I completed your last Annual Compliance visit on 1/29/24. The family child care home’s 18-month compliance history from 6/7/23 – 12/6/24 = 100%. The Clean Classrooms for Carolina Kids website reflects the following: Water Testing –completed 5/25/24 Lead Paint Testing – exempt per RTI Asbestos Testing – exempt per RTI *Maintain any paperwork received by mail or email in your Program Records file. *For more information check out the FAQ page at https://www.cleanwaterforuskids.org/en/carolina/. The FCCH Early Learning Professional, C. Beasley, was present and assisted me with the visit. The family child care home currently operates with a Two-Star License, issued 11/4/2018, earning points in the following components: Program (2 PTS) + Education (1 PT) + Quality Point (1 PT) = 4 Points = 2 Stars *Quality Point met and remains in effect: Serves no more than two (2) children younger than one-year of age. The three-year rated license was due 11/4/2021, but new legislation (Senate Bill 425) has extended the “hold harmless” period until the new Quality Rating Improvement System (QRIS) is implemented. Today, you provided me with documentation of your three-month FCCERS-R Self Study to complete a rated license.assesment. The family child care home is owned as a Sole Proprietorship. The family child care home is owned by Little Fisch Childcare LLC. According to the NC Secretary of C. Beasley reported no changes to facility contact information (phone, address, email) or license information. Please contact me prior to any information changing regarding the ownership of this facility or contact information. I monitored the indoor and outdoor spaces used for child care for applicable family child care home requirements using the NC Child Care Requirements, effective 11/1/2024 and Item Number Listing, effective 3/1/2024. I also reviewed all children’s records, operator and household member records, and program records. I checked the car designated for transportation and the required documents; the family child care home continues to be approved for transportation. The operator stated that she never uses a substitute and closes the family child care home when she is not available to provide care. The Written Plan of Care currently reflects that the operator performs routine tasks during operating hours by transporting children in her vehicle. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Written Plan of Care. Observations The operator was present with two children one year and three years of age. The children were sleeping on linen-covered mats in the child care space arranged in the living room. As they woke up, they used the bathroom and washed hands and were offered snack which was teddy grahams, yogurt-covered raisins, goldfish, and water. The operator put on Christmas carols, while the children enjoyed free play. The outdoor play area offered opportunities for gross motor and imaginative play. I observed and documented the following violation during today’s visit. Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Documentation of completion of the required ABCMS Provider Portal Training that instructs the operator how to create a facility roster and notify the Division of new household members or staff was not available. G.S. 110-90.2 & .2703(r) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4) d(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, C. Beasley. We reviewed the visit summary and violation documented during today’s visit. Correct this immediately. Send evidence of correction to me so that I receive it no later than 12/24/24. Your compliance verification letter needs to state your facility name and license number and list the date you complete the required training. Attach a copy of your training certificate to the compliance email. Send compliance verification letter in an email from your fcch’s official email address, bcbeasley@yahoo.com to my email address, Marjorie.White@dhhs.nc.gov. You may include the compliance verification as an attachment or in the body of the email. I will email a sample compliance verification letter to you. Technical Assistance With Documented Violation: #1847: Required Training - ABCMS Provider Portal Training - The process of notifying the Division when you have new staff (or household members for family child care homes and centers licensed in a residence) has changed and is now captured in ABCMS, the new Criminal Background Check portal. To access the ABCMS Provider Portal, you were required to complete the ABCMS Provider Portal Training in Moodle; documentation of completion of the training was not available. You stated that you were not aware that the training was required until a recent email from your consultant. You stated that you have tried to complete the training but cannot access it in Moodle. You stated you have a business NCID, but it is not working. You have sent an email to the Criminal Background Check Unit to assist you with completing the training. Complete the ABCMS Provider Portal Training in Moodle. For your compliance letter state the date you completed the training and send a copy of your training certificate. By your next visit, complete your facility roster even if it is just you and your husband. See below. • The ABCMS Provider Portal Training will teach you how to attach staff and/or household members to your facility. This training needs to be completed by the family child care home operator. The person taking the training will need to obtain a Business NCID and complete the ABCMS Provider Portal Training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. • Once the training is completed, you will be given your facility code (different from license id) and access to the ABCMS Provider Portal where you will verify your facility roster to ensure current household members are listed. If not, you will provide them with the facility code so they can complete a Connecting Application and be added to your roster. Consider completing EDU 119 to earn your NC Early Childhood Credential (NCECC). This will give you 64 on-going training hours in the year you complete it. After that, you would only need 10 hours annually. *Your Five-Year H&S Training is due by 7/2028 On-going Training Options 1. Southwestern Child Development Commission, www.swcdcinc.org – pay for courses individually or buy a year long individual package.. Pay for one year and get enough training for this year, next year, and carry forward hours for the following year. 2. There is a 14-hour Infant/Toddler Orientation training available on Moodle. Emergency Preparedness and Response (EPR) Requirements – EPR Plan updated 10/2024 but you were having trouble with on-line portal this year. You have requested technical assistance with J. PROGRAM STANDARDS –Two (2) Points You currently meet minimum requirements. You began working on your three month FCCERS-R Self-Assessment in August 2022, focusing on Item 5: Display, Item 13: Helping children understand language, and Item 14: Helping children use language. Your project was to create a photo cube to add pictures of children’s families for display and as a conversation tool or way to soothe children when they are missing their families. You have completed the photo cubes and showed them to me today and said that the children look at each other’s cubes every day and talk about the pictures. You also completed your FCCERS-R Self-Assessment this fall and showed me the documentation today. EDUCATION STANDARDS – One (1) Point *based on information reflected on DCDEE Works C. Beasley – HS w/ 19+y licensed family child care home experience. QUALITY POINT - One (1) Point.
