Facility ID: 92000840 Consultant: MICHELLE EZZELL Operation Type: Center Case Number: Visit Date: 12/18/2024 Number Present: 43 Completed Date: 12/18/2024 Age: From 0 To 10 Total Minutes: 195 Time In: 10:00 AM Time Out: 01:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance during your annual compliance visit. A checklist was used to note the requirements monitored. The center currently operates with a 3-star license, operating at 30 sq. ft. inside, & ratios at .0713 (a). My visit was conducted with Kashina Marrow, Director. The children were observed playing outside, preparing for lunch and eating lunch. One infant was sleeping, while others were playing with age-appropriate toys and being held by the providers. I monitored for required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, program requirements, menus, allergy lists, equipment, furnishings, medications, and transportation requirements. All infant files and four new staff files and two seasonal staff files were monitored. VERIFICATION OF RECORDS: *Last fire inspection=9-9-24 *Sanitation inspection=4-4-24 *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Kids Educational Center III, Inc.) listed as current-active. *The last playground inspection was completed on 11-18-24 by Ms. Marrow. She completed playground safety training on 7-9-19. *The last fire drill was on 11-18-24 & SIP/Lockdown was on 11-27-24 *EPR plan was last updated on 3-12-24; EPR training was completed by Dare Woodruff on 10-27-15 & Ms. Marrow on 8-26-21. *The center uses two chevy minibuses to transport children and the insurance and registration were current. *The last lead water test was completed on 2-21-24 & will expire 2-21-27. *The following violation was observed, and technical assistance was given. This violation was corrected during the visit; no corrective action letter is required. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In space four (4) a staff's handbag was hanging below 5 ft. on the back of the door; in space five (5) a bag of liquid soap that had "keep out of reach of children" on the label was in an unlocked cabinet located under the handwashing sink. The Director moved the staff's bag to a shelf over 5 ft. high in the classroom and she removed the soap from the classroom. 10A NCAC 09 .0601(a) *****TECHNICAL ASSISTANCE/REMINDERS: ******* *Storage of Hazardous Items- Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. A bag of soap that requires locked storage were improperly stored in an unlocked cabinet in space (5) five and a staff’s personal bag was hanging on the back of a door, below 5 ft. in space (4) four. It is extremely important that staff are taking the required time to lock doors, store hazardous items properly either in locked storage or five feet up from the finished floor. *PLAYGROUND-We discussed moving the white border about 5 feet out from the large slide, to allow the children additional room to exit from the slide. *INFANT & TODDLERS PRIMARY CAREGIVERS-We discussed the infants and toddlers having assigned primary caregivers. These were posted during the visit. ****Rated License Reminders***** *We discussed updates for the rated license process will be sent out from DCDEE by email in 2025. Remember to have staff’s education information on file with DCDEE/Workforce and their information must be current in WORKS database. Staff’s education standards are based on current data on file in WORKS. We’ve discussed this in detail during your previous visits. You stated you are keeping your staff’s education information current in WORKS and at the current time, you do not want ERS. My visit was conducted with you Ms. Morrow, Director. The facility currently operates with a 3-star license. Your last annual compliance visit was on 6-20-23. I observed the children playing outside and preparing for lunch. ***Verification of Records & Training*** *The last outdoor inspection was on 1-19-24 by K. Helms. K. Helms completed Playground Safety training on 10-23-21. *Your EPR plan was last updated on 3-27-23. EPR training was completed by D. Woodruff on 10-27-15. *The last fire inspection was on 8-11-23 & the last sanitation inspection was on 10-16-23. Violations-already written on form: The following violations were observed and corrected during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An electrical outlet, not in use was not covered with a safety outlet. 10A NCAC 09 .0604(c) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. One unlocked cabinet in a classroom located under the handwashing sink, there were several bags of liquid soap and one box of Scrub Buddies quick eraser. These items had Keep out of reach of children on the warning label. These items were placed in a locked cabinet during my visit. .2820(b) *We discussed electrical outlets not in use should be covered with child safety plugs. *Any items that have “keep out of reach of children” on the label should be locked or kept at least five ft. high, out of reach. ****RATED LICENSE EDUCATION RESOUCRES*** *The center’s license falls under COHORT 1. Your planning year will end on 6-30-24 and your reassessment year will end 6/30/25. *Early Educator Certificate (EEC)=The EEC certificate is optional for staff. Although, it may increase staff’s education points and may help keep or increase the number of STARS the facility has.
