Upon arrival I was greeted by Carrie Goodall, the Director. I completed a general walk-through of the center. At the time of the visit, there was a total of One hundred four (104) children present, ranging in age from infant to five (5) years old. The children were working on art projects, playing with materials, interacting with the teachers, eating lunch, playing outside, preparing for rest time, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Five (5) star license issued August 2, 2024, earning an accreditation through NAEYC. I reviewed the NC Secretary of State’s website and observed the owner of the facility Kindercare Education LLC listed as current/active. INSPECTIONS *The last fire drill was conducted on 2/25/26. *The last emergency drill was conducted on 2/10/26. *The last playground inspection was completed 2/25/26. *The last sanitation was completed on 9/19/25 with a “Superior ‘’ classification. *The fire inspection was completed on 7/1/25. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. 10% of old staff, all new staff CPR/FA, qualification letters, and 10% of enrolled children’s files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. Children in space #6 were observed not washing their hands after outside play. 15A NCAC 18A .2803(c) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space #3 it was observed that three diaper creams and in space #8 two medications did not have written authorization from the parent. 10A NCAC 09 .0803(1)(a & b) 847 Parent's medication authorization did not include required information. It was observed in space #2, #3, and #8 fifteen authorization forms for medication/diaper cream without valid authorization dates and instructions on how to use the medication/diaper cream. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. An EpiPen with an expiration date of 3/9/2026 and sunscreen with an expiration date of 12/2025 was not returned to the parents. .0803(12) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before April 7, 2026, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 90%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. EMERGENCY MEDICATION (EPIPEN)/ MEDICATION/DIAPER CREAM In space #5, the EpiPen for a child was observed expired with a date of 3/9/2026. The director stated that she will have the parent bring in a new EpiPen once the child returns to school. Sunscreen for one child was observed with an expiration date of 12/2025. In space #2, #3, and #8, fifteen diaper cream forms did not have a valid authorization date, and clear instructions on the usage of the diaper cream. In space #3 and #8, five diaper creams did not have an authorization form the parent on file. I discussed with the Administrator and teachers the importance of having the appropriate information on diaper cream and prescription medical cream forms. I would recommend having all medications and authorization forms dropped off with an administrator to review before putting them in the classrooms. I would also suggest checking all forms and medications bi-weekly or monthly for compliance. HANDWASHING I observed children transitioning from outside play to the classroom in space #7. The children did not wash their hands. The teachers had the children get their blankets and mats and place them in their rest areas. I discussed with the Administrator and teachers to have them wash their hands before touching items in the classroom. Proper handwashing can assist in lowering sickness in the center. I would recommend contacting Latasha Hopkins, the Child Care Health Consultant, to review proper handwashing procedures with all your staff. ONGOING TRAINING I observed training hours from 4/15/254 – 3/24/26. All training hours were in compliance. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. Violation Number Comment Rule 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. On April 28, 2025, a staff member grabbed a three-year-old child resulting in an injury to the child’s torso. On August 18, 2025, the same staff member handled a fifteen-month-old child roughly resulting in abrasions and bruises to both sides of the child’s body. On other occasions a staff member grabbed, yanked and pulled children by their arms. .1803(a)(1) 1810 There was a substantiation of child maltreatment. A report was received by the agency on 9/12/24. During the visit, I discussed the allegations with Carrie Goodall, Director. MONITORING I completed a walk-through of the facility. Limited monitoring of child care requirements occurred during today’s visit. I monitored (supervision, child/staff ratios, adequate/approved space, license, and permit restrictions). I observed the children engaged in free play, interacting with the teachers, eating breakfast, and completing routine tasks. ALLEGATION There are concerns regarding staff child ratio not being followed in the classroom for two-year-old children in the early morning hours. OBSERVATIONS/DISCUSSION During today’s visit I observed the Director instructing teachers to check classrooms to make sure the classrooms were in ratio. I observed in Toddler 1 classroom eight children one staff and two parents lingering around at drop off. The Director asked a staff member to be in ratio in the toddler 1 classroom so that the classroom would remain in compliance. During the discussion with the Director, I asked about if staffing was an issue. The Director stated that they have been fully staffed since the beginning of the summer. She informed me that last week (9/9/24 – 9/13/24) she had eight call outs, and staffing was a challenge. The Director from another Kindercare assisted with opening the building because of the Assistant Director being on vacation. I asked the Director to explain the process used to notify when the classes are out of ratio. It was stated that the management team are constantly checking each classroom, however, the teachers will notify by calling the front desk to let them know that they are out of ratio. The Director stated that it normally takes one to two minutes to bring the class into compliance. I interviewed three random teachers. I asked the teachers if they had any concerns with ratios. I also asked the teachers what the process used if they go out of ratio. All three teachers stated that they do not have any ratio concerns. The teachers stated that they call to the front desk to let them know that they are out of ratio. The teachers stated that it typically takes anywhere between one and two minutes to get them into compliance. I requested the name to face transitional tracking and the staff clock in and out documentation forms. I observed the documents and found non-compliance of staff child ratio in classroom Three’s B (which consisted of all two’s and one three-year-old) on 9/9, 9/10, 9/11, 9/12 and 9/13. The staff child ratio each day is documented 1:12 and 1:16 from 8:20am until 10:40am. I also observed non-compliance in the Toddler 2 classroom on 9/9 between 7:00am until 8:00am. The staff child ratio for the toddlers was 1:7. RESOLUTION Based on my discussions and observations it has been determined that a concern regarding staff child ratio did occur. The allegation has been SUBSTANTIATED. Due to a violation and substantiation of the complaint, a return visit will be made to the facility to monitor ongoing compliance. Please be advised that this facility may be subject to administrative action. The following violation was observed and documented during today’s visit. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Documents observed from last week September 9, 2024 - September 13, 2024 showed lapses in staff/child ratios for the Three's B classroom for September 9th, September 10th, September 11th, September 12th and September 13th. Lapses in staff/child ratio occurred on September 9th for the Toddler 2 Classroom. CHILD STAFF RATIO Required staff-child ratios for all children is essential for their safety. Lower ratios are most critical from birth to twenty-four months. Documents reviewed today indicated lapses in the required minimum ratios in your Toddler 1 classroom and your Three’s B classroom which consisted of two-year-olds. Center staff should notify the administrator when they reach the maximum number of children permitted, so that staff child ratios are maintained. During today’s visit I observed eight children in the toddler classroom with one teacher, however, the parents of the last two children stayed in the classroom until a second teacher was asked to be in ratio. I would recommend discussing with staff that if another child arrive before a second staff member is present parents should be asked to wait until coverage can be provided. Keep in mind that staff/child ratios are based on the youngest child present in the classroom. Since you are fully staffed, I would also suggest that you do a time study to adjust staff schedule.
