M. King-Beasley., Owner/Operator assisted with the visit. B Best, was also present and assisting. Five (5) children were observed in care today ranging from ages two to three years old. Children were engaged in free-choice play, outdoor play, completing and hand washing routines, interacting with the caregiver, eating lunch and napping. Lunch consisted of fish sticks, corn, French fries, apples and milk. Currently this program operates with a five-star License earning seven points in education, seven points in program standards and one quality point for serving no more than two children under age one. The following violations were documented today. Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. Active ant piles were observed outside on the playground while children were playing. .1719(a)(1)&(17) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Two cans of Lysol spray in aerosol dispensed containers were in a room with the door open across from the bathroom used by children. .1719 (a)(7) The violations documented must be corrected immediately. Please send a compliance verification letter to me describing how the violations were corrected. Please be aware that if it is determined the information submitted was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility's Star Rated License could be issued. If sufficient information is not received by the due date, a follow-up visit will be conducted. COMPLIANCE HISTORY: The program's compliance history was reviewed with the operator. The program’s compliance history was 99% percent as of November 5, 2024. OWNERSHIP STATUS: You (Montraila King-Beasley) are listed as the owner of this family child care home. FACILITY PROFILE INFORMATION: You stated the mailing address, phone number and email address listed for your program are correct. STAFF/CHILD INTERACTIONS: Upon arrival children were outside playing. Caregivers interacted with children during both free choice and outdoor play. Children were given water when they returned inside from outdoor play. Hand washing occurred and children were allowed to engaged in free choice play. Lunch was prepared and served. After lunch children were placed down for nap. Children were cared for in a safe and nurturing environment. SAFE ENVIRONMENT: You stated ant pile come on the playground every day. You stated the ant piles are treated to remove the ants. We discussed checking the playground daily prior to allowing children out to ensure ant piles are removed. Be reminded a safe environment must always be provided for children in care. QUALITY RATING IMPROVEMENT SYSTEM: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. This legislation also provides a pathway for child care programs currently accredited through the approved accrediting bodies to move to a 3 or 5 star license if they are not already at this star level. Providers will need to submit a request to their licensing consultant and provide a copy of their accreditation award/certificate for processing: Accreditation Licensure Request Form QRIS UPDATE: GET READY for the 3’s Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. Visit the NCRLAP’s website for more information about updated resources, credit hour trainings, and outreach assessment opportunities to help you become familiar with these tools. For official environment rating scale assessments for a NC Star Rated License, the NCRLAP will use the Revised editions until February 1, 2025. CRIMINAL BACKGROUND CHECK REQUIREMENTS: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. Upon arrival, you, M. King-Beasley, Operator, were present and available to assist with today’s visit. Your program currently operates with a five-star license issued February 1, 2024, earning 7 points in the education component, 7 points in the program standards component and 1 quality point using an age/developmentally appropriate curriculum. The last annual compliance visit was conducted on June 22, 2023. The last fire inspection was completed on January 11, 2024, and the family child care home was approved for daytime care only. The center’s compliance history was reviewed with the operator. The program’s compliance history was 100% percent as of May 16, 2024. Four (4) children aged two (2) to three (3) years-old were present. You contract with Greene Lamp Head Start and offer early head start. You stated you currently have five (5) children enrolled and two (2) participate in early head start. The children were observed playing with developmentally appropriate materials, free-choice play, art activity and completing routines such as handwashing, and toileting. You interacted with the children in a positive manner. The children were observed eating the following for lunch: ground turkey burger with noodles, apples, corn, French fries, and milk. You stated you do not provide transportation. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. There were three (3) active anthills. One was located in front of the stairs leading to the outdoor learning environment, and two were in the middle of the yard. STAR RATED LICENSE ASSESSMENT (COHORT ONE): Your three-year reassessment was due by January 2, 2021; however, your program is currently under Senate Bill 291; Hold Harmless status and you are in cohort one (1). The family child care home is currently operating with a 5 Star-Rated License. I provided you with an application for re-assessment and your Rate License Assessment Request Review, today May 17, 2024. The requirements for the reassessment of the family child care home star-rated license were reviewed during today's visit: Education Standards – According to the DCDEE WORKS system verified on May 16, 2024, you, M. King-Beasley, (operator) have completed your associate degree in early childhood education. Based on this information, it appears your program is eligible for seven (7) points in Education Standards. Program Standards - You are meeting minimum child care requirements. You are currently working on completing your three (3) month self-study using the FCCERS-R. You will provide me with documentation of the three (3) month self-study once you have completed the self-study. During today’s visit you have also requested to complete the Family Child Care Environment Rating Scale. Based on the score received from the ERS assessment will determine how many points the family child care home is qualified for in Program Standards. Points earned are pending the overall score on the Early Childhood Environment Rating Scale. You stated that you have been working with your local Partnership and at this time you do not need any technical assistance and were ready for the environment rating scale assessment. I encouraged you to review the North Carolina Rated License Assessment Project website at www.ncrlap.org. The website contains Additional Notes for Clarification, resources and NCRLAP videos regarding the assessment process. Quality Point Option - You stated that you would like to continue to use an age/developmentally appropriate curriculum. Based on this information, it appears your program will be eligible for 1 Quality Point. TECHNIAL ASSISTANCE/CONSULTATION: CHILDREN AND STAFF FILES: Your file and Ms. B. Best’s file were complete and had all the requirements available for review. Your children’s file was also complete and available for review. Your files were well organized, please continue to keep your files current and ready for review. SAFETY AND SUPERVISION: In the outdoor learning environment where children play, there were three (3) active anthills. One was located in front of the stairs leading to the outdoor learning environment, and two were in the middle of the yard. There was also a riding lawn mower located in a shed that did not have a door that was accessible to children. You immediately called to have the anthills treated and they were treated and no longer active. You also had the riding lawn mower removed. Both of these items were corrected during the visit. As we are approaching the warmer months, please continue to check the grass for anthills and treat immediately and check any pieces of climbing equipment and awnings for insect hives. CLEAN WATER FOR US KIDS: We discussed the Clean Water for Carolina Kids program and what that means for you as a family child care home provided. I also informed you that I sent an email to you on March 5, 2024, with the following information included. I also attached to that email three (3) flyers that discuss the process for testing, for lead in the water, lead in paint, and asbestos. According to the Clean Water for Kids Website, you have completed enrollment for the lead testing for your water as of April 10, 2024. You stated you have sent off your water samples. You have started enrollment in the lead paint testing and asbestos. The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. There are webinars that child care providers can watch to get instructions. Go to https://www.cleanwaterforuskids.org/en/carolina/ for more information. Currently operating child care facilities and FCCH’s are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. All existing licensed programs must complete the application summary for their program regarding lead paint and asbestos by May 1, 2024. Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Family child care homes are required to test water for lead once. Child care rule 10A NCAC 09 .1725(a) for FCCH’s - to assure the health of children through proper sanitation, the family child care home operator shall: (2) collect and submit samples of water from each water outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. Results of the analysis shall be on file in the home. This rule became effective January 1, 2024; therefore, water samples must be collected by the operator and tested within 12 months of the effective date of this rule. This means that lead in water testing must be completed by January 1, 2025. The entire home is licensed for child care and must meet the applicable child care requirements. Although you designated space to be used for child care in a specified section of your home, the license is issued for the entire location. Any monitoring visit conducted may include spaces other than those you designated for child care. Upon arrival, you, M. King-Beasley, Operator, were present and available to assist with today’s visit. Your program currently operates with a five-star license issued February 1, 2024, earning 7 points in the education component, 7 points in the program standards component and 1 quality point using an age/developmentally appropriate curriculum. The last annual compliance visit was conducted on June 22, 2023. The last fire inspection was completed on January 11, 2024, and the family child care home was approved for daytime care only. The center’s compliance history was reviewed with the operator. The program’s compliance history was 100% percent as of May 16, 2024. Four (4) children aged two (2) to three (3) years-old were present. You contract with Greene Lamp Head Start and offer early head start. You stated you currently have five (5) children enrolled and two (2) participate in early head start. The children were observed playing with developmentally appropriate materials, free-choice play, art activity and completing routines such as handwashing, and toileting. You interacted with the children in a positive manner. The children were observed eating the following for lunch: ground turkey burger with noodles, apples, corn, French fries, and milk. You stated you do not provide transportation. Any violations of the child care requirements observed today were discussed with you and documented in the visit summary report and reviewed with you at the conclusion of the visit. The following violations were documented: Violation Number Comment Rule 1409 Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. There were three (3) active anthills. One was located in front of the stairs leading to the outdoor learning environment, and two were in the middle of the yard. There was also a riding lawn mower located in a shed that did not have a door and was accessible to children. 10A NCAC 09 .1719 (a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. OWNER/FACILITY PROFILE INFORMATION: You, Montraila King-Beasley, are listed as the Sole Proprietor on the Family Child Care Home’s License and you stated during today’s visit this ownership remains the same. STAR RATED LICENSE ASSESSMENT (COHORT ONE): Your three-year reassessment was due by January 2, 2021; however, your program is currently under Senate Bill 291; Hold Harmless status and you are in cohort one (1). The family child care home is currently operating with a 5 Star-Rated License. I provided you with an application for re-assessment and your Rate License Assessment Request Review, today May 17, 2024. The requirements for the reassessment of the family child care home star-rated license were reviewed during today's visit: Education Standards – According to the DCDEE WORKS system verified on May 16, 2024, you, M. King-Beasley, (operator) have completed your associate degree in early childhood education. Based on this information, it appears your program is eligible for seven (7) points in Education Standards. Program Standards - You are meeting minimum child care requirements. You are currently working on completing your three (3) month self-study using the FCCERS-R. You will provide me with documentation of the three (3) month self-study once you have completed the self-study. During today’s visit you have also requested to complete the Family Child Care Environment Rating Scale. Based on the score received from the ERS assessment will determine how many points the family child care home is qualified for in Program Standards. Points earned are pending the overall score on the Early Childhood Environment Rating Scale. You stated that you have been working with your local Partnership and at this time you do not need any technical assistance and were ready for the environment rating scale assessment. I encouraged you to review the North Carolina Rated License Assessment Project website at www.ncrlap.org. The website contains Additional Notes for Clarification, resources and NCRLAP videos regarding the assessment process. Quality Point Option - You stated that you would like to continue to use an age/developmentally appropriate curriculum. Based on this information, it appears your program will be eligible for 1 Quality Point. TECHNIAL ASSISTANCE/CONSULTATION: CHILDREN AND STAFF FILES: Your file and Ms. B. Best’s file were complete and had all the requirements available for review. Your children’s file was also complete and available for review. Your files were well organized, please continue to keep your files current and ready for review. SAFETY AND SUPERVISION: In the outdoor learning environment where children play, there were three (3) active anthills. One was located in front of the stairs leading to the outdoor learning environment, and two were in the middle of the yard. There was also a riding lawn mower located in a shed that did not have a door that was accessible to children. You immediately called to have the anthills treated and they were treated and no longer active. You also had the riding lawn mower removed. Both of these items were corrected during the visit. As we are approaching the warmer months, please continue to check the grass for anthills and treat immediately and check any pieces of climbing equipment and awnings for insect hives. CLEAN WATER FOR US KIDS: We discussed the Clean Water for Carolina Kids program and what that means for you as a family child care home provided. I also informed you that I sent an email to you on March 5, 2024, with the following information included. I also attached to that email three (3) flyers that discuss the process for testing, for lead in the water, lead in paint, and asbestos. According to the Clean Water for Kids Website, you have completed enrollment for the lead testing for your water as of April 10, 2024. You stated you have sent off your water samples. You have started enrollment in the lead paint testing and asbestos. The Clean Water for Carolina Kids program is expanding to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. Currently operating child care facilities and FCCH’s are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Family child care homes are required to test water for lead once. Child care rule 10A NCAC 09 .1725(a) for FCCH’s - to assure the health of children through proper sanitation, the family child care home operator shall: (2) collect and submit samples of water from each water outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. Results of the analysis shall be on file in the home.
