A checklist was used to note the requirements I monitored today. During the visit the Administrator Ms. S. Johnson accompanied me on the walk-through of the facility and assisted with the visit. The Owner/Ms. L. Kornegay arrived later during today’s visit. Prior to the visit I reviewed your corporation status as current-active on the Secretary of State website. This program currently operates with a five-star license issued November 27, 2019. During the visit I reminded you to make sure all staff have sent their education to WORKS to be evaluated. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, summary of NC Child Care Law, were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. The staff and children’s records were also reviewed today. Files for new staff were monitored with a random selection of returning staff members. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files was reviewed, and the children’s record worksheet was completed. During the visit, the school age children were observed playing in the gym, while the other group of school age children were observed playing outside on the playground. Today the lesson plans had themes on science. The lunch menu consisted of NC Pre-K The NC Pre-K monitoring was not completed during today’s visit due to summer vacation. Once the NC Pre-K children return a monitoring visit will be completed in the future. Your last fire drill was completed 7/28/25 and your last monthly outdoor playground inspection was dated 7/25/25. Your last EPR drill was a lockdown dated 6/13/25. I reminded you that an EPR drill must be completed every three months and keep all paperwork on file for review. Your last fire inspection was dated 12/4/24. Your last sanitation inspection was completed 5/01/25 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed, mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. The bus used to transport children insurance expires 3/18/26 and the inspection expires 4/30/26. Today we discussed the information you needed to have on the bus if you are transporting any children. You stated you didn’t have any questions. The following violations were documented during today's visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member did not renew first aid training before the certification expired 7/24/25. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member did not renew CPR training before the certification expired 7/24/25. .1102(d) The violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By August 12, 2025, please send me a letter addressing each violation, how the violation was corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at carla.hanchey-edwards@dhhs.nc.gov Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. Please be reminded CPR/First aid trainings must be completed prior to the expiration date on your staffs’ cards. Remember any new staff hired must complete CPR/First aid training within the first ninety (90) days of employment. I reminded you that the health and safety trainings have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded the health and safety training courses are required to be completed every five years. Technical assistance regarding the following items were reviewed with you today. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. At the time of today's visit, our area has been experiencing extreme heat with temperatures in excess of 100 degrees. We discussed several concerns presented by these high temperatures and ways to address them: Outdoor time must be limited to no more than fifteen minutes and must occur in the early hours of the day. Please refer to your weather watch chart for further instructions for your youngest children in care, as well as those with special health concerns that may be made worse when exposed to high temperatures. While outside, please pay special attention to all children and staff members to watch for signs of heat exhaustion including heavy sweating, pale or clammy skin, headaches, dizziness, weakness, vomiting and/or a fast, weak pulse. Heat strokes are considered a medical emergency and require immediate medical attention. Symptoms of heat stroke include a dangerously high body temperature (104 or higher), hot and dry skin, confusion, rapid pulse and potential loss of consciousness. If you suspect a child or staff member is experiencing either of these conditions, please seek medical attention. Drinking water must be freely available to all children and should be provided during outdoor play, as well as immediately upon re-entry into the facility following outdoor play. Your building must remain at 85 degrees or less based on Environmental Health requirements. If you are not able to maintain these temperatures, you would need to move the children to spaces where the temperature can be maintained or consider closing until the air conditioning can be repaired. Before transporting children, please ensure that the air conditioning on each vehicle is working and that the vehicles can be adequately cooled. I recommended that you limit off-premises activities that include outdoor activities during this extreme weather.
