The center currently operates with a 5-star license, operating at 25 sq. ft. inside, & ratios at .2818 (b). My visit was conducted primarily with Melissa, Assistant Director. A checklist was used to note the requirements monitored. The children were observed eating lunch and preparing for a nap. The center has five (5) classrooms; one (1) is a NC pre-k classroom. The center operates a minimum of 6.5 hours a day, attendance is taken, and USDA approved meals are served. The center uses an approved curriculum. A total of two (2) staff are assigned in the classroom. I observed the SMT. I monitored required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, program records, equipment, furnishings, and medications requirements. VERIFICATION OF RECORDS: *Sanitation inspection=8-2-24 (send me your 2025 inspections) *EPR training was completed by Chandra Barbour on 10-3-16. *The EPR plan was last updated on 6-5-24. The last shelter-in-place was on 12-10-24. The last fire drill was on 1-30-25. The last outdoor playground inspection was on 1-3-25 by Shayla Peterkin. She completed playground inspection training on 9-30-23. *The last water test for lead was on 8-22-24 & will expire 8-22-27. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Little Destiny Literacy Center 2) listed as current-active. Technical Assistance was offered on how to correct the violations cited and how to maintain compliance. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately. Email a corrective action letter BY 3-10-25 to michelle.ezzell@dhhs.nc.gov Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The center's last fire inspection was completed on February 2, 2024. A copy of the 2025 fire inspection must be submitted to me by March 10, 2025. 10A NCAC 09 .0304(a) 612 Each child's bed, cot, or 2" mat was not individually assigned and identified. One infants crib was not labeled with the child's name. 15A NCAC 18A .2821(b) & (c) 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The fire drill for December 2024, was not recorded on the monthly log. .0604(t); .0302(d)(5) *****TECHNICAL ASSISTANCE/REMINDERS: ******* *Fire Drills must be recorded monthly and documented on your monthly log indicating the required information as in rule .0302 (d)(5). We discussed making sure the exact date, including the year, is listed on your log. *Fire Inspections must be completed within 364 days of the last inspection and completed on DCDEE’s form and submitted to your consultant within 5 calendar days of the inspection. *Each child’s crib must be labeled with the child’s name. **Staff and Training Worksheets** *We discussed updating the electronic version of the staff/training worksheets with the corrections made and reviewed with you (Ms. Barbour) during the visit. *****CRIMINAL BACKGROUND CHECKS: *Criminal Background checks can be completed up to 6 months before the expiration date. We discussed your corporate contact, Ms. Shirley Brown’s, CBC will expire March 6, 2025. *I reminded both of you that the ABCMS (criminal record portal) must be current and linked with all your staff’s criminal background information. Emails have been sent to you since February 2024, and I sent a detailed email on 2-4-25. Linking Criminal Background Checks – Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a childcare facility. It is also a way to for staff at the CBC Unit to keep you updated about the status of out-of-state checks if a new employee has a Provisional Qualification. New staff need to link within five days of hire or transfer to your center. Review Criminal History Record Check Requirements for Child Care Centers in the NC Child Care Requirements 10A NCAC 09 .2703. *Remember if more than one teacher works in the classroom with infants and toddlers, each child must be assigned a primary caregiver. *Remember to continue to update your Emergency Preparedness and Response (EPR) Plan every year and print out the cover page showing the current date. Also, print the pages you made changes to. Failure to log onto the website may result in losing your entire EPR plan. Your last annual compliance was on 3-5-24 by K. Wimberly. The center currently operates with a 5-star license, operating at ratios at in rule .2818 (b). My visit was conducted with Chandra, Director and Melissa, Assistant Director. The children were observed participating in teacher directed & free play activities and later eating lunch. The center has one (1) NC Pre-k classroom located in space 5. The teachers observed in this room were Ms. Toyia T. & Zhanee E., a substitute teacher. One NC pre-k teacher recently left, and the facility is in transition for another teacher. I monitored for required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, program records, equipment, furnishings, and medications. Program files were monitored, and staff files were spot checked for compliance. VERIFICATION OF RECORDS: *Last fire inspection=2-2-24 (send me your 2025 inspection within 5 days of completion) *Sanitation inspection=8-2-24 (send me all your 2024 inspections upon completion) *The lead water test was on 10-5-22 & will expire on 10-5-25. *The last outdoor inspection was on 8-29-24. The last fire drill was on 8-24-24 & lockdown drill was on 6-21-24. *EPR training was completed by Chandra Barbour (Trainer) on 10-3-16. *The EPR plan was last updated on 6-5-24. *I reviewed the NC Secretary of State’s website and observed the owner of the facility Little Destiny Literacy Center, Inc. listed as current-active. One violation was cited and corrected during the visit. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence). Feeding schedules were located in a notebook in the classrooms for infant and toddlers. These feeding schedules were posted during the visit. 10A NCAC 09 .0902(a) *We discussed how to make corrections on the electronic version of your staff/training worksheets. *We discussed which items must be posted, so they are visible at a glance. *Remember CPR & 1st Aid can only count toward on-going training hours if the staff person needs 20 training hours. *We discussed putting the child’s 1st name and last initial on the bottles vs. 1st name only. *The person completing the playground inspections must have the playground safety training. *Feeding schedules must be posted vs. in a notebook. We discussed different ways to post feeding schedules. This violation was corrected during the visit. *Remember to continue to update your Emergency Preparedness and Response (EPR) Plan every year and print out the cover page showing the current date. Today’s visit was conducted with Melissa Bullard, Assistant Director. Chandra Barbour, Director was present during my arrival but did have to leave per an appointment. I reviewed the NC Secretary of State’s website and the owner of the facility, Little Destiny Literacy Center, Inc. is listed as current-active as of 11-28-23. Program requirements The following items were observe posted: first aid poster, license, no smoking sign, summary of the law and emergency care plan. The indoor environment was monitored for safety and no items of non compliance were observed. Daily arrival and departure times of the children are documented on an electronic keypad as the children enter the center except for the NC PreK children who sign in and out on a clipboard at the entrance to the classroom. All space used by the children were adequately heated and cooled. An incident log is stored in the Director’s office. The last fire inspection was conducted on 1-30-23. I would go ahead and reach out to your fire inspector to have the new one done as the holidays does cause delay. The last Sanitation inspection was conducted on 3-24-23. The last documented outdoor playground inspection was October 2023. Fire drills and EPR drills were current and on file. Staff Records: I monitored all new staff files who were hired since the last monitoring visit. It was observed that 1 new staff member did not have a criminal background qualification letter on file. Ms. Bullard stated that the employee has been having issues getting into the CBC database to print out her letter. I discussed with Ms. Bullard that employees must have a paper copy of their letter prior to beginning their first day of work at the center and this paper copy must be on file at the center. I did attempt to log into the CBC database during my visit but it appears the database is down. Due to the fact that it could not be verified if the staff member has a qualification letter, they will need to leave the center at the end of the day and may not return until one has been obtained and placed on file at the center. I also observed that one staff member with a date of employment 8-22-22, has not completed all of the required Health & Safety trainings. Once an employee begins employment, they have 1 year from the date of employment to completed these trainings. I also observed that 1 staff did not have a current completion certificates for Recognizing and Responding to Child Maltreatment. One existing staff member did not renew their training before expiration. Staff/Child Ratios and Supervision: During today's visit I observed the children playing in centers and some were having circle time. Some children were also outside playing. The Infants were being fed and having tummy time. Sleeping baby checks were monitored and observed to be in compliance. Supervision was observed being maintained in all classrooms. Medications and Hazardous Products: All hazardous products were either locked up or placed out of reach of the children. The center only allows emergency medications and Ms. Bullard stated that there are no children at this time on emergency medications. Three items of non compliance regarding staff records was observed and must be corrected. A correction letter stating how each violation was corrected is due to me no later than December 12, 2023. The letter can be mailed or e-mailed to me at the address at the bottom of this report. As stated above the staff member without the qualification letter will need to leave the premises at the end of the day and may not return back until the qualification is on file. Violation Number Comment Rule 1757 A valid qualification letter was not on file and available to review at the facility. One staff member with a date of hire of 11-27-23 did not have a criminal background qualification letter on file. G.S. 110-90.2(b) & (d) & .2703(e) 1898 Staff did not complete the health and safety training within one year of employment. Documentation of completion of Health & Safety training was not current/on file for 2 staff. .1102(a) We did discuss today that the center does desire to have the ERS conducted as soon as possible. I discussed with Ms. Bullard that I have reached out to NCRLAP during my visit to see what their timeframe is for scales for programs not in cohorts and I will let them know once I hear back from NCRLAP.
