Upon arrival I was greeted by Vanessa Adams, the Director. You assisted me with today’s visit. I completed a general walk-through of the indoor and outdoor learning environment. At the time of the visit, there were a total of 141 children present, ranging in age from infants to five (5) years old. The children were observed in circle time, playing in centers, eating breakfast, playing outside, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a four (4) star license issued June 7, 2022, earning seven (7) points in education, two (2) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility (KAWF Corp) listed as current/active. INSPECTIONS *The last fire drill was conducted 3/31/26. *The last emergency drill was conducted 3/16/26. *The last playground inspection was completed 3/31/26. *The last sanitation was completed on 9/16/2025 with a “Superior ‘’ classification. *The fire inspection was completed on 10/27/25 and the facility is approved for daytime care only. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of three newly hired staff. I also monitored five children’s files. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. Animal feces was observed on the infant/toddler playground. 15A NCAC 18A .2832(a) 847 Parent's medication authorization did not include required information. Parent authorization forms did not include required information forms for children in room #1 and room #9. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Aquaphor was observed with an expiration date of 5/2025 in room #9. .0803(12) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. A drink from Chick-Fil-A and iced coffee from Dunkin Donuts was observed in the view of the children in space #5. .0901(i) 1867 The depth of the loose surfacing was not based on critical height of the equipment. It was observed that the mulch measured less than an inch under the wall climber. .0605(k)(1-4) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before April 23, 2026, the letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 90%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. NUTRITION I observed a Chick-Fil-A drink and Dunkin Donut iced coffee stored in view of the children in room #5. It is important that staff model the appropriate nutritional requirements as specified in the Meal Patterns for Children in Child Care Programs. I would recommend reviewing the nutritional requirements with all staff. I would also recommend teachers store their personal drinks and food in an opaque container or in a closed shelf. OUTDOOR ENVIRONMENT The infant/toddler playground was observed to have animal feces on the playground during today’s visit. The administrator stated that small animals come on the playground after hours. I would recommend having a member of management or staff checking the playground before children transition to the playground. Mulch was observed to be needed under the wall climber. The fabric under the mulch is showing. The mulch did not reach about an inch in the fall zone. I would recommend when doing your playground checks to measure the depth of the mulch monthly. MEDICATION In room #1 and room #9 diaper cream/topical cream was observed not to have appropriate instructions on the authorization form. In room #9 a topical cream was observed with an expiration date of 5/2025. The administrator stated that the child was disenrolled two weeks ago. I would recommend having all medications reviewed by administrators before being placed in the classrooms. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety trainings are now available through Moodle on the DCDEE website. These trainings are provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the trainings in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Vanessa Adams, the Director. There was a total of One hundred twenty-one (121) children present, aging in range from infants to five (5) years old upon arrival. The children were observed playing in centers, playing outside, eating lunch, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a four (4) star license issued June 7, 2022, earning seven (7) points in education, two (2) points in program standards and one (1) quality point. I reviewed the NC Secretary of State’s website and observed the owner of the facility (KAWF Corp) listed as current/active. INSPECTIONS *The last fire drill was conducted 7/31/25. *The last emergency drill was conducted 6/4/25. *The last playground inspection was completed 7/31/25. *The last sanitation was completed on 12/10/24 with a ‘Superior’ classification. *The last fire inspection was conducted on 10/29/24. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters, a percentage of old staff and children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. This facility does have children under 12 months enrolled; therefore safe sleep requirements were monitored. During today’s visit the administrator discussed with me a child was left unsupervised on the playground for about 2 minutes. The parent and another teacher noticed that the child was outside playing alone. The teacher brought the child in and notified the administrator. The administrator discussed with me that they have updated the measures with transitions. The teachers are now using walkie talkies to notify administrators when a transition will occur to be assisted. A mandatory staff meeting was held on 8/20/25 reviewing the name to face policy and transitions. During the visit I observed the staff meeting agenda. I also observed the walkie talkies, the administrators assisting during transitions, and name to face being used. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 303 Children were not adequately supervised at all times. A child one year of age was left unsupervised on the playground on 8/18/25. .1801(a)(1-5) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space #6 Boudreaux's Butt Paste was observed with an expiration date of 7/2025. In space #7 Desitin was observed with an expiration date o f 7/2025. In space #9 Blue Lizard Sunscreen was observed with an expiration date of 6/2025 and a form authorization date ended on 4/8/25. .0803(12) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff member hired on 6/2/25 did not have a health assessment on file. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A staff member hired on 6/2/25 did not have TB test or screening on file during today's visit. .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Four new staff files observed did not have orientation documentation completed. The staff members were hired on 5/19/25, 7/1/25, 7/14/25, and 11/7/24. .1101(a) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. NON-PRESCRIPTION/PRESCRIPTION I observed expired parent authorization forms space #9 with an authorization date ending on 4/8/25. Blue Lizard Sunscreen was observed with an expiration date of 6/2025. In space #7 Desitin was observed with an expiration date of 7/2025. In space #6 Boudreaux’s Butt Paste was observed with an expiration date of 7/2025. I would recommend having a member of management check monthly for expired medications and forms. I would recommend that you view the website Caring for our Children (https://nrckids.org/CFOC regarding Children with Special Health Care Needs. STAFF ORIENTATION Staff qualifications and training requirements are essential for your staff who interact with children. During today’s visit I observed four new staff files that did not have the staff orientation documentation completed. Each staff person needs to receive 16 hours of orientation within the first 6 weeks of employment. Six of the hours are required to be completed within the first two weeks of employment. When documenting orientation, please ensure that all required information is entered onto the form. STAFF FILES I observed one staff member hired on 6/2/25 that did not have a health assessment or tb on file. I would recommend making sure you collect all needed information on the first day of employment. Once collected please make sure to file all health-related documentation in a separate file from the personnel file for each staff member. SUPERVISION Adequate supervision of children is required by Child Care Rule .1801. Appropriate supervision of children is important for their safety, injury prevention, and making sure of quality child care. It can protect children from physical injury, harmful conversations, or inappropriate behavior like teasing and bullying. A child in the toddler classroom was left on the playground unattended on 8/18/25. I would highly recommend that you continue to retrain all staff on your updated procedures and policies for transitions. I would also recommend having a member of management check the name to face throughout the day. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. There was a total of One hundred fourteen (114) children present, aging in range from infant to five (5) years old upon arrival. The children were observed playing in centers, playing outside, eating breakfast and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a four (4) star license issued June 7, 2022, earning seven (7) points in education, two (2) points in program standards and one (1) quality point. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. A bottle in Infant A and a bottle in Infant B did not have a current date labeled on the bottles. 15A NCAC 18A .2804(d) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Diaper creams in Infant A, Infant B, and Toddler 1 did not have specific instructions from a child's parent documented on the form. 10A NCAC 09 .0803(1)(a & b) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff member had expired on First Aid training of 7/19/24. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff member had an expired CPR training of 7/19/24 on file during today's visit. .1102(d) 1898 Staff did not complete the health and safety training within one year of employment. Staff member employed on 6/10/23 did not complete the Health and Safety training within one year of employment. CPR/FA I observed CPR/FA expired for one staff member. The expiration date observed was 7/19/24. I would suggest that you use your outlook calendar or a reminder to let you know when staff will need to complete CPR/FA. EMERGENCY PREPAREDNESS AND RESPONSE PLAN The EPR plan was observed not to be updated with current agency information. I would recommend updating when things change as well as annually. HEALTH AND SAFETY TRAINING During today’s visit I observed a staff member hired on 6/10/23 and a staff member hired on 1/8/2022 with Health and Safety trainings not completed. I would suggest that you use an outlook calendar for new hires to remind you of when the Health and Safety trainings are due. Please be reminded that the Health and Safety training is due within the year of the hire date. The Health and Safety training must be renewed every five years thereafter. NON-PRESCRIPTION/PRESCRIPTION I observed expired parent authorization forms in space in infant A, infant B and Toddler 1 with parent authorization forms without specific instructions on how and when to apply the diaper cream. I would recommend checking all creams weekly to ensure that all forms are in compliance. INFANT BOTTLES During today’s visit I observed a bottle in infant A and a bottle in Infant B not labeled with a date. I would recommend that teachers check after parents to make sure all bottles are labeled with a name and the current date. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. The center currently operates with a 4-star license, operating at 35 sq. ft. inside, & ratios at 0713 (a). My visit was conducted with Vanessa, the Director and Suchita, Owner. A checklist was used to note the requirements monitored. The children were observed playing outside and later taking a nap. The center does not transport children. VERIFICATION OF RECORDS: *Last fire inspection=11-17-22 (send me your 2023 inspection within 5 days of completion) *Sanitation inspection=6-14-23 (send me your 2023-2024 inspections) *EPR training was completed by Vanessa on 6-21-18. *The EPR plan was last updated on 10-6-23. The last Lockdown drill was on 9-21-23. *The centers last lead test was on 11-18-21 *The last fire drill & outdoor playground inspection was on 9-29-23. Vanessa completed playground safety training on 11-29-22. *The center uses Kiddie Academy Curriculum. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (KAWF Corp) listed as current-active. Technical Assistance was offered on how to correct the violations cited and how to maintain compliance. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately. Email a corrective action letter BY 10-30-23 to michelle.ezzell@dhhs.nc.gov Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. In several classrooms candy and toys brought from home were located in children's cubbies. 10A NCAC 09 .0601(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In several classrooms sunscreens and diaper creams were located in children's cubbies. 15A NCAC 18A .2820(d) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. In several classrooms, plastic bags were located in children's cubbies. .0604(q) *****TECHNICAL ASSISTANCE/REMINDERS: ******* **PAPERWORK** *We discussed how to organize children’s records. *Transfer the notes and updates made on your staff & training worksheets to your electronic version. **WALK-THRU** *We discussed plastic bags cannot be accessible to children under 3 years old and different ways to store clean and soiled clothes so plastic bags are accessible. *Sunscreens and diaper creams were stored in a few children’s cubbies. We discussed talking with parents to make sure they’re not putting these items in their child’s cubby; these items must be kept out of reach of children or locked. *Remember to continue to update your Emergency Preparedness and Response (EPR) Plan every year and print out the cover page showing the current date. Also, print the pages you made changes to. Failure to log onto the website may result in losing your entire EPR plan. Although, it may increase staff’s education points and may help keep or increase the number of STARS the facility has.
