Today’s visit was conducted with Shakiela O’Neal, Lead Teacher. The PreK program is housed within Lake Myra Elementary School. The Principal/Administrator of the school is Nancy Ballard. Program requirements The following items were observe posted: License, Safe Arrival and Departure Procedures, NC Summary of the Law, Emergency Medical Care Plan, Daily Schedule, Activity Plan, Staff/child ratios, Allergies, Menu, First Aid chart, Fire evacuation route, hand washing signs, and cot chart. The indoor environment was monitored for safety and no items of non compliance were observed. Attendance records were current and on file indicating arrival and departure times. All space used by the children were adequately heated and cooled. An incident log is stored in the teacher’s office in a notebook. The last fire inspection conducted at the center was on 6-7-23. The last Sanitation inspection was conducted on 4-24-24 and a Superior rating was awarded with 4 demerits. The last documented lead water test was conducted on 4-29-21. Fire drills and outdoor playground inspections were current and on file. I did observe that the last documented EPR drill was 10-30-23. This is a violation of child care requirements. Please remember that EPR drills which include shelter-in-place or lockdown must be conducted every 3 months and documented for proof of them being conducted. Staff Records: During my visit today, I observed 3 staff caring for the children including the Lead Teacher, Instructional Assistant and a substitute. Ms. O’Neal stated that the previous IA resigned and there is a new one in place for the remainder of the school year. There was also a Wake County representative present and Ms. O’Neal stated he was a Training Transition Facilitator. I reviewed paperwork on file for the newly placed IA and all paperwork appeared to be on file. Criminal background check qualifying letters were current and on file for Ms. O’Neal as well as the IA but I did not see one for the substitute (L. Morgan) who was present caring for the children. I did verify in the CBC database that a current criminal background qualifying letter has not been obtained therefore Ms. Morgan will need to leave the school and may not return until one has been obtained. Staff/Child Ratios and Supervision: During today's visit I observed the children participating in circle time then transitioned outside to play. Supervision was observed being maintained. Two violations regarding Criminal Background checks and program records were observed during my visit today. A corrective action letter is due to me no later than June 4, 2024. The Substitute (L. Morgan) present in the classroom will be unable to return to the PreK classroom until a criminal record qualification letter issued by DCDEE has been obtained and is on file and must leave at the end of the business day today, 5-21-24. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. One staff member present in the classroom as a substitute did not have a criminal background check qualifying letter qualifying them to work in child care. G.S. 110-90.2(b) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented EPR drill was 10-30-23. .0604(u);.0302(d)(8) Reminders: *A new CBC qualification letter will need to be obtained for S. O'Neal no later than 12-17-24. *Please complete the Staff/Training worksheet that I left with you today for the new staff and return to me.
