The facility currently operates with a Notice of Compliance that was issued on March 28, 2012. The center's last Sanitation Inspection was conducted on November 4, 2025, and received zero (0) demerits and a “Superior" classification. The last Fire Inspection was conducted on May 5, 2025, and was approved for daytime care only. The last Fire Drill was conducted on December 17, 2025. The last Lock Down/ Shelter-in-Place was on December 17, 2025. The last Playground Inspection was conducted on December 18, 2025. Upon arrival, I was greeted by the administrator, Lauren Vick. I stated the purpose of my visit and was assisted on a walkthrough of the facility, where attendance was documented, and observations made. The children were observed participating in routines and indoor free play and teacher directed instruction. Today, a partial assessment of applicable child care requirements was conducted, which included license posted, permit restrictions, supervision, staff/child ratio, qualified criminal background checks, staff training and general safety. The following violations were observed during today’s visit and technical assistance was offered. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In classroom 214 there is a child with Asthma without a signed permission form to administer medication. In classroom 114 there is a child with an EPI pen and Benadryl with an expired permission form as of 8/10/25. 10A NCAC 09 .0803(1)(a & b) 871 Center staff did not comply with the safe sleep policy. In the infant classroom 203, there were two infants in a crib sitting in an infant seat. All infants must be placed in their cribs to sleep with nothing else in the crib. 10A NCAC 09 .0606(a) Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov. If a letter of compliance is not received in my office before 5:00 PM February 2, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Technical Assistance: Today we discussed that all Infants once they fall asleep must be placed in their cribs. All infants should be placed on their backs until they can roll over by themselves. Nothing can be kept in the cribs. No carriers or infant seats. All asthma medication forms must be completed and have a recording document with it. If the asthma medication is dispersed it must be recorded. All medication cannot be administered without a permission form signed by the parent. They must have a start and ending date on them. The last Annual Compliance visit was conducted on September 30, 2024. This facility currently operates with a Notice of Compliance that was issued on March 28, 2012. The last Sanitation Inspection was completed on February 18, 2025, with a "Superior" classification and eleven (11) demerits. The last Fire Inspection was conducted on April 5, 2025, and the facility was approved for daytime care only. Permit Restrictions: This facility can only care for children during the first shift. This facility can only care for children ages 0-6. This facility does not provide transportation. During today’s visit all medications, permission forms and medical action plans were monitored. The last Fire Drill was conducted on July 28, 2025. The last Lock Down/ Shelter-in-Place was on June 5, 2025. The last Playground Inspection was conducted on June 16, 2025. Upon arrival, I was greeted by the director, Lauren Vick. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The children were observed participating in routines and indoor free play. Infants were being held and resting, toddlers were outside playing, preschoolers were listening and engaging in teacher directed activities. A checklist was used to note the requirements I monitored today. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, and Playground Inspections. The following violations were observed during today’s visit and technical assistance was offered. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. In space 207 there was a soap dispenser that was sitting on top of the back of a toilet. This was a wall mounted dispenser. G.S. 110-91(6); .0601(b) 807 A safe indoor and outdoor environment was not provided for the children. In space 209 there was a fan on children's level that posed a hazard to children. Outside on the playground area the grass was very tall in several places for snakes to hide. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space 209 there was a spray bottle of hair detangler in a unlocked cabinet with a "keep out of children's reach" warning on it. .2820(b) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. This fan did have a guard on it, but the holes were just large enough for a child to place their fingers through. In this same space there was a spray bottle of hair detangler in an unlocked cabinet. This did have a warning on it to keep out of children’s reach. On the playground today there were several spots with very tall grass. The director stated that the “Ark” climbing structure was removed and they found snake eggs in it. Tall grass is one area snakes love to hide in, therefore grass needs to be maintained where children play. The director stated that the water leak was repaired but the tiles were not. The director stated that their maintenance person was coming to replace them. Reminders: T. Easter’s ITS-SIDS expires in October 2024. A. Hamilton’s ITS-SIDS has expired in April of 2024 however this person is not in the infant room. A. Hamilton’s Criminal background check will expire this December 2024. D. Coleman’s criminal background check has expired but this person is a substitute and has not been here.
