Tammy Rigdon, operator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Rigdon. Limited monitoring of child care requirements occurred during today’s visit. The following violation was confirmed during today’s visit. Violation Number Comment Rule 1409 Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. On April 14, 2025, the operator failed to provide adequate supervision, which created an unsafe environment for a one-year-old child and placed the child at risk of harm. 10A NCAC 09 .1719 (a) 1852 There was a substantiation of child maltreatment. On May 6, 2025, the Division determined child maltreatment based on the supervision needs of children. 110-105.6(a) 1938 Operator did not provide supervision according the individual age, needs, and capabilities of each child. On April 14, 2025, the operator was unaware when a one-year-old child left the fenced in area and walked to the home next door. .1711(a)(4)(B) Violations must be corrected immediately.
