I met with, Tina Maye, Director, I conducted a walk-through of the facility. The program operates with a four-star license issued May 7, 2021. Restrictions: Daytime and Overnight; Meets enhanced ratios; Children under 2 & 1/2 years old in rooms with direct exits only. Upon arrival I knocked on the door, a staff member, Ms. E, opened the door and stated the purpose of my visit. The teacher called Mrs. Tina, owner/administrator, and she came into the classroom and I greeted her and stated the purpose of my visit. At preschool aged children were observed in active gross motor play on the playground. School agers were observed reading and retelling books, and painting planets. The infant/toddlers were observed in toileting and diapering, engaged with fine motor toys and walking. Children were observed handwashing, and transition to lunch. Children washed their hands and sitting to wait for their lunch. Lunch consisted of corn dogs, baked beans, milk, peaches/pears. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last sanitation inspection was conducted: 7/7/25 The last fire inspection was conducted 1/7/25 . The last documented fire drill was conducted 07/11/25. The last documented shelter-in-place or lockdown drill was conducted 06/23/25. The last documented playground inspection was conducted 07/11/25. Last compliance prior to this visit was 94% I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Transportation is currently, not provided at this facility. Staff and children’s records were reviewed today. I selected ten percent of children’s records for review and two Staff. I discussed the purpose of the children’s record sheet and how to complete the Staff and training worksheet. Violation Number Comment Rule 614 Bed, cribs, playpens, cots or mats were not placed at least 18" apart or separated by partitions when in use. Mats measured less than 18" apart, and most under 10". 15A NCAC 18A .2821(e) 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. the gate opening near the back of the playground measure 6.5 inches wide. .0605(g) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Staff purse was on the table in the preschool aged children room. Hand sanitizer, expo knife were among the items in the purse. .2820(b) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Several children whose files were monitored did not meet this requirement. .0802(c) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Staff file reviewed had medical/health information combined with all staff records. .0701(d) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. REQUIRED RESPONSE: You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address or email below by July 31,2023. The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter, -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE REQUIRED RESPONSE: If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. Technical Assistance: Staff should model appropriate/healthy eating habits: Staff had a burger King cup, oreo cookies, chips and other snacks in her purse/bag that was visible to children, but she was not observed eating. Epr should be updated yearly, use the old forms with the teachers signatures and dates to keep more accurate records. Hold harmless has ended on July 1, 2025. DCDEE is has realized new pathways to gaining Stars. There are three different options available and will be discussed in a webinar. The facility operates with a four-star license and has a current compliance history of 100%. I was greeted by Mary Maxwell, Director, I completed a walkthrough of all the indoor spaces used by the children and monitored for the following: supervision, staff/child ratios, storage of hazardous products and medication, adequate and approved space, and program records. The staff files were viewed today to monitor for current CPR/First Aid, and ITS-SIDS certification. The staff and training worksheet that was completed for the last annual compliance visit was used to monitor for Health and Safety trainings, and current qualifying criminal background check letters. There have been no new staff hired since the last annual compliance visit. The last fire drill was conducted on 2/10/25. The last Shelter-in-Place and lockdown drill was conducted on 1/27/25. All emergency drills have been documented as required. The playground inspections have been documented as required. The last sanitation inspection was conducted on November 18, 2024, with 4 demerits. The last fire inspection was conducted on 1/7/25. The following items were posted for easy reference: license, NC Summary of Law, First Aid poster, evacuation routes, safe arrival and departure procedures, classroom schedules, menu, staff/child ratio charts, emergency telephone numbers, tobacco free signage, and emergency medical care plan. Upon arrival all ten (10) children present were in space #3. The children were observed completing lunch, and bathroom/handwashing routines. I spoke with Ms. Maxwell discussing the ages of the children. It was determined that two (2) of the ten (10) children present were less than 1 year of age. Ms. Maxwell brought these two children from space #3 into space #2 where they are enrolled. I explained to Ms. Maxwell that children under one year of age can not be grouped with children two years of age or older. All interactions observed were positive. During this visit, a 13 year old child who was not a volunteer was present in space #1. Tina Maye, owner, stated that the child was expelled from public school and was brought to the facility earlier in the day by a friend of Ms. Maye. I, Michell Kocan, told Ms. Maye that children who are not enrolled at the facility can not be on the premises. Ms. Maye stated that someone would arrive soon to get the child. There were five (5) violations of childcare requirements observed during the visit. Violation Number Comment Rule 103 The number of children present was not within permit capacity. Classroom space #3 has a capacity of nine (9) children; during arrival at the visit ten (10) children were observed in space #3. Providers are strongly encouraged to do the following: • Have all staff update their WORKS accounts to reflect all currently completed coursework (DCDEE-WORKS) • Request technical assistance with your child care consultant and local partners • Participate in local CCR&R related training/workshops • Reach out to your local Community College to discuss educational opportunities. • Review NCRLAP website ERS resources (www.NCRLAP.org). • Consider requesting an ERS practice assessment (free of charge). I met with, Tina Maye, Director, I conducted a walk-through of the facility. The program operates with a four-star license issued May 7, 2021. Restrictions: Daytime and Overnight; Meets enhanced ratios; Children under 2 & 1/2 years old in rooms with direct exits only. The facility had an 88% compliance history prior to today's visit. The Annual Compliance Monitoring Checklist tool for Child Care Centers was used during today’s visit. There are 26 children currently enrolled and 18 were present during today’s visit. The children were observed engaged in the following activities: free-play in activity areas, outdoor free-play, and bathroom/handwashing routines. Prior to the visit I sent an email requesting the staff and training worksheets to Tina Maye, Director. The worksheets were received prior to the visit. The facility has not employed any new staff members since the last compliance visit was conducted. I monitored files of three (3) existing staff for more than 10% of staff files. I monitored three (3) children's records during the visit for 10% of children's records. The following items were posted for easy reference: license, Summary of NC Child Care Law and Rules, safe arrival and departure procedures, emergency medical care plan, daily schedules, current activity plans, First Aid poster, menu, applicable staff/child ratio charts, tobacco free signage, and evacuation routes. The annual fire inspection was conducted January 3, 2023, and the facility was found to be satisfactory and approved for day time care only. The last sanitation inspection was conducted December13, 2022, with a score of 2 demerits for a Superior classification. The last documented fire drill was conducted on July 21, 2023. A lock-down drill was conducted on July 21, 2023. The monthly playground inspections have been completed as required. The following items were monitored today: supervision, staff/child ratio, staff, health, safety, program records, and management. Three (3) violations of childcare requirements were observed today. Violation Number Comment Rule 853 Incident logs were not completed and maintained as required. An incident log has not been completed. .0802(g)(1-6) 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. The EMC plan was not reviewed annually with five (5) of six (6) staff. 10A NCAC 09 .0802(a) 1850 Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. Two (2) vans used for transporting children enrolled in care did not have no smoking signage posted. .0604(i) The violations documented during the visit must be corrected immediately. To verify correction of the violation you shall submit a signed letter to me explaining how you have corrected the violations described above. The letter should list each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email.
