Jordan Myers, administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Mr. Myers, and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On March 5, 2026, a staff member failed to ensure a safe environment for a three-year-old child, resulting in the child falling off a chair and sustaining a laceration above the eye. 10A NCAC 09 .0601(a) 873 Center staff did not follow the EMC plan. On March 5, 2026, a three-year-old child, fell from a chair, hit their head on the classroom floor, and sustained a laceration above the left eye. Staff were unable to implement the emergency medical care plan because individuals identified as persons in center responsible for determining the degree of care needed and notifying parents were not on site. 10A NCAC 09.0802(a) 908 Discipline was not appropriate for the child's age and development. On multiple occasions, a staff member placed children in timeout for extend periods of time and required children to eat lunch with the classroom lights off for the duration of lunch time in response to children’s behaviors. .1803(b) 1876 A child was yelled at, shamed, humiliated, frightened, threatened or bullied. On multiple occasions a staff member yelled in children's faces and threatened children that they would miss family activities or would not see their parents in response to their behaviors. .1803(a)(9) Violations must be corrected immediately. This visit was conducted with Jordan Myers, Director. Your program currently operates with a Five (5) Star license effective 4/23/25. Restrictions include 1st shift; meets enhanced ratios; meets enhanced space; reduced ratios minus one. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 96% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. The children are enjoying the new sandbox on the preschool playground. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having outdoor gross motor play, infants and toddlers having outdoor play on the covered porches, and infants and toddlers having their individual needs met such as diapering. I observed materials and activities that were available to the children in the classrooms. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on 4/15/25. A sanitation inspection was completed on 12/11/25 with a Superior classification and six (6) demerits. The last fire inspection was conducted 12/22/25. A copy of this inspection was received during today’s visit. Program records and required postings were monitored. A fire drill was conducted on 2/26/26 at 2:53 pm with seventy (70) seconds to evacuate twenty (20) children and nine (9) adults by J. Myers. A lockdown emergency drill was documented on 2/24/26 at 2:56 pm with two (2) minutes, thirty-seven (37) seconds to lockdown by J. Myers. An outdoor inspection was documented on 2/26/26 at 4 pm by J. Myers. Sand washed away was noted on the playground inspection. The center now has a new sandbox area with new sand. Two (2) new staff files, one (1) administrator, one (1) assistant director and three (3) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked closets. Storage and administration of medication were monitored. Medication authorization was monitored. Today I observed one (1) Epi-pen for a peanut allergy in the Preschool space, group 4. The program does not provide transportation. The following violation was cited at today’s visit. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. This center staff is not registered on the ABCMS portal as of today 3/11/26. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Wednesday, March 25, 2026. Updated training modules are being added, including a Child Care Rule Rollout with new rules effective July 1, 2025, found under the Regulatory tab. Specifically, under the Orientation tab, I would encourage all staff to complete the training for “Child Development Module.” This training module provides a basic overview about child development and is designed to help early educators support infants, toddlers, and preschool children to reach their next developmental milestone. In addition to gaining introductory knowledge about child development, early educators will learn about the many resources and professional agencies available in North Carolina to help with promoting a responsive and engaging learning environment to support all children served. Upon completion, participants will receive one (1) contact hour credit (CHC). An individual NCID username and password is needed to access the DCDEE Moodle. A sanitation inspection was completed on 12/11/25 with a Superior classification and six (6) demerits. The last fire inspection was conducted 12/22/25. No assessments were conducted due to COVID 19. You submitted your application for re-assessment using the new ITERS-3 and ECERS-3 on 1/27/25 during Hold & Harmless. You are eligible for a new Star License based on the following: • Education: Points are earned based on the lowest level of education for any one position. The Administrator has BA/BS in EC/CD, has a Level III Administrator Equivalency, has BSAC and has seven (7) years’ experience, and ECE Level 11 earning seven (7) points. • The Program Coordinator has BSAC, is lead teacher qualified, and has 750 hours’ experience in a licensed school age care earning six (6) points. • The facility has four (4) required lead teachers. Each of them has NCECC or a 4 yr. Birth-Kindergarten degree and fifty (50) percent of them have completed at least twenty-one (21) semester hours and they have at least two (2) years of experience in childcare, earning seven (7) points. • Three (3) teachers are required, 50% of them have NCECC and at least one (1) year of experience earning them six (6) points. • One (1) group leader is required. They have completed SA coursework, and have at least 750 hours experience in school age care earning them six (6) points. Based on the information noted above, your program earned six (6) points in the Education component. • Program Standards: The program meets enhanced ratios, enhanced space, and enhanced ratios minus one. The facility completed the request for the Environment Rating Scales and submitted it with the reassessment packet on 1/27/25. The ERS was requested on 1/29/25. The ITERS-3 and ECERS-3, were all administered on 3/11/25 and 2/26/25. The assessment report was reviewed during a rated license visit on 4/15/25. The scores are as follows: ITERS-3 =5.94, ECERS-3 = 5.76. The average score is 5.85 earning this facility seven (7) points in this component. • Quality Point: You have chosen to meet a Programmatic option which is Staff Benefits Package and Infrastructure of Parent Involvement. I verified the Staff Benefits Package, and the Infrastructure of Parent Involvement is being implemented, therefore your program earned 1 Quality Point. • Total: Six (6) points in Education + seven (7) points in Program + (1) Quality Point = 14 = for a five (5) Star License. A new license will be mailed directly to you and must be posted upon receipt. At that time, you will return the old license to me as it remains the property of the State of North Carolina. It is recommended that Ms. Absher review the Facility Summary Report along with this visit summary with all staff and obtain any training needed and technical assistance from your local resource and referral agency, which will serve to assist you in providing enhanced care and improve future assessment scores. ERS summaries were reviewed today and items scoring less than a four (4) could constitute a violation of childcare requirements and therefore were monitored. These items are noted below: ECERS-3 assessment: Date:2/26/25 Teachers: B. Bennett, J. Coffey Total score: 5.76 Item:10 Indicator: 3.2 Score: 2 Assessor Comments: This indicator considers sanitary nap provisions. When stored, the cots were stacked on top of one another with a blanket folded and placed under the sheet in the middle of each cot; the sheets touched the bottom of the cots stacked above. The cots were made of a permeable material, allowing for the potential transfer of germs between the children's bedding and the cots. ERS Requirements: Some attempt was made to practice sanitary nap procedures (ex: children have sheets on the sleeping surface; most cots are at least 18 inches apart, and none are very close together. Observations today: Today I observed the cots had a Ziploc bag containing the blankets. The Ziploc bags are stored in the closet until nap/rest is over and then blankets go back in the Ziploc bag which is stored on top of each cot until use the next day. The facility completed the request for the Environment Rating Scales and submitted it with the reassessment packet on 1/27/25. The ERS was requested on 1/29/25. The ITERS-3 and ECERS-3, were all administered on 3/11/25 and 2/26/25. The assessment report was reviewed during a rated license visit on 4/15/25. The scores are as follows: ITERS-3 =5.94, ECERS-3 = 5.76. The average score is 5.85 earning this facility seven (7) points in this component. • Quality Point: You have chosen to meet a Programmatic option which is Staff Benefits Package and Infrastructure of Parent Involvement. I verified the Staff Benefits Package, and the Infrastructure of Parent Involvement is being implemented, therefore your program earned 1 Quality Point. • Total: Six (6) points in Education + seven (7) points in Program + (1) Quality Point = 14 = for a five (5) Star License. A new license will be mailed directly to you and must be posted upon receipt. At that time, you will return the old license to me as it remains the property of the State of North Carolina. When stored, the cots were stacked on top of one another with a blanket folded and placed under the sheet in the middle of each cot; the sheets touched the bottom of the cots stacked above. The cots were made of a permeable material, allowing for the potential transfer of germs between the children's bedding and the cots. ERS Requirements: Some attempt was made to practice sanitary nap procedures (ex: children have sheets on the sleeping surface; most cots are at least 18 inches apart, and none are very close together. Observations today: Today I observed the cots had a Ziploc bag containing the blankets. The Ziploc bags are stored in the closet until nap/rest is over and then blankets go back in the Ziploc bag which is stored on top of each cot until use the next day. The NC Secretary of State website was reviewed on 4/11/25 and Ashe Services for Aging Incorporated was listed as current and active. The program’s compliance history was 96% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having free choice center play, preparing for lunch and nap/rest time, infants having individual needs met and time on the rug with teachers. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Foundations embedded in the lesson plans. The last annual compliance visit was conducted on 8/5/24. A sanitation inspection was completed on 11/25/24 with a Superior classification and two (2) demerits. The last fire inspection was conducted on 12/19/24. Program records and required postings were monitored. A fire drill was conducted on 3/26/25 at 3:10 pm with sixty-eight (68) seconds to evacuate thirty-one (31) children and ten (10) adults by K. Absher. A lockdown emergency drill was documented on 2/10/25 at 9:25 am with three (3) minutes and forty-two (42) seconds to lockdown by K. Absher. An outdoor inspection was documented on 3/21/25 at 9:18 am by K. Absher. Two (2) staff files and four (4) children’s records were monitored per DCDEE procedures. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice play on the covered outdoor porches, having gross motor play on the outdoor playground, and having lunch. I observed developmentally appropriate materials and activities were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on 8/15/23. A sanitation inspection was completed 6/6/24 with a Superior classification and two (2) demerits. The last fire inspection was conducted 12/20/23 by Josh Steelman. Program records and required postings were monitored. A fire drill was conducted on 7/24/24 at 10:20 am with sixty-nine (69) seconds to evacuate thirty-five (35) children and eleven (11) adults by K. Absher. A shelter in place drill was documented on 5/24/24 at 3:25 pm with three (3) minutes, fourteen (14) seconds to shelter-in-place. An outdoor inspection was documented on 7/22/24 by K. Absher with no items noted on the playground inspection sheet. Two (2) regular staff files, one (1) new staff file, and four (4) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored for over-the-counter diaper creams and sunscreens. There was on sezuire medication at the facility. The program does not provide transportation. One (1) violation was cited at today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. One child in the 2-3 year old classroom did not have an amount listed on the permission to administer form for sunscreen. 10A NCAC 09 .0803(4)(6-9) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
