Name of Operation: WALNUT COVE ELEMENTARY PRE-K Facility ID: 85000083 Consultant: KELLY HEALY Operation Type: Center Case Number: Visit Date: 10/2/2023 Number Present: 26 Completed Date: 10/3/2023 Age: From 3 To 5 Total Minutes: 120 Time In: 10:00 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor this program for compliance with applicable childcare requirements for a Routine Unannounced visit. This visit was conducted with Ms. Penny Linville, Teacher. This program currently operates with a Five Star license. Restrictions include daytime care only, meets all enhanced ratios minus one, Dev Day Certified, use of approved bathrooms only, PE and janitor closets locked when not occupied. The license was posted and the restrictions were being followed. The NC Secretary of State website was reviewed on 08-02-2023. I am unable to check the compliance history, as Regulatory is down today. Programs are required to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I have observed indoor and outdoor areas today. I monitored staff/child ratios and space capacity today, as is indicated on the attached worksheet. I observed proper supervision of children. During the visit, the children were eating lunch and playing in activity centers. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions Ms. Linville reports that there are no new staff persons hired since my last visit. A playground inspection was conducted on 08-26-2023. A fire drill was conducted on 09-07-2023. The center has not conducted a shelter in place drill this year. A sanitation inspection was conducted on November 30, 2023. I observed the storage of hazardous items and cleaning supplies. I observed both the storage of medications and the administration of medication paperwork. This program provides transportation. Ms. Linville has provided me with the Public off-site records verification form today. The following violations were documented during today’s visit: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current lesson plan was not posted in room 411. GS 110-91(12); .0508(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A current menu was not posted. 10A NCAC 09 .0901(b) The violations were corrected during the visit. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. The following Technical Assistance was provided during today’s visit: Allergies should be posted but not covered. The allergies were posted today. Two classrooms have exit doors that are locked with a child lock. Only an adult can open these doors and they are not one motion. Ms. Linville reports that the fire marshal approved these exits on his visit. I will call him and ask if these child locks are approved and document his answer for the file.
