A checklist was used to note the requirements monitored today. Upon arrival I was greeted by Myisha Hopkins, the Assistant Director. The Director, Peggy Sackett, assisted me with today’s visit. There was a total of one hundred one (101) children present, aging in range from infants to five years old. The children were observed playing in centers, playing outside, doing a group activity, participating in circle time, eating lunch, napping, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Four (4) star license issued June 29, 2022, earning seven (7) points in education, two (2) points in program standards and one (1) quality point. This center operates at 30sq. ft. inside, & ratios at .0713(a). I reviewed the NC Secretary of State’s website and observed the owner of the facility Premier NC Wake Forest LLC listed as current/active. INSPECTIONS *The last fire drill was conducted 1/15/2026. *The last emergency drill was conducted 12/8/2025. *The last playground inspection was completed 2/5/2026. *The last sanitation was completed on 4/14/2025 with a ‘Superior’ classification. *The last fire inspection was conducted on 3/4/2025. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. The following violations were observed and documented during today’s visit. Violation Number Comment Rule 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. A child was observed not washing their hands when arriving in the classroom in space #4. 15A NCAC 18A .2803(c) 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. It was observed in Adventures, Climbers, and Wee School chipping paint on the lower half of the walls. The three classrooms care for children from one to two years of age. 15A NCAC 18A .2825(a) 847 Parent's medication authorization did not include required information. A form was observed with an expired authorization date of 1/14/26 in the K-Ready 1 classroom. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Aquaphor was observed with an expiration date of 12/2025 in space #7 (Adventures). .0803(12) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before March 4, 2026, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 89%. According to NC General Statue 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. CLASSROOM WALLS During today’s visit I observed the wall in Adventures, Climbers, and Wee School classrooms with chipping paint on the lower half of the walls. The Director stated that a work order has been placed to paint the classrooms. I would recommend that you review Chapter 5 (Facilities, Supplies, Equipment and Environmental Health) with Caring For Our Kids at https://nrckids.org. It is very important to make sure that all walls are free from chipping paint and are in good repair. PRESCRIPTION/NON-PRESCRIPTION MEDICATION Expired medication was observed in Space #7 in Adventures. Aquaphor was not returned to the parents or thrown away once expired. The expiration date observed was 12/2025. In Space #5 (K-Ready 1) I observed an authorization form with authorization date to end on 1/14/2026 for Aquaphor. I would recommend having a member of the management check all medications weekly or bi-weekly to make sure all forms and medications are complete and not expired. HANDWASHING A child was observed being dropped off in space #4. The child did not wash their hands when entering the classroom. I discussed with the teachers to make sure all children wash their hands upon arrival. Proper handwashing can lower the spread of sickness in the center. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. QRIS MODERNIZATION The administrators have chosen to complete Pathway 1. It was discussed with me that they would like to go through the process in mid-August. The center has already reached out to NCRLAP for additional help. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the training in 2021, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. ONGOING TRAINING HOURS Training hours were monitored from 2/18/25 to 2/18/26. Upon arrival I was greeted by Kayla Nowak, the Director. You assisted me with today’s visit. The Area Manager, Shea Cochran arrived within an hour and a half. I completed a general walk-through of the indoor and outdoor learning environment. The Written Warning Administrative Action was reviewed with the Director, Kayla Nowak, and Area Manager, Shea Cochran during the visit. At the time of the visit, there were a total of 81 children present, ranging in age from infant to five (5) years old. The children were observed eating breakfast, playing outside, participating in circle time, playing in centers, completing routine tasks, and enjoying a buggy ride. LICENSE STATUS Currently, this Child Care Center operates with a Four (4) star license issued June 29, 2022, earning seven (7) points in education, two (2) points in program standards and one (1) quality point. This center operates at 30sq. ft. inside, & ratios at .0713(a). I reviewed the NC Secretary of State’s website and observed the owner of the facility Premier NC Wake Forest LLC listed as current/active. INSPECTIONS *The last fire drill was conducted on 7/29/25. *The last emergency drill was conducted 6/6/25. *The last playground inspection was completed 8/18/25. *The last sanitation was completed on 4/14/25 with a “Superior ‘’ classification. *The fire inspection was completed on 3/24/25 and the facility is approved for daytime care only. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. Ms. Nowak informed me that she does not have any new staff, therefore I did not monitor any newly hired staff files. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. I observed the notification of the administrative action posted in the front lobby behind the front desk. The following violations were observed and documented during today’s visit: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. In space #4 the parent's medication authorization form was observed without the expiration date, the time frame to administer medication, and how the medication should be applied. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space #3 Aquaphor was observed without a parent authorization form and was not returned to the parent. In space # Parent's Choice Diaper Rash Ointment was observed with an expiration date of 12/2023. .0803(12) 1912 The written feeding plan did not include the type of milk, formula or food and/or the frequency of the feedings. Failure to do so may result in the issuance of a provisional license or other administrative action. TECHNICAL ASSISTANCE The following items were discussed during today’s visit to help you maintain quality within your program and to prevent further violations. MEDICATION During today’s visit I observed in space #3 Aquaphor for a child without written authorization and instructions from the parent/guardian. In space #4 parent’s medication authorization did not include all required information. I observed four authorization forms without expiration date, authorization time frame, and how the medication should be applied. In space #4 Parent’s Choice Diaper Rash Ointment was observed with an expiration date of 12/2023. I would highly recommend having a member of management check all diaper creams monthly. I would also recommend that a member of management check all medications and forms before sending to the classrooms. INFANT FEEDING SCHEDULE During today’s visit I observed an infant feeding schedule without the frequency of feeding and type of milk. I would recommend checking the feeding schedules monthly to ensure all feeding schedules are in compliance and updated as needed. CONSULTATION AND REMINDERS The following items were discussed during today’s visit to improve the quality of your program. Upon arrival, we were greeted by the Director, Kayla Nowak. You assisted me with today’s visit. I conducted a general walkthrough of the center. At the time of the visit, there were a total of 57 children present, ranging in age from infants to six (6) years old. The children were observed playing in centers, interacting with the teachers, eating breakfast, and completing routine tasks. During today’s visit I monitored child staff ratio, and interactions with teachers and children. While completing my walkthrough I observed a five-year-old child grouped with children younger than 24 months of age in space #3 at 8:20am. The child was taken to the K-Prep 2 classroom. The child had remained with the parent who is the teacher for space #3. The parent (teacher) arrived at 7:00am. While completing the walkthrough a teacher in space #1 asked the Director if she could have more than nine children since she only had one two-year-old child. The Director stated yes. I discussed with the Director and teacher that they would have to run the ratio of the youngest child in the classroom which would be one staff member to nine children. I suggested to maintain ratio this morning, the two-year-old child could be shifted to the two-year-old classroom until the second teacher arrived. The following violation was observed and documented during today’s visit. Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. A five year old child was observed grouped with children younger than 24 months old in space #3 at 8:20am during today's visit. 10A NCAC 09 .0713(a)(6) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. You may want to consider them to avoid violations in the future: -Technical assistance was given to Ms. Nowak regarding the arrival time of her staff based on the arrival time of the children. Arrival time is not always predictable for each child daily; therefore, I would highly recommend completing a time study over a week basis to determine staff schedules. Please be mindful that moving the oldest or youngest child each morning to the next classroom to meet staff/child ratios is not best practice. When children arrive at school they should be allowed to arrive in their classroom and not have to move to another classroom because of not having enough staff present on site. I would also recommend meeting with all staff to discuss ratios and grouping of children. In the discussion I would recommend using the staff child ratio sheet. I would also recommend reviewing the staff child ratio and grouping rules. Upon arrival, we were greeted by the Assistant Director. The Director, Kayla Nowak, assisted with the visit. We conducted a general walkthrough of the center. We then discussed the nature of the visit. Amanda Annunziata, Child care consultant, accompanied me on this visit. At the time of the visit, there were a total of 96 children present, ranging in age from infants to five (5) years old. The children were observed playing in centers, interacting with the teachers, playing outside, participating in soccer shots, eating lunch, and completing routine tasks. While conducting the walk-through I asked the Director if she has had staff turnover. The Director stated that she terminated a teacher last Friday. However, today she has a number of teachers that have called out. The Director was asked if they have had child staff ratio issues. The Director stated that she has had staffing challenges, however, she has not had any child staff ratio issues. It was discussed with me that they have been able to maintain ratios by putting herself and the assistant director in ratio when needed. I requested documentation of the name to face documentation for May 20th and 21st. I also interviewed the teacher in the K-Ready 2 classroom regarding ratios and group size. The teacher stated that she is often out of ratio by three children. I asked about group size. The teacher stated there are times when she and another teacher would have thirty-two plus children in the classroom. I discussed it with the Director, and she admitted that she has had some child staff ratio and group size issues. We discussed with the Director a concern that a child was being held and handled in a dangerous position. The Director stated that she had terminated a teacher for inappropriately disciplining and handling children. The Director stated that the teacher had many write ups before being terminated. I requested to observe the documentation. On 5/20/25 the teacher had documentation of lifting and handling children inappropriately. On 4/30/25 there was documentation of the teacher speaking inappropriately to three different children. We discussed with the director a concern regarding a child leaving out of the gate. The Director admitted that a child went out of the gate when the child saw their parent. The teacher was at a one staff child ratio. When the teacher noticed the child, the teacher called the child and the parent over and discussed the importance of letting the parent pick up through the building. During the visit I discussed the concern of a child being placed outside on the playground from the toddler classroom. The Director stated that she was unaware of a child being placed on the playground from the toddler classroom. I interviewed the teachers of the classroom. One teacher stated that she was partnering with the parent to find ways to help the child not scream and cry. The teacher stated that she places the child outside of the classroom on the playground and allows the child to scream. The teacher stated that she never leaves the child alone. The teacher stated that she also places another child outside to run around when the older children are outside on the playground. The teacher stated that she only sends that child out when the child is running around the classroom. I discussed with the Director what was stated. We also discussed the importance of not allowing teachers to place children out of the classroom because of behavior. We also discussed that having a toddler or two-year-old child on the playground with an older age group could possibly cause the class to be out of ratio. During the visit two parents arrived with a concern that a teacher grabbed their child by the neck and squeezed the neck of the child. The parent discussed the concern with me and then discussed it with the Director. The parents stated this is not the first concern that they have had. The parents discussed that they asked to view the camera footage but was unable to because the camera footage was not working properly. After the parents discussed their concerns with the director. The director discussed that no other teacher observed the child’s neck being squeezed. The director informed me that the administrators told the parents that they will make sure the teacher is not working with their child and that the video camera system will be upgraded. The following violations were cited during today's visit: Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. The teacher in K-Ready 2 admitted that she was out of ratio during the week of 5/20/25 and frequently. The teacher also admitted that her class had thirty-two children plus with another teacher the week of 5/20/25. GS 110-91(7);.0713(a-d) 303 Children were not adequately supervised at all times. The administrator admitted that a child walked out of the gate on 5/20/25 while the teacher was lining the class up to go inside from the playground. A teacher admitted that she places a child outside of the classroom on the playground when the child is screaming and crying. The teacher also admitted that she places another child outside on the playground to run when the child is running in the classroom. .1801(a)(1-5) 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. Children were handled in a rough manner 5/20/25. .1803(a)(1) 1876 A child was yelled at, shamed, humiliated, frightened, threatened or bullied. Three children were yelled at and talked to inappropriately by a teacher on 4/30/25. .1803(a)(9) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before June 11, 2025, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future. Please make sure to include: -Facility name -Facility ID number -Each item number The letter can be emailed to Shamequa.Wilkerson-Harris@dhhs.nc.gov RESOLUTION Based on discussion with the administrator, and the teacher admitting being out of child staff ratio and group size, the allegation of child staff ratio and group size is substantiated. Based on discussion with the administrator, and documentation observed, the allegation concerning inappropriate discipline is substantiated. Based on discussions with the teacher and the teacher admitting children being placed outside of the classroom on the playground. Based on the discussion with the administrator admitting that a child went outside of the gate, the allegation concerning a lack of supervision is substantiated. Based on discussions with the administrator and observations concerning children walking to another classroom unsupervised, I am unable to determine if a lapse of supervision occurred, therefore the allegation is unsubstantiated. TECHNICAL ASSISTANCE The following items were discussed during today’s visit. You may want to consider them to avoid violations in the future: -Technical assistance was given to Ms. Nowak in regard to the arrival time of her staff based on the arrival time of the children. Arrival time is not always predictable for each child daily; therefore, I would recommend completing a time study over a week basis to determine staff schedules. Please be mindful that moving the oldest or youngest child each morning to the next classroom to meet staff/child ratios is not best practice. When children arrive at school they should be allowed to arrive in their classroom and not have to move to another classroom because of not having enough staff present on site. - One of the biggest challenges that early childhood educators and administrators face is children’s behavior. Often these behaviors are developmentally appropriate, however, some behaviors can be difficult for a teacher to manage. Providing technical assistance, coaching and professional development for staff to prevent and address these challenging behaviors, as well as promote children’s social competence can be beneficial. If you would like assistance with dealing with challenging behaviors of children in your care, you can contact a Healthy Social Behavior Specialist at Early Years; they can be reached at (919) 403-6950. -Positive verbal interactions are essential to the emotional and social development of a child. It is important to ensure that your teachers are not overwhelmed to have a positive classroom environment. I would highly recommend viewing the Early Years website at www.earlyyears.nc.org and have your staff take training modules pertaining to positive interactions. We also discussed conscious discipline training during today’s visit. There was a total of one hundred four (104) children present, aging in range from infant to five old. The children were observed playing in centers, playing outside, doing a group activity, participating in circle time, eating lunch, napping, and completing routine tasks. LICENSE STATUS Currently, this Child Care Center operates with a Four (4) star license issued June 29, 2022, earning seven (7) points in education, two (2) points in program standards and one (1) quality point. This center operates at 30sq. ft. inside, & ratios at .0713(a). I reviewed the NC Secretary of State’s website and observed the owner of the facility Premier NC Wake Forest LLC listed as current/active. INSPECTIONS *The last fire drill was conducted 1/31/25. *The last emergency drill was conducted 11/7/24. *The last playground inspection was completed 1/20/25. *The last sanitation was completed on 5/31/24 with a ‘Superior’ classification. *The last fire inspection was conducted on 4/1/24. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. New staff CPR/FA, qualification letters and newly enrolled children files were reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. The program records were reviewed for current inspections. Transportation is not provided; therefore, these requirements were not monitored. In Space #2 (Voyagers), I observed Aquaphor with an expiration date of 12/23 authorized to be used from 1/9/25 – 2/9/25. In Space #1 (Wee School) I observed two prescription medications not in its original pharmacy labeled container for two separate children. In Space #2 (Voyagers) I observed an incomplete authorization form for Vaseline. The form did not have instructions on how much the parent wanted to be applied to the child. I would recommend having a member of the management check all medications weekly or bi-weekly to make sure all forms and medications are complete and not expired. I would also recommend sending out a memo to parents reminding parents to send the original pharmacy labeled package for prescription medications. SAFE INDOOR/OUTDOOR On the playground that the K-2 use has a tripping hazard between the fence and the sidewalk. The depth of the sidewalk to the ground was three inches. I would recommend having the Environmental Health Consultant send you verification in writing. Once you send the verification please forward it to me, your licensing consultant. In the meantime, I would recommend finding another location to store the cots for the classroom. I would recommend storing the Swiffer jet mop in a locked closet. CPR/FIRST AID Three staff members CPR/FA expired on 1/2023; one staff member CPR/FA expired on 1/2025. I the Administrator stated that she feels they completed CPR/FA in January but is unable to locate the certificates. HEALTH AND SAFETY TRAINING Health and Safety training is now available through Moodle on the DCDEE website. This training is provided free of charge. You should use your NCID username and password to access the Moodle portal. The Health and Safety trainings must be renewed at least every five (5) years. If you completed the training in 2020, you should begin to retake the training now. CRIMINAL BACKGROUND As of July 11, 2022, criminal record checks are now to be renewed every five (5) years instead of three (3) years. Please make arrangements to complete your 5-year re-qualification before the expiration date. ONGOING TRAINING HOURS Training hours were monitored from 2/21/24 to 2/18/25. Upon arrival I was greeted by Kayla Nowak, the Director. You assisted me with today’s visit. I completed a general walk-through of the indoor and outdoor learning environment. At the time of the visit, there were a total of 89 children present, ranging in age from infant to five (5) years old. The children were observed napping, completing routine tasks, and enjoying a buggy ride. LICENSE STATUS Currently, this Child Care Center operates with a Four (4) star license issued June 29, 2022, earning seven (7) points in education, two (2) points in program standards and one (1) quality point. This center operates at 30sq. ft. inside, & ratios at .0713(a). I reviewed the NC Secretary of State’s website and observed the owner of the facility Premier NC Wake Forest LLC listed as current/active. INSPECTIONS *The last fire drill was conducted 8/2/24. *The last emergency drill was conducted 8/7/24. *The last playground inspection was completed 7/22/24. *The last sanitation was completed on 5/31/24 with a “Superior ‘’ classification. *The fire inspection was completed on 4/1/24 and the facility is approved for daytime care only. MONITORING During today’s visit, a partial assessment of Child Care Requirements was conducted for all classrooms. The monitoring included CPR/First Aid, criminal background checks and special training requirements of newly hired staff. The monitoring also included License and permit restrictions, staff/child ratios, supervision, storage of hazardous items, medication, materials/equipment, and all space occupied by the children including outdoor play environment. During today’s visit I observed space #8 (Two’s classroom) closed due to an electrical fire in the bathroom of that classroom. The actual classroom has been cleaned and sanitized, however, no children are using the classroom at the time. The following violation was observed and documented during today’s visit: Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. In space #5 and Epi-Pen was observed with an expiration date of July 2024. 10A NCAC 09 .0803(1)(d) To comply with the NC Laws and Rules any violations cited today MUST be corrected immediately. A compliance letter must be received on or before September 3, 2024, letter must address each violation, explain how it has been corrected and how the violation will maintain compliance in the future.
