Today’s visit was conducted with Lorraine Dixon, Owner/Administrator. I reviewed the NC Secretary of State’s website and the owner of the facility, ABC Land, Inc. is listed as current-active as of 4-23-25. Program requirements The following items were observe posted: first aid poster, license, no smoking sign, summary of the law and emergency care plan. The indoor environment was monitored for safety and no items of non compliance were observed. Daily arrival and departure times of the children are documented on an electronic program as the children enter the center. All space used by the children were adequately heated and cooled. An incident log is stored in the Director’s office. The last fire inspection was conducted on 11-13-24. The last Sanitation inspection was conducted on 1-30-25. The last documented outdoor playground inspection was April 2025. Fire drills and EPR drills were current and on file. The last fire drill was conducted on 3-28-25. The last EPR drill was conducted on 1-30-25. Remember that another one must be conducted prior to 4-30-25 in order to remain in compliance. Staff Records: I monitored one new staff file who was hired since the last annual visit. Criminal background checks were also monitored for all staff and were observed to be in compliance. Staff/Child Ratios and Supervision: During todays visit I observed two classrooms playing outside and two classrooms were getting up from nap time. The only ages which are cared for at this site are ages four, five and school age. During my walk-through today, I observed one staff member inside coming from a closet area and then returned back outside where the children were playing. During the period of time that the staff member was inside the building, one teacher was left outside with 5-four year old children and 11-five year old children. This is a violation of staff/child ratios. I discussed with the staff member as a reminder that when breaks are needed, another staff member must be called for classroom coverage in order to remain in compliance. This violation was corrected during the visit. Medications and Hazardous Products: All hazardous products were either locked up or placed out of reach of the children. One item of non compliance pertaining to staff/child ratios was observed out of compliance. Another visit will be made at a later date to verify that staff/child ratios are being maintained. A corrective action letter stating what has been put into place to assure that staff/child ratios are being maintained as required for your rated license is due to me no later than May 7, 2025. The letter can be mailed or e-mailed to me at the address at the bottom of this report. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. Because younger children need more direct one-on-one interaction, response, and supervision, staff-to-child ratios are lower for younger children than for older ones. Ratio and group size are two factors that are critical to a child’s health, safety, and development. Ratios and group sizes help ensure that a child gets enough one-on-one attention from an adult who is available to take care of each child’s unique needs. This responsive caregiving is extremely important to a child’s social and emotional development, physical well-being, and overall learning. Name of Operation: ABC LAND, INC II Facility ID: 92001651 Consultant: KIMBERLY WIMBERLY Operation Type: Center Case Number: Visit Date: 7/29/2024 Number Present: 25 Completed Date: 7/29/2024 Age: From 5 To 11 Total Minutes: 120 Time In: 09:30 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to inspect the facility for compliance with child care requirements during your annual compliance visit. The last annual compliance was conducted on 8-7-23. The center currently operates with a 5 STAR rated license. The center is approved for 114 children ages 3-12. My visit was conducted with Lorraine Dixon, Owner/Director. During today’s visit an assessment of Child Care Requirements was conducted, inspecting all space used by children. I verified on the NC Secretary of State website that the owner of the program, ABC Land, Inc. is listed as current-active as of 7-29-24. Upon arrival to the program today, the children were observed playing on the outside playground. During the summer months, the program only operates for school age children as the NC PreK classrooms are out for the summer There are six (6) classrooms approved for care of children but only two of the six classrooms are being used during the summer months. The center has recently been repainted on the inside so Ms. Dixon stated the walls have to dry a few more days until things can be posted but I did observe that there is a cork board leaning up against the walls which contains all the required information. While monitoring the Kitchen, I did observe that current allergies were not posted. Ms. Dixon printed a new copy and this violation was corrected during the visit. The center does allow medications but there is only one child on a medication at this time and that medication is in a locked cabinet in the classroom. Ms. Dixon stated that there are no children on emergency medications at this time. All hazardous items were either locked up or placed out of reach of the children. For outside time, the children play in fenced areas. The following items were observed out of compliance on the outdoor playground. Ms. Dixon stated that the person who cuts the playgrounds is scheduled to come out this week to handle all of the items which were discussed. 1. The trike tracks had a lot of the rubber mulch on them which had washed out of the mulched areas during the recent heavy storms. During my visit, a child was running from one of the covered areas to another area and slipped on the rubber mulch pieces and fell down. No injuries were observed but the trike tracks must be swept clean. 2. There is an arch with metal panels which has blackberry bushes growing on it. The area where the blackberries are were observed to need to be pruned back. The weeds and grass had grown up around the bushes and panels which could promote vermin to hang out. You will also need to secure the blackberry bushes to the panels a little tighter because of the prickly thorns. This will help keep the children from getting scratched as they go under the archway. 3. I also observed large mushrooms growing underneath the bushy blackberry vines. As we discussed, if ingested, this could be hazardous to the children. 4. There were other patches of overgrowth on the outside playground which will give snakes and vermin places to hide and not be seen. Staff and training worksheets were not submitted to me today due to the fact Ms. Dixon stated that she was not finished completing them. There have been no new staff since my last visit to the center in November 2023. A spot check was completed on two staff files and were observed to be in compliance. As we discussed, please send the Staff and Training worksheets to me no later than Friday, August 1, 2024 at 5pm. Ten children's files were monitored and observed to be in compliance. I inspected the facilities posted information such as the license, EMC plan, emergency telephone numbers, staff/ratio sheets, allergy lists, etc. and all were observed to be in compliance. Fire drills, EPR drills and outdoor playground inspections were all current and on file. I reminded Ms. Dixon that a fire drill and an EPR drill will need to be conducted prior to 7-31-24 in order to remain in compliance. Fire and sanitation inspections were also current and on file. The EPR Plan was also current and on file. There are two transportation mini buses that the center uses for transportation of the school age children. Both buses were monitored during my visit today. We discussed that two seats on the bus with license plate XVC-7806 need some TLC before the seat gets to the point it cannot be repaired. I did observe one of the seat belts in the very back of the bus had a latch missing on one of the seat belts. We did discuss that since the program is only caring for school age children during the summer months, they can use the other mini bus until the seat belt can be repaired which can be done this week. Both mini buses had current license plates and liability insurance. *****Violations: Two violations regarding safe environment and program records were observed out of compliance during my visit today. A corrective action letter stating how the violation which was not corrected during the visit was corrected is due to me no later than August 12, 2024 and can be e-mailed or mailed to me at the contact information at the end of this report. Violation Number Comment Rule 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. An allergy list was not posted in the food prep area. .0901(g) 807 A safe indoor and outdoor environment was not provided for the children. The outside playground had multiple areas where the grass/weeds were grown up and need to be mowed and trimmed down. There were mushrooms observed growing on the outside playground in an area that had overgrowth. The trike track had a lot of the rubber mulch which had washed away from mulched areas during recent storms. The last annual compliance was conducted on 8-11-22. The center currently operates with a 5 STAR rated license. The center is approved for 78 children ages 3-12. My visit was conducted with Lorraine Dixon, Owner/Director. During today’s visit an assessment of Child Care Requirements was conducted, inspecting all space used by children. Upon arrival to the program today, the children were observed participating in various activities and some were outside playing. All equipment and furnishings were child size and developmentally appropriate. All hazardous items were locked up or stored out of reach of the children. The incident log is stored at the front desk. For outside time, the children play in fenced areas. I did observe today that a small section of the outside fence was missing. Ms. Dixon stated that during the construction of the additional outdoor playground, a portion of the fence had been removed temporarily. We discussed today that it is a safety concern for the fence to have a section missing even temporarily. Ms. Dixon did have the fence temporarily fixed during my visit but a permanent solution must be completed. We also discussed that you need to rake the existing mulch to remeasure so that you can assure the correct depths are being met. Ms. Dixon also ordered more mulch during my visit today. Staff and training worksheets were completed by Director and submitted to me during today's visit. All new staff files were monitored for compliance. Children's files were monitored during my visit and observed to be in compliance. I inspected the facilities posted information such as the license, EMC plan, emergency telephone numbers, staff/ratio sheets, allergy lists, etc. and all were observed to be in compliance. Fire drills, EPR drills and outdoor playground inspections were all current and on file. Fire and sanitation inspections were also current and on file. The EPR Plan was also current and on file. *****Violations: One violation of child care requirements regarding fencing was observed and was corrected during the visit with a temporary fence which did meet the 4 foot height requirement. As an additional safety precaution I would like children to be kept away from this part of the outdoor environment until a permanent fence is back in place. Violation Number Comment Rule 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. A small section of the outside playground fence was missing during my visit. GS 110-91(6); .0605((i) As we discussed today, before the new space can be used, I will need to stop by once the furniture has been moved in the classrooms have been set up. Approved Building, Fire and Sanitation inspections will also need to be submitted to me.
