A checklist was used to note the requirements monitored today. Upon arrival we were greeted by Donna Stroud, the Director. Vanessa Price, Child Care Consultant accompanied me on this visit. There was a total of ninety-two (92) children present, aging in range from infant to five (5) years old. The children were observed playing in centers, playing outside, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued December 18, 2018, earning six (6) points in education, six (6) points in program standards and one (1) quality point. The center operates at 30 sq. ft. inside and ratios at .2818 (b). I reviewed the NC Secretary of State’s website and observed the owner of the facility as Great Start Inc. listed as current/active. INSPECTIONS *The last fire drill was conducted 7/29/24. *The last emergency drill was conducted 6/19/24. *The last playground inspection was completed 8/1/24. *The last sanitation was completed on 8/5/24 with a ‘Superior’ classification. *The last fire inspection was conducted on 1/29/24. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. This facility does have children under 12 months enrolled, therefore safe sleep requirements were monitored. The following violations were observed and documented during today’s visit Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted in each classroom during today's visit. The date on the lesson plan posted was 7/29/24 - 8/2/2024. GS 110-91(12); .0508(a) 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Bottles in the infant room had a date of 8/5/24 and a sippy cup was not labeled with a name and date. 15A NCAC 18A .2804(d) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic zip lock bags were stored under the sink in an unlocked cabinet during today's visit. .0604(q) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff member hired on 6/20/24 did not have a medical on file. A staff member hired on 11/29/21 did not have a medical on file during today's visit. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A staff member hired on 11/29/21 did not have a TB test/screening on file during today's visit. .0701(a) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before August 20, 2024, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. HAZARDOUS ITEMS During today’s visit I observed plastic zip lock bags stored in an unlocked cabinet accessible to the children aged one that was in care. The teacher stated that the children broke the cabinet this morning. It is important to keep all plastic bags, materials that can be torn apart, and items that can be swallowed out of reach of children under three years of age. I would recommend storing all hazardous materials 5ft up in a locked device or storage. I would also recommend you visit the website Caring for Our Children pertaining to health and safety. INFANT BOTTLES/SIPPY CUPS During today’s visit I observed bottles with the date of 8/5/24 and a sippy cup that was not labeled with a name or date. I would recommend that before items are placed in the refrigerator that the teacher check that the bottles/sippy cups are labeled properly. STAFF RECORDS During today’s visit a staff member hired on 6/20/24 did not have a medical on file. A staff member hired on 11/29/21 that returned to work for the summer did not have a medical or TB Screening/Test on file. The director stated that she believes she may have shredded the items when the staff member went back to college. I would recommend using the staff file checklist to make sure all items are collected on or before the first day of employment. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety training must be renewed at least every five (5) years. If you completed the training in 2019, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. ONGOING TRAINING HOURS I observed training hours from 8/30/23 to 8/6/24. Please have M. Ghahremani complete the Health and Safety training for Medication on or before 7/15/2025. MEDICATION REMINDERS The following items will expire by the following: J. Galer (Aquaphor) – 9/2024 C. Facility ID: 92002251 Consultant: SHAMEQUA WILKERSON-HARRIS Operation Type: Center Case Number: 0724-266L Visit Date: 7/24/2024 Number Present: 101 Completed Date: 7/25/2024 Age: From 0 To 6 Total Minutes: 102 Time In: 08:00 AM Time Out: 09:42 AM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit is to investigating allegations of non-compliance with the NC Child Care Requirements regarding ratios. A report was received by the agency on 7/23/2024. During the visit, I discussed the allegations with Audrey Carver, the owner. MONITORING I completed a walk-through of the facility. Limited monitoring of child care requirements occurred during today’s visit. I monitored (supervision, child/staff ratios, adequate/approved space, license, and permit restrictions). I observed One hundred and one (101) children ranging in age of infant to six (6) years old engaged in free play, interacting with the teachers, eating breakfast, and completing routine tasks. ALLEGATION There are concerns regarding staff/child ratios not being maintained. OBSERVATIONS/DISCUSSION During today’s visit I observed all classrooms in compliance with ratios. The infant room was observed initially with two children and one staff. An additional staff member arrived at 8:10 am to assist in the infant room once three additional children arrived. I asked the teacher that arrived if this was her normal procedure. She stated for the last two to three weeks she has been covering in the infant room. I asked the teacher if she could show that she could sign in and out of the class. She was able to show me an app on the iPad that allows the teachers to sign in and out. The teacher was able to show that she was signed in during today’s visit. The teacher stated that she doesn’t always sign in and out. During the visit I spoke with the owner concerning ratios. The owner stated that they are overstaffed. She stated that typically they have three (3) teachers in the infant room to assist. I asked the owner if she had any documentation showing that the classrooms were in compliance at all times. The owner was able to give me name to face, attendance, and clock in and out of all the staff. I was able to compare the staff sign in and out, attendance, and name to face for any lapse in ratio coverage during the day. I observed the documentation from Monday, July 8th through Tuesday, July 23rd. RESOLUTION Allegation #1 (Concerns regarding staff/child ratios not being maintained) Based on my discussions, documents received, and observation I am unable to determine that staff/child ratio is not being maintained, therefore, this allegation is Unsubstantiated. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. During today's visit I observed in multiple classrooms that children did not was their hands upon arrival in the classrooms. 15A NCAC 18A .2803(c) 871 Center staff did not comply with the safe sleep policy. SAFE SLEEP During today’s visit I observed an infant sleeping in a bouncy sit. The teacher stated that the child had only been asleep for about five (5) minutes and that she was going to move him to his bed. I discussed with the teacher and the owner the importance of following the safe sleep policy. The owner stated that she would be making sure that all staff are retrained on the safe sleep policy. I would recommend retraining all staff on the safe sleep policy. HANDWASHING During today’s visit I observed in multiple classrooms that handwashing did not take place when the children arrived. The owner stated that she would send out a memo to the parents and retrain all staff concerning handwashing. I would recommend that you visit the website Caring for Our Children concerning proper handwashing and health safety. CONSULTATION The following item were discussed during today’s visit to improve the quality of your program. I discussed with the owner to train and retrain all staff on the process of completing their name to face accurately each day. I also discussed that best practice a member of management should monitor to ensure all staff are completely the name to face accurately each day. This would include teachers signing in and out of each classroom. I discussed with the owner the importance of being able to determine which staff member worked in any classroom and the actual time. Your last annual compliance was on 9-21-22. The center currently operates with a 5-star license, operating at 30 sq. ft. inside, & ratios at .2818 (b). My visit was conducted with you, Mrs. Stroud, Director. A checklist was used to note the requirements monitored. The children were observed playing outside, eating lunch and later taking naps. I monitored for required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, program records, equipment, furnishings, and medications requirements. The facility does not provide transportation; therefore, these requirements were not monitored. VERIFICATION OF RECORDS: *Last fire inspection=1-27-23 (send me your 2024 inspection within 5 days of completion) *Sanitation inspection=3-24-23 (always scan me your inspections) *The center uses The Goddard School’s curriculum, which is an approved curriculum. *EPR plan was last updated on 1-3-23 & EPR training was completed by Dee Stroud on 10-27-15. *The last fire drill was on 7-24-23. The last lockdown drill was on 7-24-23. *The last outdoor playground inspection was completed on 8-7-23 by Dee. Dee completed playground safety training on 5-18-23. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Great start Inc.) listed as current-active. The following violation was observed and must be corrected immediately. Send me a corrective action letter stating how the violation has been corrected by 9-12-23 to michelle.ezzell@dhhs.nc.gov. PROGRAM STANDARDS: Once the assessors are completing assessments again, your education and program standards will be reassessed.
