A checklist was used to note the requirements I monitored today. Today’s visit was conducted with, you, Melanie, Provider. Your FCCH operates with a 1-STAR license by choice, with a capacity of 8 for 1st shift care. The children were observed playing with age-appropriate toys and eating lunch. No infants are currently enrolled. The primary care area of the FCCH is to the left of the front door and the children play in a fenced in back yard. One pet dog is in the home and stays in a crate or outside during operating hours. **VERIFICATION OF RECORDS: *EPR Training completed on=8-20-16. *Annual Professional Dev. Plan was completed on 1-3-24. *The last fire drill was on 4-1-24, the last SIP/lockdown drill was on 3-12-24 and the last outdoor inspection was on 4-11-24. *April 3, 2024, Melanie completed the questionnaire for Asbestos & Paint and submitted water samples to test for Lead. The tests are pending. *Prevention of Shaken Baby syndrome and Abusive Head Trauma Policy=9-18-17. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Meme’s Daycare, LLC) is listed as current-active. Owner and Provider, Melanie Erle’s five (5) year criminal background check expired on March 15, 2024. To maintain compliance with NC laws and rules regarding childcare operation, the following violation must be corrected. Email me a copy of your current criminal background check (CBC) qualifying letter by 5-2-24. If not received on or by 5-2-24, your FCCH may be put on inactive status until your CBC is completed. This means no enrolled children can be in your care/on the premises. Email a copy of your qualifying letter to michelle.ezzell@dhhs.nc.gov Failure to correct any documented violations within the established time, the Division of Child Development & Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). Melanie Erle, Provider's criminal background letter expired on March 15, 2024. A new qualifying letter must be completed and a copy sent to me by 5-2-24. We discussed contacting the CBC unit directly with any issues or questions. The contact information is on the expired letter, which was observed in the FCCH today. *Remember your husband’s criminal background check (CBC) expires June 12, 2024. *Your pets rabies expires 7-24-24. *Remember to continue to update your Emergency Preparedness and Response (EPR) Plan every year and print out any pages you made changes to and the cover page showing the current date. Failure to log onto the website may result in losing your entire EPR plan. A checklist was used to note the requirements I monitored today. Today’s visit was conducted with, you, Melanie, owner/operator. Your FCCH operates with a 1-STAR license by choice, with a capacity of 8 for 1st shift care. There were four children present, they were playing with age-appropriate toys, eating lunch & preparing for nap. The primary care area of the FCCH is to the left of the front door and the children play in a fenced in back yard. You are approved to transport children and drive a Honda van. One pet dog is in the home and stays in the crate or outside during operating hours as indicated on the pet acknowledgement form. **VERIFICATION OF RECORDS: *EPR Training completed on=8-20-16 and the EPR plan was updated on 1-8-23. *Prevention of Shaken Baby syndrome and Abusive Head Trauma Policy=9-18-17. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Meme’s Daycare, LLC) is listed as current-active. *Your last outdoor inspection was on 8-21-23; the last fire drill was on 8-21-23; the last shelter-in-place/lockdown drill was on 6-5-23. The following violation was observed and must be corrected immediately. Send a corrective action letter to michelle.ezzell@dhhs.nc.gov by 9-13-23 indicating how the violation has been corrected. Violation Number Comment Rule 1405 Authorization for up to 12 months to apply over-the-counter medications, topical ointment, topical teething ointment or gel, insect repellents, fluoridated toothpaste, lotions, creams, and powders; such as sunscreen, diapering creams, baby lotion, and baby powder, to a child, when needed, was not in writing and did not contain information required by rule .1720(b)(7). Two children's permission to administer diaper cream and sunscreen did not have the exact name of the diaper cream and sunscreen on the permission slip. 10A NCAC 09 .1720(c)(4) *****TECHNICAL ASSISTANCE/REMINDERS: ***** *We discussed the exact name of the sunscreen or diaper cream should be on the permission to administer medication form.
