The center currently operates with a one-star license by choice, operating at 25 sq. ft. inside, & ratios at 0713 (a). My visit was conducted with Julia, Director. The children were observed taking a nap, playing in centers, and going home. A checklist was used to monitor for compliance. The children bring their meals and snacks from home. The facility does not transport children; therefore, these requirements were not monitored. VERIFICATION OF RECORDS: *Last fire inspection=1-11-24 *Sanitation inspection=10-23-23 (send me your 2024 inspections within 5 days of completion). The center’s water test expires on 3-22-26. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (K Maree Educational Services, Corp.) listed as current-active. *EPR training was completed by Julia Thompson on 9-29-16 and the EPR plan was last updated on 1-10-24. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately. Please email me a signed and dated corrective action letter with your ID # and facility name BY 2-9-24 to michelle.ezzell@dhhs.nc.gov. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. One permission to administer medication form did not have an amount of diaper cream to be administered on the permission slip. 10A NCAC 09 .0803(4)(6-9) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. A monthly playground inspection was not completed in December 2023. .0605(q) *****TECHNICAL ASSISTANCE/REMINDERS: ******* *On the permission to administer medication form under “amount to be applied” it should indicate an exact amount such as a dime size, quarter size, etc. *Monthly Outdoor Inspections must be completed monthly. We discussed different ways to remember when to complete these inspections. *Remember to continue to update your Emergency Preparedness and Response (EPR) Plan every year and print out the cover page showing the current date. Julia Thompson, Administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Thompson and two additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children.
