    Name of Operation: NURTURING FAMILY CHILD CARE TOO Facility ID: 92003665 Consultant: SHAMEQUA WILKERSON-HARRIS Operation Type: Family CC Home Case Number: Visit Date: 10/23/2024 Number Present: 6 Completed Date: 10/23/2024 Age: From 1 To 4 Total Minutes: 90 Time In: 10:30 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to assess applicable child-care requirements during your routine unannounced visit. The Family Child Care Home Providers, Sharon Lowery and Christin Braxton, were present and assisted me during today’s visit. I completed a general walk-through of the children’s space, outside play area, kitchen area and bathroom. At the time of the visit, there were six (6) children present aging in range from one to four (4) years old. The children were observed having free play outside, completing routine tasks, and interacting with the providers. LICENSE STATUS Currently this program operates with a Four-Star License issued August 1, 2019. The program earned seven (7) points in staff education, two (2) points in program standards, and one (1) quality point. MONITORING During today’s visit, a partial assessment was conducted, monitoring all spaces occupied by the children including the outside play environment. The records of the new child were monitored. The providers qualification letters were monitored as well as CPR/First Aid. All spaces were monitored for supervision, child capacity, materials, and equipment. The program records were reviewed for current inspections. The provider does not have children under 12 months enrolled at this time, therefore safe sleep requirements were not monitored. No children enrolled have medications or medical care plans at this time, so these requirements were not monitored. Transportation is not provided; therefore, these requirements were not monitored. --PROGRAM RECORDS *The last documented fire drill was conducted 10/21/24. *The last documented shelter-in-place or lockdown drill was conducted 9/20/24. *The last documented playground inspection was conducted 10/2/24. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 102 Number of preschool children exceeded five children, including the provider's own preschool children. Six preschool age children were observed in care during today's visit. GS 110-91(7)(b) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Lysol was observed in the children's area in reach of children during today's visit. .1719 (a)(7) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before November 6, 2024, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov or mailed to: Shamequa Wilkerson-Harris 300 Sugar Maple Avenue Wake Forest, NC 27587 COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 98%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. FCCH CAPACITY During today’s visit the provider was caring for six preschool age children. The provider stated that one of the students was her grandchild. The provider stated that she was unaware that her grandchild counted within the number of children being cared for during today’s visit. I discussed with the provider that her grandchild caused her to exceed five children because her grandchild is considered a preschool age child. I would recommend considering applying for a capacity change moving forward. This will allow you to accommodate caring for your grandchild and remaining in compliance. I informed the provider if she would like to move forward with a capacity change to contact me when ready. HAZARDOUS ITEM During today’s visit I observed Lysol in the children’s area. I discussed with the provider to make sure that all hazardous items are kept in a locked storage when children are in care. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2019, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules. A checklist was used to note the requirements I monitored today. My visit was conducted with you, Sharon L., Provider. The second provider, Christin B., was not present. Your FCCH operates with a 4-STAR license with a capacity of eight children. There were four children present. I observed them playing with age-appropriate toys and eating lunch. The primary care area of the FCCH is a converted bedroom to the left of the front entrance. You stated, in addition to the primary care area, the children have access to the bathroom located beside the primary care room, and the kitchen during mealtimes. You stated the children play outside in the front yard and there are no pets in the home. You stated you do not provide transportation and no medications are administered at the current time. **VERIFICATION OF RECORDS: *EPR Training was completed by both providers on 3-15-21. *The EPR plan was updated on 10-31-23. *The last fire drill and SIP/Lockdown was on 10-21-23. *Prevention of Shaken Baby syndrome & Abusive Head Trauma Policy was completed by both providers on 8-13-19. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Nurturing Family Child Care LLC) is listed as current-active. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected. Email a signed and dated corrective action letter BY 11-9-23 to michelle.ezzell@dhhs.nc.gov A corrective action letter means you have corrected all the items on non-compliance and is a legal document. Failure to correct any documented violations within the established time, the Division of Child Development & Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The provider could not locate the outdoor inspections for March, April and June 2023. This is a repeat violation. 10A NCAC .1721(e)(5)(A-F) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. One child did not proof of immunizations on file. GS 110-91(1); .1721(a)(2) 912 Written feeding schedule was not on file for each child under 15 months of age and/or was not updated regularly to reflect changes in the child's needs. One infant did not have a written feeding schedule on file. .1706(i) *****TECHNICAL ASSISTANCE/REMINDERS: ***** *Any outlets not being used should have a protective cover over them. *I suggest using the Children’s file checklist that found on DCDEE’s website to avoid missing any children’s records and to understand which forms should be on file the 1st day of attendance vs. 30 days of enrollment. *Health/Safety Trainings are due every 5 years from the completion date. *We discussed how to organize files. I suggest using your filing cabinet for hanging files vs. notebooks. This may be a better way to keep files easily accessible. Files must be accessible during the visit. *Since J. Lowery is 16 yrs. old, a criminal background check (CBC) must be submitted to the criminal record unit before moving back into the home. Rule .2703 *Remember to continue to update your Emergency Preparedness and Response (EPR) Plan every year and print out the cover page showing the current date. Also, print the pages you made changes to. Failure to log onto the website may result in losing your entire EPR plan.
