The Family Child Care Home Provider, Cynthia Hunter, was present and assisted me during today’s visit. I completed a general walk-through of the children’s space, outside play area, kitchen area and bathroom. At the time of the visit, there were four (4) children present, infant to three (3) years old. The children were observed having free play, singing, completing routine tasks, and interacting with the provider. LICENSE STATUS Currently this program operates with a Three-Star License issued January 31, 2020. The program earned four (4) points in staff education, two (2) points in program standards, and one (1) quality point. MONITORING During today’s visit, a full assessment was conducted, monitoring all spaces occupied by the children including the outside play environment. The provider’s and children’s files were reviewed. All spaces were monitored for supervision, child capacity, materials, and equipment. The program records were reviewed for current inspections. The provider does have a child under 12 months enrolled at this time, therefore safe sleep requirements were not monitored and found not in compliance. No children enrolled have medications or medical care plans at this time, so these requirements were not monitored. Transportation is not provided; therefore, these requirements were not monitored. --PROGRAM RECORDS *The last documented fire drill, shelter-in-place/lockdown, and playground inspection were unable to be monitored. The provider was unable to find documentation. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The outdoor play inspection was not available during today's visit. 10A NCAC .1721(e)(5)(A-F) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. Three children enrolled did not have immunization records on file. GS 110-91(1); .1721(a)(2) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. A child under 12 months did not have documentation of visually checking when the child is sleeping. .1724(a)(8)&(f) 1853 The operator did not conduct a monthly fire drill. The records for the monthly fire drill was not available during today's visit. .1719(a)(15) & .1721( e)(2) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The records for the quarterly lockdown/shelter in place drills were not available to be monitored during today's visit. .1719(a )(16) & .1721(e )(7) 1892 Application was not on file for each enrolled child, including his/her own preschool child(ren) who are not school-age. A child enrolled on 3/3/25 did not have an application on file. .1721(a)(3) To comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be received on or before March 18, 2025, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. CHILDREN FILES During today’s visit a child enrolled on 3/3/25 did not have a file. The provider stated that this was the child’s second day and did not collect all required documentation for the file. Immunization records were not in the file for three children enrolled during today’s visit. A child with health care needs, such as allergies etc., did not have health care needs (medical action plan) documented on the child’s application. I would recommend collecting all required documentation on or before the child’s first day. I would also recommend using the Children’s Checklist. SAFE SLEEP Documentation for safe sleep chart was not completed for a child under 12 months. The provider stated that she thought 15-minute safe sleep charts were not supposed to be completed once a child is able to roll over. I would recommend reviewing your safe sleep policy. I would also recommend viewing Caring For Our Children, https://ncrckids.org, Chapter 3: Health Promotion and Protection concerning safe sleep. A safe sleep documentation chart was left with the provider. Monthly/Quarterly Drills I was unable to monitor the fire drill, emergency drill, and playground inspection. The provider was unable to find the documentation. I would recommend keeping all drills in a binder or placing them on the wall for easy access. I would also recommend that you use your outlook calendar or a reminder to remind you when the drills are due. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2020, you should begin to retake the training now. CLEAN CLASSROOMS FOR CAROLINA KIDS With the recent impacts of Hurricane Helene and other natural disasters, the Clean Classrooms for Carolina Kids enrollment deadline has been revised statewide, and schools and child care facilities that have not yet enrolled should complete the online process by May 31, 2025. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date.
