This visit was conducted by Karen Young, Childcare Consultant with Donna LaPlante & Shannon Miller, lead teachers. This is an NCPK program that consists of 2 classrooms and is housed in a public school. It is operated by the Wilkes County School system. Your program currently operates with a FIVE (5) Star license effective 12/13/19. Restrictions include 1st shift daytime care; meets enhanced ratios; meets all enhanced requirements and reduced ratios minus one. School age playground cannot be used by preschoolers. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children coming in from outdoor gross motor play, having free choice center time, and preparing to go outside. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold with Foundations embedded in the lesson plans. The last annual compliance visit was conducted on 3/8/24. A sanitation inspection was completed 11/6/24 with a Superior classification and eight (8) demerits. The last fire inspection was conducted 8/14/24. Program records and required postings were monitored. A fire drill was conducted on 1/17/25 at 2:15 pm with two (2) minutes twenty-seven (27) seconds to evacuate twenty-five (25) children and four (4) adults by Becky Speers, principal. A lockdown emergency drill was documented on 1/7/25 at 10:30am by D. LaPlante. An outdoor inspection was documented on 2/4/25 by C. Messick. Two (2) staff files and four (4) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked cabinets. Storage and administration of medication were monitored. Medication authorization was monitored. There was one (1) Epi-Pen with Benadryl in Space 118 for peanut allergy and two (2) inhalers in Space 154. You stated that you do provide transportation in the morning and afternoon for the NC Pre-K children. During today’s visit, I monitored the Public-School Off-Site Records Verification form signed by Eric M. Barker Director of Transportation on 8/20/24. I observed the Public-School Staff Records signed by Dr. Westley Wood, Asst. Superintendent of Personnel and Human Resources. The Transportation Policies for Parents for each child who is transported was observed in the children’s files. NC Pre-K requirements The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teachers, Donna LaPlante and Shannon Miller, and teachers M. Miller & K. Messick. A selection of files was monitored for complete health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold and Foundations embedded in the lesson plans. You stated you were in the process of completing the middle year (winter) screenings. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/26/24. The classroom operates from 7:15 a.m. to 3:15 p.m. Parent conferences are held at least two (2) times per year. You stated that you use Remind App, and phone calls to communicate with parents. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by Parent Teacher Organization (PTO) meetings, family meetings , conferences, and monthly newsletter. The environment rating scale score received on 12/11/19 was 6.35. The following violation was cited at today’s visit. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The record was incomplete as it did not include the time it took to reach the designated area. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Tuesday, March 4, 2025. ECERS-3 and ITERS 3and FCCERS 3 are being used now and began 2/1/25. • The QRIS is in the process of being implemented and is now open for public comment. Go to the Division’s website, under What’s New and scroll down to QRIS Modernization, then read about QRIS Modernization Rulemaking Update. The Public comment is open 2/3/25 through 4/4/25. This is your opportunity to comment. • The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Due to the recent impacts of Hurricane Helene and other natural disasters, this deadline has been revised statewide, and facilities that have not yet enrolled should complete the online process by May 31, 2025. The Clean Classrooms for Carolina Kids Program provides free facility-wide testing and inspections without any out-of-pocket costs. Due to the volume of testing and inspections, completion of the online enrollment process in the program is considered temporary compliance with the testing and inspection requirement and will allow facilities to participate in the funding mechanisms. Lead teacher, Donna LaPlante and S. Miller assisted me with the visit. Your program currently operates with a FIVE (5) Star license effective 12/13/19. Restrictions include 1st shift daytime care; meets enhanced ratios; meets all enhanced requirements and reduced ratios minus one. School age playground cannot be used by preschoolers. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time and outside gross motor time on the enclosed preschool playground. One (1) new teacher assistant staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Two (2) medications were observed in Space 118 and one (1) Epi Pen was observed in Space 154. Storage hazardous items was monitored today. A playground inspection was recorded 9/3/24. A fire drill was recorded 8/29/24 at 9:47 am with two (2) minutes fifteen (15) seconds to evacuate thirty (30) children and four (4) adults by S. Byers. A shelter in place drill for the facility was last recorded 1/26/24. The most recent fire inspection was on 8/14/24. I observed a sanitation inspection was last conducted on 4/8/24 earning a Superior classification with eight (8) demerits. You stated that you do provide transportation for NC Pre-K children on the Wilkes County School buses. The following violations were cited during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. I observed there was no documentation of a recent fire inspection in the center file for this program. This violation was corrected during the visit today as I received the documentation of the recent fire drill on 8/14/24 from the front office. 10A NCAC 09 .0304(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. Lead teacher, Donna LaPlante and S. Miller assisted me with the visit. Your program currently operates with a FIVE (5) Star license effective 12/13/19. Restrictions include 1st shift daytime care; meets enhanced ratios; meets all enhanced requirements and reduced ratios minus one. School age playground cannot be used by preschoolers. The program’s compliance history was 97% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time and outside gross motor time on the enclosed preschool playground. One (1) new teacher assistant staff were reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Two (2) medications were observed in Space 118 and one (1) Epi Pen was observed in Space 154. Storage hazardous items was monitored today. A playground inspection was recorded 9/3/24. A fire drill was recorded 8/29/24 at 9:47 am with two (2) minutes fifteen (15) seconds to evacuate thirty (30) children and four (4) adults by S. Byers. A shelter in place drill for the facility was last recorded 1/26/24. The most recent fire inspection was on 8/14/24. I observed a sanitation inspection was last conducted on 4/8/24 earning a Superior classification with eight (8) demerits. You stated that you do provide transportation for NC Pre-K children on the Wilkes County School buses. The following violations were cited during today’s visit: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. I observed there was no documentation of a recent fire inspection in the center file for this program. This violation was corrected during the visit today as I received the documentation of the recent fire drill on 8/14/24 from the front office. 10A NCAC 09 .0304(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. I ovserved the last shelter-in-place drill was on 1/26/24. I observed no further documentation of emergency drills being conducted since that date. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Program records and required postings were monitored. A fire drill was conducted on 2/28/24 at 8:23 am with one (1) minute thirty-nine (39) seconds to evacuate twenty-four(24) children and four (4) adults by S. Bumgarner, Assistant Principal. A shelter in place drill was documented on 1/16/24 by S. Bumgarner. An outdoor inspection was documented on 3/1/24 by C. Messick, teacher assistant. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. You stated that you do provide transportation am and pm for Pre-K children. During today’s visit, I observed the Public Schools Off-site Records Verification - Staff Records, signed by Dr. Westley Wood, Assistant Superintendent on 8/23/23. I observed the Public-School Off-Site Records Verification -Transportation Requirements signed by Eric Barker Director of Transportation signed on 8/23/23. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teachers, D. LaPlante, and S. Miller and teachers, C. Messick and M. Jenkins. Ms. LaPlante and Ms. Miller both have a BS degree in B-K Early Childhood. Ms. Messick has an AA degree in Early Childhood and Ms. Jenkins has a general education degree. A selection of children’s files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold. You completed the Dial-4 screenings for February. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/28/23. The classroom operates from 7:50 a.m. to 3:05 p.m. Parent conferences are held two (2) times per year. You stated that you use Class Dojo to communicate with parents. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by inviting parents into the classroom for special activities and allowing parents to send materials and snacks into the classroom for special projects. The environment rating scale score received on 12/13/19 was 6.35. The following violations were cited at today’s visit. This visit was also used to monitor compliance with applicable childcare requirements located in Child Care Rule Section .3000 in spaces 118 & 154, where children participating in the NC Pre-K program are cared for. This visit was conducted by Karen Young, childcare consultant, with S. Miller, lead teacher. Ms. D. LaPlante was absent from school today. M. Miller was her substitute in Space 154. Your program currently operates with a FIVE (5) Star license. Restrictions include 1st shift daytime care; meets enhanced ratios; meets all enhanced requirements and reduced ratios minus one. School age playground cannot be used by preschoolers. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice center time, having dance/movement time, and having gross motor play outside on the playground. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on 3/13/23. A sanitation inspection was completed 11/16/23 with a Superior classification and nine (9) demerits. The last fire inspection was unavailable for review today. Program records and required postings were monitored. A fire drill was conducted on 2/28/24 at 8:23 am with one (1) minute thirty-nine (39) seconds to evacuate twenty-four(24) children and four (4) adults by S. Bumgarner, Assistant Principal. A shelter in place drill was documented on 1/16/24 by S. Bumgarner. An outdoor inspection was documented on 3/1/24 by C. Messick, teacher assistant. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. You stated that you do provide transportation am and pm for Pre-K children. During today’s visit, I observed the Public Schools Off-site Records Verification - Staff Records, signed by Dr. Westley Wood, Assistant Superintendent on 8/23/23. I observed the Public-School Off-Site Records Verification -Transportation Requirements signed by Eric Barker Director of Transportation signed on 8/23/23. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teachers, D. LaPlante, and S. Miller and teachers, C. Messick and M. Jenkins. Ms. LaPlante and Ms. Miller both have a BS degree in B-K Early Childhood. Ms. Messick has an AA degree in Early Childhood and Ms. Jenkins has a general education degree. A selection of children’s files was monitored for completed health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold. You completed the Dial-4 screenings for February. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/28/23. The classroom operates from 7:50 a.m. to 3:05 p.m. Parent conferences are held two (2) times per year. You stated that you use Class Dojo to communicate with parents. In addition, a daily report is sent home to the parents which includes any information about the child’s day and any reminders. You verified that you incorporate family engagement by inviting parents into the classroom for special activities and allowing parents to send materials and snacks into the classroom for special projects. The environment rating scale score received on 12/13/19 was 6.35. The following violations were cited at today’s visit.
