This program currently operates with a Temporary License issued November 25, 2025. The temporary license is valid through May 25, 2026. This facility is owned by Eagle’s Nest Early Learning Center & Preschool INC. The NC Secretary of State website was reviewed on May 1, 2026, and was listed as current- active. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed, and the facility telephone number was updated. It is important to frequently check your email to remain informed about any new changes that have or may occur. Administrator Information - The information for the 2nd administrator was updated during the visit. K. Sherman will now serve as the legal designee and administrative assistant for the facility. N. Smith, administrator, accompanied me during today’s walk through. Children were engaged in various activities throughout the facility. It was also picture day. One staff person accompanied the children and remained in the space until completed. Lunch included sloppy joe sandwiches, baked beans, potatoes, ketchup, and milk. The fenced outdoor spaces located at the rear of the facility include both anchored and portable play equipment as well as a shaded play space. Staff, children, and program records were monitored. Inspections - A sanitation inspection completed on 12/22/25 was submitted on 1/5/26. A fire inspection was completed and received on 3/26/26. A building inspection was completed on 2/12/26 and received on 4/17/26. Lead and Asbestos Testing Requirements - All North Carolina child care facilities are required to test for lead in water used for drinking or cooking, inspect facilities for lead-based paint hazards, and inspect facilities for asbestos hazards. I reviewed the Facility Data Mapper through the Clean Classrooms for Carolina Kids website. Based on the information reviewed, lead in water results were completed on 7/19/23. An exemption was granted for lead -based paint and asbestos to Eagle’s Nest ELC & Preschool on 4/17/26. Please be advised that lead in water testing is current for 3 years; therefore, testing must be completed before 7/19/26. Transportation Requirements - You indicated that you want to utilize a transportation service for field trips during the summer. We reviewed Section .1000 Transportation requirements during today's visit. Compliance History - Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The compliance history for this facility prior to today’s visit is 89%. The violation(s) documented may impact the compliance history score and must be corrected immediately. First aid skills are the most likely tools caregivers will need. Minor injuries are common. For emergency situations that require attention from a health professional, first aid procedures can be used to control the situation until a health professional can provide definitive care. An individual hired on 5/15/26 has not completed CPR & First Aid training within 90 days of employment. You indicated the individual completed it in another state and was unable to obtain a copy and you forgot to follow-up with the individual. You contacted an approved trainer during the visit and scheduled the training (5/11/26). The knowledge and ability to perform emergency measures on another person whose breathing or heart activity has stopped is critical to potentially saving a life. Documentation of the successful completion of pediatric first aid and demonstration of pediatric CPR skills is required by all staff within 90 days of employment. Additional Comments: Rated License Assessment – You have chosen to apply for Pathway 2 – Classroom and Instructional Quality. We reviewed the Application for Assessment for a Rated License for Centers, and all supporting documentation. Education Standards - The Staff Information and Education worksheet was completed by you on 4/15/26. I verified each individual identified through DCDEE WORKS evaluations. At 50% in both the lead teacher and other columns you are meeting the 5-Star Level in this component. Once your facility is meeting compliance with all violations documented today, a 5-Star license will be issued. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The children and teacher in Space 5 were completing handwashing routines in preparation for lunch. The children and teacher in Space 5 were playing Bingo before transitioning to lunch. Lunch included sausage, potatoes, apple sauce, a biscuit and milk. A medical report was not on file for one staff person. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Neither a TB test or screening was on file for one staff person. .0701(a) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Each child shall have a health assessment before being admitted or within 30 days following admission to a child care facility. One child enrolled on 12/2/25, did have documentation of a medical report on file; however, the medical report was dated 6/12/23. The parent(s) must provide you with a medical report that is current within the last 12 months. If the parent does not provide documentation by the compliance letter due date, you may no longer care for the child until the documentation is provided. Immunization Record - Routine immunizations at the appropriate age are the best means of protecting children against vaccine-preventable diseases. According to Caring for Our Children, immunization is particularly important for children in child care because preschool-aged children have the highest age-specific incidence or are at high risk of complications from many vaccine-preventable diseases. Each child shall have a current immunization record before being admitted or within 30 days following admission to a child care facility. One preschool child enrolled on 12/2/25, does have an immunization record on file dated 12/29/23. The immunization record on file is older than 12 months prior to the enrollment date. The parent(s) must provide an immunization record that is current within the last 12 months. If the parent does not provide documentation by the compliance letter due date, you may no longer care for the child until the documentation is provided. Medications - Administering medication requires skill, knowledge and careful attention to detail. The medications stored at your facility must be reviewed regularly to ensure they have not expired. Authorization periods for administering emergency medications such as epi-pens are valid for six months at a time. This was discussed with you because the 6-month authorization to administer emergency medication was not on file for one child with a medical action plan and medication on premises. Please have the parent complete medication authorization form and verify to me in writing that the form was updated within two weeks of today's visit. Create a plan to review medications, permission forms, and medical action plans to ensure they are current and accurate. Electrical Outlets- Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. For this reason, all electrical outlets, including those on power strips, not in use that are in areas accessible to children must be protected with a safety plug or a tamper-resistant electrical outlet. In space 104, two open outlets were observed. You did not indicate why. Taping a reminder on the wall over each socket and power stick can serve as a reminder to add the safety cover when not in use or have the wall plate on the socket replaced with a tamper resistant electrical outlet. Staff Records: The health and safety of children requires employment documents for each staff member working with children meet the qualifications and are readily available. Staff records consist of various documentation to include information needed prior to hire and within certain timeframes after hire. Documentation of a health questionnaire, medical clearance and a negative TB result were not on file for N. Smith, administrator. You did not indicate why. It is important to utilize the staff file checklist to ensure all files have required documents. CPR/ FA - To ensure the health and safety of children in a child care setting, staff qualified to respond to life-threatening emergencies must be in attendance at all times. All staff must be trained in pediatric first aid, including pediatric CPR, coupled with a facility that has been designed to ensure the safety of children, can mitigate the consequences of injury, and reduce the potential for death from life-threatening conditions. Documentation of the completion of CPR/FA training is not on file for any individual hired on 1/21/25. Knowledge of pediatric first aid, including pediatric CPR which addresses management of a blocked airway and rescue breathing, and the confidence to use these skills, are critically important to the outcome of an emergency situation. Documentation of the completion must be submitted on or before 2/19/26. Additional Comments: Record keeping is one of the most time-consuming tasks associated with managing a child care business. Maintaining complete and accurate records for the program, staff and children is a vital part of being an administrator. Setting aside regularly scheduled times each week to review records can assist you in keeping up with the task. A tracking and reminder systems are also essential to keep track of tasks, due dates, and expiration dates for both staff and children. Per our discussion, you must set aside time to review files to ensure forms are complete. Checklists for children’s files can be found under provider documents on the Division’s website. Placing a copy in each file will help you to see that you have all the required information on file. Being proactive with an organized approach to record-keeping helps you ensure easy access to information when needed, especially in emergency situations. Administrator Qualifications - Within six months of an individual assuming child care administrator duties, you must have a copy of the credential certificate. If the individual does not have a credential, they must provide a copy of notification from the Division that they meet the equivalency or that they do not meet the equivalency and must enroll in coursework. If the individual does not have a credential or equivalency they must provide documentation of enrollment in credential coursework. K. Sherman, administrator, hired on 11/22/25 indicated she is currently enrolled in EDU 261 with an expectation to complete the course in March 2026. Rated License - Discussions occurred today regarding the Pathways to the Stars. You indicated plans to apply for a rated license under Pathway 2 (Classroom and Instructional Quality). We also discussed the immediate need to have all education evaluated through WORKS for all staff. This facility is owned by Eagle’s Nest Early Learning Center & Preschool INC. The NC Secretary of State website was reviewed on February 4, 2026, and was listed as current- active. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed, and the facility telephone number was updated. It is important to frequently check your email to remain informed about any new changes that have or may occur. The children in Space 1 were engaged in indoor play. One of the teachers was on the floor in the block area, building a structure with the children. The other teacher was engaged in pretend play with a child in the dramatic play area. The children and teacher in Space 5 were completing handwashing routines in preparation for lunch. The children and teacher in Space 5 were playing Bingo before transitioning to lunch. Lunch included sausage, potatoes, apple sauce, a biscuit and milk. The fenced outdoor spaces located at the rear of the facility include both anchored and portable play equipment as well as a shaded play space. Staff, children, and program records were monitored. Inspections - A sanitation inspection completed on 12/22/25 was submitted on 1/5/26. You must submit an approved building and fire inspection by February 28, 2026. Lead and Asbestos Testing Requirements - All North Carolina child care facilities are required to test for lead in water used for drinking or cooking, inspect facilities for lead-based paint hazards, and inspect facilities for asbestos hazards. I reviewed the Facility Data Mapper through the Clean Classrooms for Carolina Kids website. Based on the information reviewed, lead in water results were completed on 7/19/23; an exemption was granted for lead -based paint and on 10/3/24. Please be advised that lead in water testing are current for 3 years; therefore, testing must be completed before 7/19/26. Additionally, the address of the facility is listed under the name of the previous program. Authorization periods for administering emergency medications such as epi-pens are valid for six months at a time. This was discussed with you because the 6-month authorization to administer emergency medication was not on file for one child with a medical action plan and medication on premises. Please have the parent complete medication authorization form and verify to me in writing that the form was updated within two weeks of today's visit. Create a plan to review medications, permission forms, and medical action plans to ensure they are current and accurate. Electrical Outlets- Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. For this reason, all electrical outlets, including those on power strips, not in use that are in areas accessible to children must be protected with a safety plug or a tamper-resistant electrical outlet. In space 104, two open outlets were observed. You did not indicate why. Taping a reminder on the wall over each socket and power stick can serve as a reminder to add the safety cover when not in use or have the wall plate on the socket replaced with a tamper resistant electrical outlet. Staff Records: The health and safety of children requires employment documents for each staff member working with children meet the qualifications and are readily available. Staff records consist of various documentation to include information needed prior to hire and within certain timeframes after hire. Documentation of a health questionnaire, medical clearance and a negative TB result were not on file for N. Smith, administrator. You did not indicate why. It is important to utilize the staff file checklist to ensure all files have required documents. CPR/ FA - To ensure the health and safety of children in a child care setting, staff qualified to respond to life-threatening emergencies must be in attendance at all times. All staff must be trained in pediatric first aid, including pediatric CPR, coupled with a facility that has been designed to ensure the safety of children, can mitigate the consequences of injury, and reduce the potential for death from life-threatening conditions. Documentation of the completion of CPR/FA training is not on file for any individual hired on 1/21/25. Knowledge of pediatric first aid, including pediatric CPR which addresses management of a blocked airway and rescue breathing, and the confidence to use these skills, are critically important to the outcome of an emergency situation. Maintaining complete and accurate records for the program, staff and children is a vital part of being an administrator. A tracking and reminder systems are also essential to keep track of tasks, due dates, and expiration dates for both staff and children. Per our discussion, you must set aside time to review files to ensure forms are complete. Checklists for children’s files can be found under provider documents on the Division’s website. Placing a copy in each file will help you to see that you have all the required information on file. Being proactive with an organized approach to record-keeping helps you ensure easy access to information when needed, especially in emergency situations. The WORKS portal is available online and requires the user to create and maintain an individual North Carolina Identity Management (NCID) username and password. Electronic official transcripts can be submitted to the Workforce Education Unit from a person’s college or university provided the school participates with the National Clearing House. If not, then official transcripts must be mailed to the Workforce Education Unit at DCDEE for evaluation. All staff education should be current within four months of the six month temporary time period (March 2026).
