Helen Cole, Operator, assisted me with the visit. Your program currently operates with a four star license, issued September 30, 2025. Restrictions are daytime and overnight, capacity of 10 each shift, ages zero to twelve years of age, fireplace not used during hours of operation, serves no more than two infants under one year of age. Each indoor and outdoor space was monitored. Children were observed having free play in centers and interacting with staff. Lunch consisted of baked ham, baked sweet potatoes, cooked apples, wheat bread and milk. There was one violation documented and discussed with Ms. Cole today. Violation Number Comment Rule 1831 A valid qualification letter was not on file and available for review at the facility. One staff person did not have a letter on file. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 9, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Upon arrival Ms. Danielle Dixon, Additional Caregiver was present, and you, Helen Cole, Operator arrived later during the visit. Applicable childcare requirements were discussed with you. All indoor and outdoor areas approved for child care were monitored during today’s visit. Three (3) children between the ages of 3 and 4 years old were present. The children were observed washing their hands before and after eating lunch. After lunch the children were observed resting on individual mats with linens. The children were adequately supervised and appropriate interactions were observed. The following were also monitored during the visit: supervision; capacity; CPR/FA, ITS SIDS; medicine and hazardous product storage; adequate/approved space; staff records; program records; children records; posted license; and permit restrictions. For lunch the children were served chicken nuggets, mixed fruit, French fires, sliced whole wheat bread, and milk. You do patriciate in the food program. During today’s visit you requested to change the name of your FCCH from Helen Cole to Helen Cole’s Child Care. This information was updated in Regulatory, and you were issued a new temporary license effective December 5, 2024 to June 5, 2025, reflecting the name change during the visit. You also returned the old license to me. At the completion of the visit, the Visit Summary was reviewed with you and a computerized copy was given to you. Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. All medication was not kept in locked storage while children were in care. The was corrected during the visit when the medications were placed in locked storage. .1719(a)(5) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. This was corrected during the visit when the items were placed so they were not accessible to children in care. 10A NCAC .1719(a)(28) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. The following were also monitored during the visit: supervision; capacity; CPR/FA, ITS SIDS; medicine and hazardous product storage; adequate/approved space; staff records; program records; children records; posted license; and permit restrictions. For lunch the children were served chicken nuggets, mixed fruit, French fires, sliced whole wheat bread, and milk. You do patriciate in the food program. During today’s visit you requested to change the name of your FCCH from Helen Cole to Helen Cole’s Child Care. This information was updated in Regulatory, and you were issued a new temporary license effective December 5, 2024 to June 5, 2025, reflecting the name change during the visit. You also returned the old license to me. At the completion of the visit, the Visit Summary was reviewed with you and a computerized copy was given to you. Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. All medication was not kept in locked storage while children were in care. The was corrected during the visit when the medications were placed in locked storage. .1719(a)(5) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. The door to the laundry room where detergent was stored was not kept locked when children were in care. This was corrected during the visit when the door was locked. .1719 (a)(7) 729 Appliances with heating elements, such as bottle warmers, crock pots, curling irons, and electrical cords were accessible to children. Items with heating elements in the kitchen and bathroom were accessible to children in care. This was corrected during the visit when the items were placed so they were not accessible to children in care. 10A NCAC .1719(a)(28) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. This was not on file for child A. O. within 30 days of enrollment. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1720 Operator did not give written notice of the written plan of care amendment to parents of all children enrolled, at least 30 days before the change was implemented. Each parent did not sign a statement acknowledging the receipt and explanation of the amendment. The operator did not retain the acknowledgment in the child's record for as long as the child was enrolled in the home and a copy was not maintained on file for review by Division representatives. The amended Written Plan of Care was not given to parents at least 30 days prior to implementing. .1712(f) The violation/s documented above must be corrected immediately. A signed and dated letter of compliance must be mailed to me by April 22, 2025, stating that all violation/s have been corrected. The letter can be emailed to me at leisa.benson@dhhs.nc.gov. If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by April 22, 2025, you must contact me with a purposed timeline of the corrections. In some cases, this timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit, may negatively affect your compliance score. As of today’s, visit your compliance history is 87%. You were also given a copy of your Compliance History Assessment. Rated License: I received your Rated License Assessment Request Review for the FCCERS-R on April 7, 2025. I requested your assessment on April 7, 2025. Keep in mind that I can’t process your star rated license until your assessment has been completed. Once completed they will send me your assessment scores and I will contact you to schedule a visit to review them with you. Also, you will need to complete your three (3) month self-study self-assessment by May 5, 2025. No extension is provided for your temporary license.
