Patsy Reavis, Director, was off premises today. Shirley Campbell, assistant director assisted me with the visit. Emily Lamquaye, child care consultant, assisted me with the visit to this certified developmental day program. Your program currently operates with a FIVE (5) Star license effective 10/17/19. Restrictions include 1st shift day time care; meets enhanced ratios; meets enhanced space; Meets all enhanced requirements and reduced ratios minus one. Can care for children up to age 21; Certified Developmental Day. The program’s compliance history was 99% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time, and outside time, daily routine care in the infant room, and transitions from room to outside. No new staff were reported at this program, no files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. We observed over the counter diaper creams, ointments, and a nasal spray. We observed two (2) Epi pens and two (2) inhaler medications today. Storage hazardous items was monitored today. A playground inspection was recorded 8/12/24. Removal of large plastic block that was cracked, mulch was added around swings and circle track and garden boxes were removed from large playground so grass can grow were items noted on the playground inspection sheet. The large playground was closed on 8/12/24 and remains closed due to a tree that produces poisonous berries that fall on to the playground surface. The county has been notified and the tree will be removed by 8/26/24, the first day back to school for the fall semester. The children have an alternate outdoor fenced in space for gross motor play while the large playground is closed. A fire drill was recorded 8/13/24 at 10:40 am with one (1) minute, sixteen (16) seconds to evacuate thirty-four (34) children and eighteen (18) adults by D. Roope. A lockdown drill for the facility was last recorded 7/25/24 at 11:27 am by P. Reavis. The most recent fire inspection was on 8/30/23. I observed a sanitation inspection was last conducted on 2/28/24 earning a Superior classification with thirteen (13) demerits. You stated that you do not provide transportation. The following violation was cited during today’s visit: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. We observed there was a hole in the window screen in Space 4. A cracked refiregerator cabinet door was also observed in this space 4. G.S. 110-91(6); .0601(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violation cited during today's visit immediately and send me documentation verifying compliance on or before Thursday, August 29, 2024. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action taken by the DCDEE. Technical assistance provided during today’s visit: • Any broken furnishings and equipment must be replaced or repaired immediately. See Child Care Rule 10A NCAC .09 .0601(b). Consultation during today’s visit: • You must give me a thirty (30) day notice for any change in location, address, or ownership status. • You are in Cohort 1. Your prep year is over, and you are now in your reassessment year, however, the Cohorts are on hold. You are still in Hold & Harmless. I will let you know of any updates as I am made aware by the Division. • The assistant director stated that all rugs in the center are cleaned on a regular cleaning schedule by a professional company. • A tent was observed in Space 3b that was not fire retardant. The teacher is taking the tent out to spray with a fire-retardant spray before placing back in the classroom. • Today we discussed with assistant director and Pam Hauser, supervisor, via phone, that this certified developmental day center has two (2) NC PreK classrooms. They have one certified BK licensed lead teacher to serve the developmentally delayed children and NC PreK children. The other NC PreK classroom has an Associate degreed lead teacher in Early Childhood with fourteen (14) years of experience who is currently pursuing her BK degree. The Wilkes County NC Pre K coordinator, Jeff Johnson and state NC PreK representative, Jennifer Griffith, has agreed to allow this Associate degreed teacher to be the lead for twenty-four (24) weeks while the center continues recruitment.
