure all required information is included. We also talked about periodically checking all medication and creams to ensure expiration dates are current. I explained any time medication or topical creams expire, they should be returned to the parent or discarded. MODELING APPROPRIATE EATING BEHAVIOURS: A cup of soda was sitting on a shelf at the entrance door where children could access it. I reminded you staff should model appropriate eating behaviors in the presence of children and this includes drinks. I explained that a child could have picked up the soda to consume it without the caregiver’s knowledge. Staff should always remain aware of keeping personal drinks, food and items out of reach. We talked about ensuring staff role model appropriate eating behaviors by consuming only food or beverages that meet the nutritional requirements. IT'S SIDS TRAINING: Based on my observation, a new employee was scheduled to complete virtual IT’S SIDS training. I informed you the first IT’S SIDS training must be a seated course. You stated there has been difficulty finding seated training offered in your local area. I suggested checking with partnerships or other trainers in neighboring counties. TRANSPORTATION REQUIREMENTS: You stated the program transports school-age children during the school year; however, that has not started yet. I reminded you to ensure photographs are added to the emergency cards for upcoming riders. I informed you three seatbelts were broken; therefore, those seats cannot be used during transportation. You stated a request would be submitted to have the seat belts repaired prior to using those spaces on the vehicle. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 10/18/2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. Parents must complete a medication authorization form upon delivering the medication to the child care facility. In space 2, a valid medication authorization form was not available for an Albuterol inhaler and a bottle of Ceterizine. You stated the child was transitioned to this room and that you thought all of the forms had been updated. You removed the medication from the classroom and informed the parent to complete the form upon pickup. Create a plan and share it with your staff to ensure all medications received have the correct authorization form and it is completed properly before accepting it from the parent. New Criminal Background Check Changes – Legislative changes approved on 7/11/2022 made two important changes to Criminal Background Checks required for child care professionals: 1. The period for background check qualification renewals was changed from every 3 years to every 5 years 2. The provisional qualification period was reduced to 45 days Public Health Toolkit Updates - The ChildCareStrongNC Public Health Toolkit for Child Care (updated 7/2022) available on the DHHS website, and sent to your facility contact email, provides guidance on requirements and recommendations to reduce the spread of COVID-19 in child care facilities. Annual Licensing Fee – Licensed facilities that have a current license (whether active or inactive) on October 1st of each year will receive an invoice for a licensing fee based on their licensed capacity.
