Name of Operation: Lightbridge Academy of Wake Forest Facility ID: 92004043 Consultant: SHAMEQUA WILKERSON-HARRIS Operation Type: Center Case Number: Visit Date: 5/20/2026 Number Present: 143 Completed Date: 5/20/2026 Age: From 0 To 5 Total Minutes: 245 Time In: 09:40 AM Time Out: 01:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to assess applicable child-care requirements during your annual compliance visit. A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Tammy Hicks, the Director. You and the Assistant Director, Amanda Brown, assisted me with today’s visit. There was a total of one hundred-forty-three (143) children present, aging in range from infants to five old. The children were observed playing in centers, doing a group activity, participating in water play, eating lunch, napping, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued February 11, 2026, through the Program Assessment Pathway to the Stars. The center is currently licensed for 1st and 2nd shift meeting enhanced ratios. I reviewed the NC Secretary of State’s website and observed the owner of the facility 626 Wake Forest LLC listed as current/active. INSPECTIONS *The last fire drill was conducted on 4/28/26. *The last emergency drill was conducted 2/9/26. *The last playground inspection was completed 5/12/26. *The last sanitation was completed on 9/19/25 with a ‘Superior’ classification. *The last fire inspection was conducted on 2/6/26. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. I reviewed twelve staff, qualification letters and 14 enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored and found in compliance. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space #12 three diaper cream ointments did not have written authorization from a parent or authorized health professional. 10A NCAC 09 .0803(1)(a & b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. A prescription medication did not have a prescription label or in it's original box. .0803(2)(a) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before June 3, 2026, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 97%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Technical Assistance The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. MEDICATION In space #12 it was observed that a prescription medication was not in its original box or with the prescription label. Three diaper cream ointments did not have a parent authorization form. I discussed with the administrators several different options that can be used to maintain compliance. I would highly recommend an administrator checking all medications in each room biweekly for completion of forms and prescription medications. I would also recommend having medications left at the office for review before sending to the classrooms. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the training in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules. I spoke with Amanda Brown, the Assistant Director. LICENSE STATUS Currently, this Child Care Center operates with a five (5) star license issued November 20, 2025. The Assistant Director discussed with me through a phone call what she witnessed while conducting camera observations. It was discussed that the Assistant Director saw concerns of a teacher not handling a child in an appropriate manner in the three/four classroom. The child was exhibiting disruptive behaviors by throwing the soft chairs in the cozy area. The Administrators required the teachers to take five training courses pertaining to classroom management, positive reinforcement and supervision. Based on the self-report, interviews with staff members, and screenshot pictures, it was determined that lapses in compliance with the Child Care Requirements have occurred, therefore the concerns related to a child inappropriately being handled. The following violation was documented. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A child was not handled in an appropriate manner while being redirected in the three/four classroom. A compliance letter must be received on or before December 24, 2025, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. One of the biggest challenges that early childhood educators and administrators face is children’s behavior. Often these behaviors are developmentally appropriate, however, some behaviors can be difficult for a teacher to manage. Providing technical assistance, coaching and professional development for staff to prevent and address these challenging behaviors, as well as promote children’s social competence can be beneficial. If you would like assistance with dealing with challenging behaviors of children in your care, you can contact a Healthy Social Behavior Specialist at Early Years; they can be reached at (919) 403-6950. I would recommend all staff receiving trainings to assist all teachers with appropriately responding to challenging behaviors in the classroom. I would also retrain staff members on the appropriate way to move a child. It is important not to grab or pull on a child who is refusing to move. It is possible for an injury to occur such as nursesmaid’s elbow. Injuries like this can occur by pulling and grabbing a child in the wrong manner or to help a child avoid a dangerous situation. Nursesmaid’s elbow is a common injury among toddlers and preschoolers. I would also recommend continuing to do frequent camera observations. Completing frequent camera observation can allow you to provide proper training resources to all teachers. I would also recommend documenting and discussing the observations with the teachers.
