A checklist was used to note the requirements I monitored today. This program currently operates with a Four-Star License, issued February 9, 2023. Restrictions include: a capacity of 8, age range of 0-12, first shift only, max. of 5 preschool children at any time, children in care on ground level only, and serves no more than 2 children under age one. The license was posted, and the restrictions were in compliance. The last annual compliance visit was conducted on July 16, 2025. As of today, your facility had an 18-month compliance history of 100%. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The NC Secretary of State website was reviewed today. The owner of this facility, Sunny Trail Child Care and Learning Center, LLC, was listed as current and active. Owner/Operator, Salina Dixon, was present and available for consultation. Ms. Dixon’s daughter, Samaria Dixon, was also present during the visit. Ms. Dixon advised that she is the primary caregiver for the children on all three shifts; Samaria Dixon and Sequoia Dixon are back up/substitute caregivers and used on an as needed basis. All spaces used by the children were monitored. A daily schedule and activity plan was observed posted. Appropriate toys, materials, and equipment were observed in these areas. There were five children present, aged 1-7. Children had eaten lunch. Lunch consisted of chicken nuggets, green beans, fruit cup, and milk. After lunch, children observed nap/rest time. Children were observed sleeping on covered cots with blankets. All children were adequately supervised. Caregivers interacted in a positive and appropriate manner. Reminder: Capacity - You currently have a total of eleven children enrolled at your facility on first, second, and third shifts, including drop ins. You cannot have more than eight children, five preschool children and three school-age children, present at any time, no exceptions. There is no shift overlap, and it is vital that you keep accurate sign in/out sheets and attendance records to verify times when children are in attendance. The was one violation observed today. The NC Secretary of State website was reviewed on July 23, 2024, the owner of this facility, Sunny Trail Child Care and Learning Center, LLC was listed as current and active. Please pay close attention to the charts located in 10A NCAC 09 .1721(f)(4) which indicate how long records must be kept on file. I also suggest you utilize the checklists for staff files, children’s files and program records to ensure you have all required documents on file within the required timeframe. To be consistent, I would suggest your set up your staff files and children’s files based on the format of the checklists. These checklists are available on the DCDEE website under the Provider Documents tab. The family child care home (FCCH) operator and additional caregivers shall meet on-going training requirements by attending child-care workshops, conferences, seminars, or courses. In your planning year, you may review resources available at www.ncrlap.org and contact Smart Start of Pender County or Region 4 CCR&R for assistance with completion of the self-study component. I am also available to assist you upon request. The “hold harmless” state of rated license reassessments has been extended until the new Quality Rating Improvement System standards are put in place. This means that you may not be required to have a reassessment according to timelines discussed previously, however you may request one voluntarily. May 1, 2024, was the initial deadline to enroll your facility in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. Facilities that have not yet enrolled should complete the process by November 1, 2024. This program is an expansion of Clean Water for Carolina Kids, which previously tested all child care centers in the state for lead in water at drinking and food preparation taps. As this is a legislatively mandated effort, facilities are required to meet rule requirements to have tap water tested every three years(centers) and to have inspections for lead paint and asbestos if applicable. I also suggest you utilize the checklists for staff files, children’s files and program records to ensure you have all required documents on file within the required timeframe. To be consistent, I would suggest your set up your staff files and children’s files based on the format of the checklists. These checklists are available on the DCDEE website under the Provider Documents tab. The family child care home (FCCH) operator and additional caregivers shall meet on-going training requirements by attending child-care workshops, conferences, seminars, or courses. Technical Assistance: During the visit, SaQuoia Dixon placed a child into the crib. While moving the crib into place in the kitchen area to better supervise the child, Ms. Dixon held onto the child’s arm between the elbow and the underarm, so he did not fall over. Ms. Dixon did not pick up the child or lift the child from the crib and a violation was not cited. I asked her if she had ever been trained or taught about safe lifting practices as I saw an opportunity to give technical assistance. Never pull or lift the child by the hands, wrist, or upper arm. Knowing how to properly pick up and carry an infant or toddler given the child’s size, muscle structure, and age is critical to handling them safely. Lift the child by grasping the child’s body under the arms or around the body. Lifting a child inappropriately can cause Nursemaid’s Elbow injury. This injury usually doesn’t cause swelling, but the child will complain that the elbow hurts, or cry when her arm is moved. A child will typically hold her arm close to the side, with the elbow slightly bent and the palm turned toward the body. If someone tries to straighten the elbow or turn the palm upward, the child will resist because of the pain. If a staff member is holding a child’s hand and the child flops to the ground, lower the child slowly and then pick the child up under their arms to carry. Reminders: Be reminded that your license includes a restriction stating that all outdoor play occurs in the fenced area. Today a slide going down the porch steps was observed. This slide and the area set up at the bottom of the stairs is not a licensed area and therefore cannot be used during operating hours with children enrolled in the facility. The operator shall retain the acknowledgement in the child's records as long as the child is enrolled in the home and a copy shall be maintained on file for review by me. A written plan of care form is located on the website. This must be on file for all children enrolled at your facility. Safe sleep charts must be available and maintained for all children, age 12 months and under. Ensure you have an infant feeding schedule completed and on file for all children under 15 months of age. Infant feeding schedules must be followed. It is vital that attendance sheets be accurate and maintained at all times. This ensures the safety of the children in your care at all times. Staff should be marking children present as they arrive and have knowledge of how many children are present in their classroom at any given time should an emergency arise. Staff should be able to give an accurate account of the children they are responsible for daily. Ensure health questionnaire forms and emergency information is updated on staff yearly. Medical statements and TB skins test results must be on file and available for review for all staff. Emergency Preparedness and Response(EPR) – EPR training should be completed by August 8, 2023, one year from date your facility was licensed. Once the EPR training has been completed, you must complete your EPR plan within four (4) months. Once your EPR plan has been completed, print a copy and have it available for review. You will need to complete either a shelter-in-place or lock -down drill each quarterly and notate this information on the Emergency drill log. Health and Safety Trainings - These training topics outlined in Child Care Rule 10A NCAC 09 .1703(b) must be completed within the first year of employment for new staff, with the exception of Recognizing and Responding to Suspicions of Child Maltreatment – it must be completed within 90 days of hire. Restrictions include: a capacity of 8, age range of 0-12, first shift only, max. of 5 preschool children at any time, children in care on ground level only, and serves no more than 2 children under age one. This facility was licensed on August 8, 2022. As of August 2, 2023, your facility had an 18-month compliance history of 100%. The NC Secretary of State website was reviewed on August 1, 2023, the owner of this facility, Sunny Trail Child Care and Learning Center, LLC was listed as current and active. Samaria Dixon, teacher, was present and available for consultation. This facility has four spaces set up for child care. Appropriate toys, materials, and equipment were observed in these areas. Additional teacher, SaQuoia Dixon, arrived an hour into the visit and Owner/Operator, Salina Dixon, arrived back at the facility as I was finishing the visit. All spaces used by the children were monitored. There were five preschool children, including an infant, present today. Infants received care based on their individual needs. Infant feeding schedule must be completed, and safe sleep charts maintained for infants. All children were observed participating in various free choice activities indoors and outdoors. All children were adequately supervised. Children also competing handwashing routines and ate lunch. Lunch consisted of BBQ chicken, macaroni and cheese, beans and rice, pineapples, and milk. . There were two violations observed today. All violations must be corrected immediately. Continued compliance with applicable child care requirements is required.
