Michelle O’Kelley, Child Care Consultant, accompanied me during today’s visit. Your program currently operates with a three-star license, issued 9/9/22, earning 4 points in the education component, 2 points in the program standards component and 1 quality point for serving no more than 2 children under one year of age. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed during today’s visit. It is important to frequently check your email to remain informed about any new changes that have or may occur. The last annual compliance visit was conducted 12/11/23. The family childcare home’s compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent as of 11/8/24. The NC Secretary of State website was reviewed on 11/8/24 and this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding ownership of this facility. A walk-through of the family child care home was completed today, all indoor and outdoor areas were monitored. No children were present. Activity plan was posted. The fenced outdoor play area in the back yard consisted of portable play equipment, ride-on toys, and an area for shade. The following violations were documented: Violation Number Comment Rule 1897 Application did not have an a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. A child’s physical reviewed today stated the child has a peanut allergy and it is recommended to have an EPI pen. There was no EPI pen, medication authorization, and medical action plan available for review. .1721 (a )(4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 11/26/24, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Diaper changing procedures were observed. The provider talked to the infant as she prepared the diaper changing table. The outdoor play area in the back yard provided additional space and materials to promote physical development and gross motor skills. The following violations were documented. Your program currently operates with a three-star license, issued 9/9/22, earning 4 points in the education component, 2 points in the program standards component and 1 quality point for serving no more than 2 children under one year of age. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed during today’s visit and an update was made to the email address listed on the owner screen. It is important to frequently check your email to remain informed about any new changes that have or may occur. The last annual compliance visit was conducted 1/10/23. The family childcare home’s compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 7/11/23. A walk-through of the family child care home was completed today, all indoor and outdoor areas were monitored. One (1) infant and the provider were present when I arrived. Diaper changing procedures were observed. The provider talked to the infant as she prepared the diaper changing table. The outdoor play area in the back yard provided additional space and materials to promote physical development and gross motor skills. The following violations were documented. A child enrolled in March 2023 did not have a signed and dated discipline policy attesting a copy was received by the parent/guardian. G.S.110-91(10); .1727(a)&(b) 1704 Summary of the NC Child Care Law was not given to each child's parent, guardian, or full-time custodian before the child was enrolled in the home and/or signed statement was not on file. A child enrolled in March 2023 did not have documentation attesting the summary of Child Care law was received. GS 110-102 1895 Application did not include health care needs or concerns, symptoms of and the type of response required for the health care needs or concerns. The health care needs section of the enrollment application for the infant enrolled in March was blank. .1721 (a ) (3)(D) 2030 Operator did not provide a written statement to parents regarding the smoking and tobacco restriction. A child enrolled in March 2023 did not have documentation of a written statement regarding smoking and tobacco restrictions received by the parent/guardian. .1719(a)(11) * Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 7/28/23, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 TECHNICAL ASSISTANCE WITH THE DOCUMENTED VIOLATION (S): Children's Records - Maintaining accurate documentation at the child care center is important because it demonstrates compliance with the child care rules and ensures the health and safety of each child. Children's records are required to be completed and maintained for each child who attends your child care (full-time, part-time, or drop-in). In the event of an emergency, child care providers must know about a child’s special health care needs, so they can offer protection for that child. This requires you to obtain the health and safety needs of individual children on the application prior to enrollment. The health care needs section of the infant enrolled in March was blank. Additionally, the hospital preference and telephone number section of the application was blank. The first day a child attends there must be documentation of the receipt of the child care’s discipline policy, documentation of the receipt of the NC Summary of Child Care Law and a signed notification of smoking and tobacco restrictions. There was no documentation verifying the receipt of the items in the file for a child enrolled in March 0f 2023. This equivalency exam assesses the foundations of culturally responsive, equitable and inclusive early childhood education, planning intentional developmentally appropriate experiences, learning activities, and teaching strategies for indoor and outdoor environments for all young children, guidance techniques, and professionalism.
