Name of Operation: MIRACLE WORKERS DAY CARE Facility ID: 09000103 Consultant: MIRIAM BYRD Operation Type: Family CC Home Case Number: Visit Date: 12/10/2025 Number Present: 1 Completed Date: 12/10/2025 Age: From 8 To 8 Total Minutes: 100 Time In: 09:50 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements pertinent to a Routine Unannounced visit. Your program currently operates with a 4-star license, issued June 13, 2023, earning 7 points in Staff Education, 2 points in the Program Standards component meeting minimum requirements, and 1 point in Quality for restricting infant capacity to no more than two children under one year of age. Anna Freeman, home operator, was present during today’s visit with one school-age child. The last annual compliance visit was conducted on May 21, 2025. The license was posted and the restrictions were in compliance. During today’s visit I reviewed the Verification of Required Information for Ms. Freeman. The following was verified in compliance: Current CPR and First Aid certification (expires February 2026), ITS-SIDS (expires April 2026), and Current Health Questionnaire. Your last Well water test date was August 18, 2023. The well water test must be completed every two years. There was one violation of child care requirements documented during today’s visit. Computer-generated report was reviewed with Ms. Freeman, and a copy was printed at the conclusion of today’s visit. Violation Number Comment Rule 2059 The family child care home operator did not have results of the water sample analysis on file in the home, showing they are free of bacteria and lead hazards. The last well water test was completed on August 18, 2023. 10A NCAC 09 .1725(a)(1)&(2) The above violation must be corrected immediately. In your response to the violation, you must submit a compliance letter addressing the violation as to how it was corrected. Due to the upcoming Christmas Holiday, the compliance letter must be received, via email (Miriam.Byrd@dhhs.nc.gov) on or before December 31, 2025. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Your compliance history was reviewed with you during today’s visit. Your compliance history score is 95%. Any violation(s) documented may impact the compliance history score. QRIS: Pathway to the Stars: Pathways to the Stars: During today's visit I reviewed the QRIS Pathways with Ms. Freeman. She stated she is interested in Pathway 2: Classroom and Instructional Quality. I reviewed each of the requirements to apply for a 5-star license. We agreed that Ms. Freeman would apply by May 2026. I gave Ms. Freeman information on a free Curriculum and Formative Assessment that she could review before May 2026. You are currently using Creative Curriculum but are interested in updating the curriculum to better align with children’s developmental needs and current best practices. I reviewed the Family Community Engagement Foundational Practices with Ms. Freeman. She shared current strategies used to communicate with families, and community resources. Opportunities for strengthening documentation and enhancing engagement activities were discussed. I informed Ms. Freeman I would review the family community engagement practices she selected in May 2026. Reminders: -Your Criminal Background Qualification letter expires December 15, 2025. To renew your 5-year NC Child Care Criminal Background Check (CBC), you must apply online via the NC ABCMS portal using your NCID, complete personal/work info, pay the fee, print the fingerprint release form, and get electronically fingerprinted at an approved law enforcement agency to be submitted for your federal check, with results sent to you and the facility. -Your son will turn 16 years of age December 2026. An individual over the age of 15, who resides in the home must complete and submit required forms for a Criminal Background Check within five (5) business days of turning 16. -When the puppy is of age to receive vaccinations, you must keep rabies vaccination records on file. Technical Assistance with the following was provided: -Family and Community Engagement Standards -The discussion focused on strategies for building positive, ongoing relationships with families, enhancing communication methods, and incorporating community resources into the program activities. Some examples of ways you can show proof of the engagement standards are: -maintaining sign-in sheets for family events and meetings -keeping a log of communication with families about community resources -documenting volunteer opportunities and cultural exchanges -taking photos or creating a portfolio of events -Coaching/Training options- I informed Ms. Freeman the local Child Care Resource & Referral Agency can offer technical assistance and on-site coaching to assist with the Rated License process. They can provide training on professional development, record keeping and portfolio preparation, and quality improvement planning. Bladen County falls under Region 4@SCCNC.edu.
