Upon arrival I was greeted by Mariluz Santiago, the Director. You assisted me with today’s visit. I completed a general walk-through of the indoor and outdoor learning environment. A checklist was used to note the requirements monitored today. At the time of the visit, there were a total of one hundred twenty-nine (129) children present, ranging in age from infants to five (5) years old. The children were observed in circle time, transitioning to the playground, interacting with the teachers, completing routine tasks, eating lunch, and resting at nap time. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued June 9, 2022, earning seven (7) points in education, six (6) points in program standards and one (1) quality point. This center operates at 35sq. ft. inside, & ratios at .2818(b). I reviewed the NC Secretary of State’s website and observed the owner of the facility 7 Hills Learning Wake Forest LLC listed as current/active. INSPECTIONS *The last fire drill was conducted 1/20/2026. *The last emergency drill was conducted 1/20/2026. *The last playground inspection was completed 2/7/2026. *The last sanitation was completed on 9/15/2025 with a “Superior ‘’ classification. *The fire inspection was completed on 10/10/2025 and the facility is approved for daytime care only. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. Transportation is provided, therefore these requirements were monitored and found in compliance. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. In space #2 two sippy cups containing milk was not labeled with a date. 15A NCAC 18A .2804(d) 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence) In space #1 a child who started on 2/17/26 did not have a feeding schedule posted. In space #2 a child who transitioned the week of 2/9/26 from space #1 did not have a feeding schedule posted. 10A NCAC 09 .0902(a) 847 Parent's medication authorization did not include required information. In space #2 two medication authorization forms did not include valid dates of authorization, when and how the medication should be applied. 10A NCAC 09 .0803(4)(6-9) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before March 5, 2026, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 90%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. FEEDING SCHEDULE In space #1 a feeding schedule for a child who started on 2/17/2026 was not posted in the classroom. The teachers stated that the child was not in attendance today. The administrator stated that the feeding schedule will be signed and completed when the child returns. In space #2 a child did not have a feeding schedule posted. The teachers stated the feeding schedule may be in space #1. It was also stated that the child has been in space #2 receiving care since last week. I discussed with the administrator the importance of having a feeding schedule for each individual child posted or readily available to the teachers. I would recommend retraining the teachers on the importance of using feeding schedules. I would recommend a member of the management check monthly to ensure that all feeding schedules are up to date. I would also remind teachers when a child transitions to send all necessary paperwork with the child. INFANT BOTTLES/SIPPY CUPS In space #2 it was observed that two sippy cups containing milk were not labeled with a date. I would remind all staff to check the name and dates of all bottles and sippy cups parents put in the refrigerator daily. It is important that all items are labeled with a current date to avoid spoilage. MEDICATION AUTHORIZATION In space #2 diaper cream forms did not have valid authorization dates. A form for Destin stated as needed. It is important that parents specify instructions such as when to give the medication and how much. I would suggest having a member of management to check all medication forms to ensure they are completed properly. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. QRIS Modernization The QRIS transitional plan was created during the visit. The administrator stated that they will be choosing Pathway 1. They anticipate going through the process in September. HEALTH AND SAFETY TRAINING Health and Safety training courses are now available through Moodle on the DCDEE website. This training courses are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules. The last documented playground inspection was conducted on 8/4/25. .0605(q) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Three children did not have documentation of sleeping. The last documentation for the children were 12pm, 10:52am, and 10:00am. SAFE SLEEP During today’s visit I observed infants sleeping without any documentation of when the child fell asleep as well as documenting the infant being physically checked. The teacher of the infant classroom was scheduled off. Documentation of a sleeping infant is very important for the safety of infants. I would recommend having your infant teachers and floaters watch ‘Keeping Them Safe’ Infant Sleep Safety on the website for Caring for Our Children. I would also recommend sending a reminder to your infant teachers concerning safe sleep documentation. INFANT BOTTLES I observed sippy cups in the Infant B class without a name or date. I would recommend retraining teachers on properly labeling sippy cups and bottles with name and a current date. MEDICATION Diaper creams were either without parent authorization forms, and/or expired dates in space #2, #3, and #5. The teachers in each classroom stated that they did not have any diaper cream or medications. When monitoring the classroom, it was observed that diaper cream was in the cabinets under the changing table. I would recommend checking all classrooms for diaper creams and medication to return to parents or discard. FEEDING CHARTS It was observed that one child in space #2 did not have an infant feeding schedule. It is important to have an individual feeding chart for each child to ensure proper nutrition and safety. I would recommend retraining all staff by following the posted feeding schedule at all times. I would also recommend receiving all necessary paperwork required on or before enrollment. MONTHLY PLAYGROUND INSPECTION The last monthly playground inspection was conducted in August of 2025. I would recommend using an outlook calendar or a reminder for all monthly inspections. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. I would also recommend sending a reminder to your infant teachers concerning safe sleep documentation. INFANT BOTTLES I observed sippy cups in the Infant B class without a name or date. I would recommend retraining teachers on properly labeling sippy cups and bottles with name and a current date. MEDICATION Diaper creams were either without parent authorization forms, and/or expired dates in space #2, #3, and #5. The teachers in each classroom stated that they did not have any diaper cream or medications. When monitoring the classroom, it was observed that diaper cream was in the cabinets under the changing table. I would recommend checking all classrooms for diaper creams and medication to return to parents or discard. FEEDING CHARTS It was observed that one child in space #2 did not have an infant feeding schedule. It is important to have an individual feeding chart for each child to ensure proper nutrition and safety. I would recommend retraining all staff by following the posted feeding schedule at all times. I would also recommend receiving all necessary paperwork required on or before enrollment. MONTHLY PLAYGROUND INSPECTION The last monthly playground inspection was conducted in August of 2025. I would recommend using an outlook calendar or a reminder for all monthly inspections. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. QRIS MODERNIZATION I was unable to discuss the QRIS modernization during today’s visit. The Director and owner were not available during the visit. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. Ms. Santiago stated that when they arrived at the school on Tuesday morning 7/15/25 they immediately contacted Joe Powell with Glass Solutions to fix the window. Ms. Santiago placed a thick red tarp over the window and Mr. Powell placed film on the inside and outside of the window to keep the window from shattering. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued June 9, 2022, earning seven (7) points in education, six (6) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility 7 Hills Learning Wake Forest LLC listed as current/active. INSPECTIONS *The last fire drill was conducted 2/6/25. *The last emergency drill was conducted 2/6/25. *The last playground inspection was completed 2/10/25. *The last sanitation was completed on 12/9/24 with a “Superior ‘’ classification. *The fire inspection was completed on 10/11/24 and the facility is approved for daytime care only. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. SAFE SLEEP During today’s visit I observed an infant sleeping without any documentation of when the child fell asleep as well as documenting the infant being physically checked in space #104. The teacher stated that the Lead Teacher is out sick. I only had documentation of safe sleep charts to the date of 2/12/25. I would recommend having your infant teachers watch ‘Keeping Them Safe’ Infant Sleep Safety on the website for Caring for Our Children. I would also recommend training all staff who will work in the infant classrooms in safe sleep documentation. a reminder to your infant teachers concerning safe sleep documentation. NUTRITION FOR INFANTS/TODDLERS I observed an infant being placed in his crib to drink his bottle in space #104. I discussed with the teacher and director that the child would need to be held or placed in feeding chairs when drinking a bottle. I would recommend retraining all staff with ITS-SIDS, as well as reviewing information from Caring For Our Children, Chapter 4 concerning Nutrition and Food Service at https://nrckids.org. FEEDING SCHEDULE Feeding Schedules for three (3) infants in space #104 were not updated as the child’s needs changed. It is important to get in writing the approval of any changes to feeding schedules for infants. I would recommend a member of the management check monthly to ensure that all feeding schedules are up to date. CHILDREN FILES A child enrolled on 12/16/24 did not have a medical or health assessment record on file. The parent sign off for the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was observed not signed by the parents. I would recommend using the Children File Checklist as well as doing a monthly check of all files to ensure the files are accurate and complete. SAFETY AND HEALTH A staff member’s bag was observed hanging near the bathroom door in Preppers A classroom. I asked if the bag was the First Aid kit. The teacher stated yes and then said no it was her personal bag. In Preppers B classroom I observed a staff member’s purse under the sink in an unlocked cabinet. The teacher stated that the cabinet was broken and is supposed to be fixed. I would recommend checking all cabinets and drawers to make sure that they are working properly. I would also recommend putting all items that need to be locked in the cabinets that can look and are not in reach of the children. Please note that a number of the cabinets and drawers in each classroom are broken and need to be repaired. I would recommend a member of the management check monthly to ensure that all feeding schedules are up to date. CHILDREN FILES A child enrolled on 12/16/24 did not have a medical or health assessment record on file. The parent sign off for the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was observed not signed by the parents. I would recommend using the Children File Checklist as well as doing a monthly check of all files to ensure the files are accurate and complete. At the time of the visit, there were a total of one hundred thirty-nine (139) children present, ranging in age from infants to five (5) years old. The children were observed in circle time, transitioning to the playground, completing routine tasks, and preparing for lunch. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued June 9, 2022, earning seven (7) points in education, six (6) points in program standards and one (1) quality point. This center operates at 35sq. ft. inside, & ratios at .2818(b). I reviewed the NC Secretary of State’s website and observed the owner of the facility 7 Hills Learning Wake Forest LLC listed as current/active. INSPECTIONS *The last fire drill was conducted 2/6/25. *The last emergency drill was conducted 2/6/25. *The last playground inspection was completed 2/10/25. *The last sanitation was completed on 12/9/24 with a “Superior ‘’ classification. *The fire inspection was completed on 10/11/24 and the facility is approved for daytime care only. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. The child continued to cry and had a difficult time for about four minutes. I would recommend doing a retrain with teachers on techniques and strategies to assist toddlers to adjust to his/her environment in keeping with the child’s developmental needs. I would also recommend viewing the Caring for Our Children website and review Responsive Interactions with toddlers. INFANT BOTTLES I observed an infant bottle in the Infant B class with a date of 10/17/24. I would recommend teachers send home bottles at the end of the day. A report was received by the agency on 8/29/2024. During the visit, I discussed the allegation with the Assistant Director, Marcia Hinton. MONITORING I completed a walk-through of the facility. Limited monitoring of child care requirements occurred during today’s visit. I monitored (supervision, child/staff ratios, adequate/approved space, license and permit restrictions). I observed one hundred twelve (112) children engaged in free play, interacting with the teachers, completing routine tasks, and playing outside. ALLEGATION #1 There were concerns about Sanitation/Health; staff and children are not being excluded when sick. ALLEGATION #2 There are concerns of sanitation regarding properly cleaning and sanitizing the center. OBSERVATIONS/DISCUSSIONS ALLEGATION #1 During today’s visit the Assistant Director was able to share with me the centers sick policy and log. I observed one recent documentation logged for a sick child dated 8/14/24. The Director was out of town but was available to be interviewed over the phone. I discussed with the director the allegations. I asked the director has she had any concerns with staff and children being sick with a communicable illness. The Director stated that she had one staff member to notify her of a communicable illness the morning of 8/29/24. The director and assistant director stated that neither notified the parents and staff of the communicable illness that was disclosed. I also, interviewed three teachers randomly regarding any concerns they may have had with staff and children being sick while at the center. During today’s visit I observed staff members with active symptoms of illness working. RESOLUTION Based on information received from the administrator’s admission of not notifying staff and parents of a communicable illness, and today’s observations, the allegation is SUBSTANTIED. ALLEGATION #2 During today’s visit I observed mouthed toy bins of toys being sanitized in the classrooms. I interviewed three teachers randomly concerning their cleaning and sanitizing policies and procedures. The teachers informed me of the process that they do to keep the center clean and sanitized. The Assistant Director stated that the center also has cleaners that do a deep clean of the center over the weekends. I observed the center clean during today’s visit. RESOLUTION Based on the discussions with the staff, and observations from today, I am unable to determine concerns with cleanliness and sanitation of toys. The following violation was observed and documented during today’s visit. I monitored required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, program requirements, equipment, transportation, and furnishings. No medications are currently at the center. VERIFICATION OF RECORDS: *Last fire inspection=10-9-23 (send me your 2024 inspection within 5 days of completion) *Sanitation inspection=12-1-23 (forward me any additional 2024 inspections). *Water test for lead was on 5-5-21 and expires 5-5-24. We discussed the process to renew the water test and complete the asbestos test. *The last playground safety inspection was on 3-11-24 by Mariluz. Mariluz completed playground safety training on 11-20-10. *The EPR plan was last updated on 2-21-24. Today, two classrooms did not have current cot/mat charts, this violation was corrected during the visit. *Lesson plans must be current and posted. Today two lesson plans were not posted during my walk-through. Staff quickly found the lesson plan and corrected this violation. *I suggest having a separate folder for staff’s on-going training certificates. *Remember to transfer the notes from your hard copy of the staff/training worksheets to your electronic version. *Remember to continue to update your Emergency Preparedness and Response (EPR) Plan every year and print out the cover page showing the current date.
