Currently, this facility operates with a Three-Star license issued on February 12, 2020. The center's last annual compliance visit was completed on September 10, 2024. A one-page summary was left at the completion of the visit due to connectivity issues. A checklist was used to note the requirements monitored during the visit. Valerie Pinto, Owner/Operator, was present and available to assist. Two children were present during the visit. Children were observed in free choice activities and during lunch. The caregiver interacted with children in a positive manner. Playground inspections, fire and lockdown drills were all current. You, V. Pinto, are the sole proprietor listed on the facility license. Prior to changing the type of business operation, you should notify the consultant to ensure all procedures are completed and compliance is maintained. The following violations of child care requirements were documented during the visit. A child did not have documentation of their immunization record on file. The ABCMS provider portal training had not been completed. G.S. 110-90.2 & .2703(r) 2019 Operator did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within the required time frame. The operator did not complete the recognizing and responding to suspicions of child maltreatment training within five years of the previous training dated February 2020. .1703(a)(5) 2031 Operator did not provide a copy of the shaken baby syndrome and abusive head trauma policy to parents at time of enrollment, and / or within fourteen days of a changes to the policy. Two children's files did not have documentation of the shaken baby syndrome and abusive head trauma policy. .1726(b)&(c) The following violations must be corrected immediately. The letter must describe, in detail, specifically how each violation has been corrected and any supporting documents. You can mail or email me with verification of compliance regarding the violations that were documented. If you choose to mail, two signed and dated copies must be sent and received by September 3, 2025. Please be aware that any written information submitted by you regarding correction of the violations documented during today's visit is legal documentation. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always if they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you meet all requirements is to periodically review the child care law and rules. Compliance History: Prior to today's visit, this facility maintained an 18-month compliance score of 95%. TECHNICAL ASSISTANCE/CONSULTATION: RECORDS: Children records were monitored and were not in compliance. Two children enrolled were missing the shaken baby syndrome and abusive head trauma policy and one child did not have immunizations on file for review. To ensure compliance, you should continue to use the children’s checklist, and you are strongly encouraged to complete a file review quarterly. Caregiver verification of required information, and statement of responsibility form was completed and information verified. Caregiver records were not in compliance. Recognizing and Responding to Suspicion of Child Maltreatment was not completed within five (5) years of the previous training date February 2020. To ensure compliance, you should create a document with documentation of due dates and or document on a calendar as a reminder to complete trainings before expiration. HOUSEHOLD MEMBERS: All household members criminal qualification letters were current. If there are changes in your household, please notify me immediately. CRIMINIAL BACKGROUND APPLICATIONS: A New feature links criminal background applications to facilities. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. To ensure compliance, you should complete the ABCMS provider portal training in MOODLE. Once completed, all household members, including yourself should be connected to the facility. CLEAN WATER FOR US KIDS: In the Clean Water for Carolina Kids facility data, status for lead in water, lead base paint and asbestos have not been reported. To ensure compliance, you should complete the enrollment for all three immediately. Once completed, you should notify me. STAR RATED LICENSE (SRL)/QRIS Pathway to Stars: The Pathways to the Stars transition will begin soon. This month, we’ll provide a rule roll out module in the DCDEE e-learning Moodle platform. We’ll also host informational webinars for family child care homes August 20, 2025, at 1pm. All requirements must be included if you choose to use your own forms electronically or hard copy. Child care rules were updated effective November 1, 2024. This family child care home currently operates with a Three-Star license issued on February 12, 2020, earning four (4) points in the Education Component, two (2) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on October 24, 2023. You, V. Pinto was present and notified of the purpose of the visit. Four children ages zero (0) to (3) three were observed engaging in free choice play, hand washing and toileting routines, and breakfast. Breakfast consisted of whole grain waffles, grapes, and milk. A walkthrough of the home was completed today. Fire drills, shelter in place/lock down drills, and playground inspections were all current. Monthly fire drills had not been completed and documented for the months of July and August 2024. Quarterly shelter in place/lockdown drills had not been completed for the month of July 2024. I used the Annual Compliance Monitoring Checklist for Family Child Care Homes and the Family Child Care Home Item Number Listing (DCD 0357) as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. Please remember the entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. Due to connectivity issues, a handwritten visit summary was reviewed and left with you at the conclusion of the visit. A computer-generated visit summary will be emailed to you within two (2) business days. The following violations were documented today. You stated you think you had completed it but did not have a copy to show. The Child Care Rules state that programs must use the EPR template provided by the Division. Programs will access the EPR template from the NC Risk Management Portal, which houses emergency planning tools for a variety of hazards. To access the EPR Template, the trained staff must have an NCID. Once you get an NCID, you will access the EPR template on the NC Risk Management Portal website. Use the access instructions once you are on the NC Risk Management Portal website. CRIMINAL BACKGROUND The Criminal background check (CBC) for household member, A Poteat, expired 1/9/2024. The CBC has not been updated and was not on file for review. We discussed the CBC check should be completed every 5 years and if not received the person in reference cannot have any contact with the children until the updated CBC is received. EMERGENCY PROCEDURES Monthly fire drills were not practiced and/or documented for the months of July and August 2024. The last documented drill was in June 2024. Quarterly shelter in place/ lockdown drills were not practiced and/or documented for the month of July 2024. The last documented drill was in April 2024.
