This facility currently operates with a four Star rated license effective September 18, 2018. This facility operates three shifts a This visit was conducted during the first shift. Upon arrival I rang the door bell, I was greeted by D. Polk, teacher. I stated the purpose of my visit and asked her to make Mrs. Smith, Owner/Administrator aware of my presence. I proceeded with the walk through of the facility unassisted until later during the visit when Mrs. Smith, owner/director, assisted with program files. Children were observed playing with fine motor toys. Transitioning from indoor to outdoor play where observed and children ere actively engaged in gross motor play. When children transitioned from outside to handwashing. Children were served gold fish and water for morning snack, and barbeque chicken bites, wheat thin crackers, fruit cocktail, and milk. INSPECTIONS: The last sanitation inspection was conducted 04-07-2026. The last fire inspection was conducted 7-23-2025. The last documented fire drill was conducted 3/1/2026. The last documented shelter-in-place or lockdown drill was conducted 3-1-2026. The last documented playground inspection was conducted 5-1- 2026. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. This facility currently does not provide transportation. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Two staff had CBC that expired in April 2026. G.S. 110-90.2(b) & .2703(n)&(o) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Two children files have not been updated since 2024/2025. .0802(c) 1821 The EPR Plan did not include the date of the last revision of the plan. Information in the EPR plan was outdated, staff listed are no longer staffed and children in file are not current. I greeted the teacher and stated my purpose, and she stated she did not hear the door and to go back to the main entrance. After two minutes I was greeted by D. Polk, who opened the door. I restated my purpose and asked her to make Mrs. Smith, Owner/Administrator aware of my presence. I proceeded with the walk through of the facility unassisted until later during the visit when Ms. T. Spencer, who went in the kitchen to take over cooling lunch that was already cooking at my arrival and Mrs. Smith assisted with program files. Children were observed participating in small group activities of listening to books read by a teacher and sitting at the table playing with fine motor toys. Children were observed transitioning from indoor to outdoor play where they were actively engaged in gross motor play. When children transitioned from outside to handwashing. The posted menu was for corn dogs, corndog bread, fries, fruit cocktail, and milk, however children were served steak, rice, carrots, with gravy, fruit cocktail, and milk. Evening Snack , which consisted of ham sandwiches, oranges, and milk. Arrival from Elementary school, departure from the center. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. Upon arrival I was greeted by S. Rorie, staff in charge in the directors absence. Ms. Rorie greeted me as I was getting out of the care and I stated the purpose of my visit. Ms. Rorie contacted Mrs. Smith. I proceeded with the visit unassisted until Mrs. Smith arrived. This facility currently operates with a four Star rated license effective September 18, 2018. This facility operates three shifts and is typically open from 6:00 am to 7:00 pm. This visit was conducted during the second shift. When entering the building I noticed there were eight children to one teacher in space two and thirteen to one teacher in space one. Both classes were not in ratio. Children were observed playing in centers, transitioning from indoor to outdoor play where they were actively engaged in gross motor play. When children transitioned from outside to handwashing, they were served Ham sandwich with bread, carrots, oranges, and milk for supper. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. INSPECTIONS: The last sanitation inspection was conducted 11-22-2023. The last fire inspection was conducted 6-29-2023. The last documented fire drill was conducted 5/1/2024. The last documented shelter-in-place or lockdown drill was conducted 3-1-2024. The last documented playground inspection was conducted 5-1- 2024. Last compliance history prior to today’s visit was 94% I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. This facility does not provide transportation. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Upon arrival there were 21 children and two staff members. Space two consisted of infant up to two years of age to one teacher and space one consisted of thirteen children age three up to eleven years of age and one teacher. GS 110-91(7);.0713(a-d) 606 Running water, soap and individual sanitary towels, or other approved hand-drying devices were not supplied at each lavatory. Paper towels were kept in the classroom away from the bathroom that had the toilet and sink. 15A NCAC 18A .2818(b) & (d) 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. there were several areas on the fence that had entrapments. On the left side of the fence closest to the building there were two areas that measured 4.5 inches, 5 inches and on the backside of the fence. .0605(g) 807 A safe indoor and outdoor environment was not provided for the children. There were three fire ant hills on the playground. Medication for AJ.D, J/T, D, and several others were missing required information: amount to be given, when to administer. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Destin, inset repellent, Hyland cold and cough syurp, Childrens acetamiophen, and Aquaphor. .0803(12) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Staff was unable to show February, March, and April sleep checks. A school-aged child was told to go into the building and retrieve a towel out of the closet. And a Checklist was given to you, at the close of the visit. Upon arrival there were 21 children and two staff members. Space two consisted of infant up to two years of age to one teacher and space one consisted of thirteen children age three up to eleven years of age and one teacher. On the left side of the fence closest to the building there were two areas that measured 4.5 inches, 5 inches and on the backside of the fence. .0605(g) 807 A safe indoor and outdoor environment was not provided for the children. There were three fire ant hills on the playground. Epi-pen storage and Medical Action Plans (M.A.P.) were discussed and it was decided to leave the epi-pen five feet high or in a first aid bag that is five feet high just incase there is an emergency. A copy of the M.A.P. should be placed with the Epi-pen and in the child’s file. A copy of the forms related to medical administration, and Medical Action Plan forms were emailed to you. And a Checklist was given to you, at the close of the visit. Upon arrival I was greeted by S. Rorie, staff in charge in the directors absence. Ms. Rorie greeted me as I was getting out of the care and I stated the purpose of my visit. Ms. Rorie contacted Mrs. Smith. I proceeded with the visit unassisted until Mrs. Smith arrived. This facility currently operates with a four Star rated license effective September 18, 2018. This facility operates three shifts and is typically open from 6:00 am to 7:00 pm. This visit was conducted during the second shift. When entering the building I noticed there were eight children to one teacher in space two and thirteen to one teacher in space one. Both classes were not in ratio. Children were observed playing in centers, transitioning from indoor to outdoor play where they were actively engaged in gross motor play. When children transitioned from outside to handwashing, they were served Ham sandwich with bread, carrots, oranges, and milk for supper. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. INSPECTIONS: The last sanitation inspection was conducted 11-22-2023.
