Two infants were found asleep in strollers while the class was outside. Technical assistance was given on how to respond to infants that fall asleep outside. The teachers immediately took the children back inside to transition to a crib. The ratio requirements were in compliance during today's visit. The following violation was cited during todays visit. Your program currently operates with a three-star license, issued September 4, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-two percent as of June 11, 2026. The NC Secretary of State website was reviewed on June 11, 2026, and Christ Way Deliverance Center Inc. was listed as current- active. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and interactions to be in compliance. Space three was over ratio. There were twelve children between the ages of 3-5 years old. The enhanced ratios require a 1:10 staff/child ratio. Children throughout the facility were participating in group time, free play in activity areas, transitions and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. Files for new staff were reviewed. The following violations were cited during today’s inspection. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. Space three was over ratio. The space had 12 children between the ages of 3-5 and one teacher. Alita Hamilton, administrator, assisted me with today’s visit. All indoor and outdoor areas were monitored. Your program currently operates with a three-star license, issued July 26, 2024. Restrictions are first and second shift, capacity of 40 on each shift, ages zero to twelve years of age, children under 2 ½ in room with direct exits only, meets enhanced ratios. The NC Secretary of State website was reviewed on August 19, 2025 and the facility is active under Christ Way Deliverance, INC. The last annual compliance visit was conducted August 30, 2024. The sanitation inspection was completed July 24, 2025 with a “Superior” classification. The last fire inspection was conducted on March 29, 2025 and your facility was approved for day time care only. The children were observed playing outside, toileting, having tummy time and group activities, preparing for and eating lunch. Staff interaction and supervision were appropriate. Lunch today consisted of peanut butter and jelly sandwich, applesauce, carrots and milk. There was one violation documented today. Janet Edwards, Licensing Supervisor, accompanied me on the visit. Jeannie Ollis, co-administrator, assisted me with the visit. Your program currently operates with a three star license issued September 4, 2024. Restrictions are daytime and overnight, ages zero to twelve years of age, children under 2 ½ in classrooms with direct exits only, capacity of 40, meets enhanced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety six percent as of February 24, 2025. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Supervision and staff interaction were appropriate for the children in care. Children were observed having free play in centers, playing outside, toileting, having tummy time and napping, preparing for and eating lunch and resting. Lunch consisted of tuna and crackers, green beans, corn and milk. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violation Number Comment Rule 445 For children under three years of age, developmentally appropriate toys and activities were not provided as outlined in Rule. The classroom for one and two year old children did not have any dolls or blocks. .0510 (e ) (1)(A-G) 535 Any excess formula, thawed human milk and/or bottled beverage were not discarded or returned home at the end of each day. A bottle of milk and a sippy cup of milk were sitting on a shelf after being fed. They were emptied during the visit. 15A NCAC 18A .2804(d) 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. The railings on the ramp leading from the preschool building to the infant toddler building were more than 3 1/2 inches but less than nine inches. .0605(g) 721 All equipment and furnishings were not in good repair. 1. The base of the basketball goal was cracked on one side. 2. There were no handles on a blue tricycle. It was removed during the visit. She asked if the center had to have a diaper change table. I explained a cleanable mat can be placed on the floor and children can be changed on it. The deck outside the classroom for preschoolers leading to the playground is worn and needs to be resealed. The sand around the rock climber on the playground for preschoolers needs to be fluffed on a regular basis. It is recommended the Lil Tykes log cabin be moved away from the playground fence at least six feet to prevent children from climbing over the fence. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. There were four violations documented and discussed today. Violation Number Comment Rule 445 For children under three years of age, developmentally appropriate toys and activities were not provided as outlined in Rule. The classroom for one and two year old children did not have any dolls or blocks. .0510 (e ) (1)(A-G) 535 Any excess formula, thawed human milk and/or bottled beverage were not discarded or returned home at the end of each day. A bottle of milk and a sippy cup of milk were sitting on a shelf after being fed. They were emptied during the visit. 15A NCAC 18A .2804(d) 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. The railings on the ramp leading from the preschool building to the infant toddler building were more than 3 1/2 inches but less than nine inches. .0605(g) 721 All equipment and furnishings were not in good repair. 1. The base of the basketball goal was cracked on one side. 2. There were no handles on a blue tricycle. It was removed during the visit. G.S. 110-91(6); .0601(b) On or before March 11, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Technical Assistance- Mrs. Hamilton asked for assistance regarding diaper changing procedures. She asked if the center had to have a diaper change table. I explained a cleanable mat can be placed on the floor and children can be changed on it. The deck outside the classroom for preschoolers leading to the playground is worn and needs to be resealed. The sand around the rock climber on the playground for preschoolers needs to be fluffed on a regular basis. It is recommended the Lil Tykes log cabin be moved away from the playground fence at least six feet to prevent children from climbing over the fence.
