BENSON Operation Type: Family CC Home Case Number: Visit Date: 1/7/2026 Number Present: 0 Completed Date: 1/7/2026 Age: From 0 To 0 Total Minutes: 200 Time In: 08:40 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's visit was to monitor compliance with applicable child care requirements pertinent to the first Temporary Time Period visit. This program currently operates with a temporary license effective September 22, 2025 to March 22, 2026. During this visit you, Arnita Graham Operator, were present. I attempted to conduct a visit on November 13, 2025. You are required to have an annual fire inspection. Your fire inspection was conducted on January 6, 2026. During this visit, I monitored the approved indoor and outdoor areas for child care. Program records and the operator’s files were also monitored. I could not monitor children’s files because you stated that there are currently no children enrolled in your FCCH. I discussed with you that I must conduct at least one (1) visit when children are present. I instructed you to contact me as soon as a child enrolls in your FCCH. At the completion of the visit, the Visit Summary was reviewed with you, and you were also given a computerized copy to keep for your records. Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. The pet's vaccinations expired on 11/5/2025. They were updated on 12/1/2025. This was corrected during the visit because the pet's current vaccinations were observed on file. .1719(b)(1) 908 Health questionnaire was not completed annually. The operator's health questionnaire expired on 10/2/2025. It was updated on 1/7/2026. This was corrected during the visit because the operator's current health questionnaire was observed on file. .1703(a)(1) All violations must be corrected immediately. The two (2) violations that were cited during this visit were corrected during the visit; therefore, you are not required to submit a compliance letter. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during this visit, may negatively affect your compliance score. As of today, your compliance history is 96%. The compliance history assessment sheet was reviewed with you and you were also given a copy for your records. Rated License In order to maintain subsidy payments at the end of the temporary time period, the facility must transition to a three to five star rated license. You have decided to go with Pathway #2-Classroom and Instructional Quality. During this visit I provided you with the Application For Assessment for a Rated License for a FCCH and also demonstrated how to access the application and forms on the DCDEE website at: www.ncchildcare.nc.gov. I also recommend that you reach out to the Scotland County Partnership for Children for additional technical assistance at: www.scotlandss.org. I discussed with you that all required information for Pathway #2 must be completed no later than February 22, 2026, to ensure that your rated license application is completed before your temporary license expires. Your Temporary License can’t be extended; therefore, I will use the information available at the time of review to process your application. I discussed with you that your education information should be submitted to WORKS no later than the fifth month of the temporary license period. As a rated license must be processed prior to the expiration date of your temporary license which is March 22, 2026. The education on file at that time will be used to process your Rated License application, which may affect the number of stars you could receive. Therefore, it is imperative that all required and current education documentation is submitted and on file prior to processing. For additional information, A new Pathways to the Stars Rules Roll Out module is now available on the DCDEE Moodle digital learning platform. To access the links on the application you can visit: the DCDEE website at: www.ncchildcare.nc.gov and click on the provider tab and then scroll down and click on the provider documents and forms and then click on the Application for Assessment for a Rated License for FCCH. Technical Assistance: Maintaining current and accurate records is essential to demonstrate compliance with licensing requirements, support effective monitoring, and ensure the health and safety of children in care. To maintain current and accurate records, regularly review all program files and update them as changes occur and keep documentation organized. Reminders: You completed the EPR training on September 11, 2025. You were licensed on September 22, 2025. You are required to complete and upload your EPR plan within four (4) months of becoming licensed. You can access the Risk Management Portal at: https://rmp.nc.gov/Portal/. I demonstrated how to access EPR information during this visit at: www.ncchildcare.nc.gov Visit the DCDEE website (www.ncchildcare.nc.gov) for updated requirements, forms, and other pertinent information.
