    Name of Operation: HOPE LUTHERAN PRESCHOOL Facility ID: 92003271 Consultant: SHAMEQUA WILKERSON-HARRIS Operation Type: Center Case Number: Visit Date: 11/13/2025 Number Present: 135 Completed Date: 11/13/2025 Age: From 2 To 5 Total Minutes: 165 Time In: 11:15 AM Time Out: 02:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to assess applicable child-care requirements during your annual compliance visit. A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Angela Whitted, the Director. You assisted me with the visit. There was a total of One Hundred-thirty-five (135) children present, aging in range from two (2) to five (5) years old. The children were observed playing in centers, interacting during circle time, working on art projects, playing outside, eating lunch and completing routine tasks. LICENSE STATUS Currently, this facility operates with a Notice of Compliance issued on 5/2/2016. I reviewed the NC Secretary of State’s website and observed the owner of the facility Hope Lutheran Preschool, Inc. listed as current/active. INSPECTIONS *The last fire drill was conducted 11/3/25. *The last emergency drill was conducted on 10/14/25. *The last playground inspection was completed 11/6/25. *The last sanitation was completed on 12/6/24 with a ‘Superior’ classification. *The last fire inspection was conducted on 9/16/25. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters, a percentage of old staff and children’s files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. This facility does not have children under 12 months enrolled, therefore safe sleep requirements were not monitored. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. Shelving in space #4, #7, and #8 were not in good repair. The shelves were observed with chipping paint, chipping wood, splinters, and were not sturdy. .0601(c) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff member hired on 8/11/25 had a medical report on file. However, the question regarding if the applicant is emotionally and physically capable of working with children stated that the applicant was not emotionally and physically capable. 10A NCAC 09 .0701(a) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before November 27, 2025, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 92%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. STAFF MEDICAL A staff medical examination was observed on file, however stating that the applicant is not emotionally and physically capable of caring for children on a daily basis. It is important to observe the medical report fully to ensure that each applicant is capable emotionally and physically to care for children. Ms. Whitted has asked the staff member to return to the doctor to receive an updated medical report before returning to work. INDOOR EQUIPMENT AND FURNISHINGS The shelves in space #4, #7, and #8 are not in good repair and are not sturdy. The shelves have chipping paint, chipped wood, and are easily pushed over. It is important to make sure shelving, and any furniture is free of sharp edges, splinters, chipping paint, protrusions and pinch points. This will assist in making sure all children are safe. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. The facility holds a notice of compliance and does not serve children with subsidy therefore, the staff are not required to complete the health and safety trainings. Although you are not required to complete the health and safety trainings you may access the trainings in MOODLE on the DCDEE website. CLOSING CENTERS IN CLASSROOMS Although your facility holds a notice of compliance it is best practice that all your centers are open throughout the day. I observed several centers in space #1, #2, #4, and #5 closed. Giving the children a wide range of centers and activities to choose from will assist in behavior management, self-directed play, social-emotional growth, and uninterrupted creative play. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. AGENCY UPDATES Please remember to check the Division of Child Development and Early Education website (ncchildcare.gov) on a weekly basis to stay up to date on all new policies and rules.
