This facility operates with five stars, meets all enhanced ratios and space and approved for developmental day. Currently, this facility only has one classroom and does not have any children enrolled in need of developmental day services. Upon arrival I was greeted on the playground by staff. I greeted them and met the interim director, K. Gerald, at the door. I greeted her and stated the purpose of my visit, and we proceeded with the walk-through of the facility. The children and teachers were actively engaged in gross motor and other various activities set up for the children. Children were observed in gross motor play, transitions from indoors to outdoors, and mealtime. Children were served pepperoni pizza, corn, peaches and milk for lunch. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. There were no new staff hired at this facility. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. The wall near the sink and window has peeling paint. 15A NCAC 18A .2825(a) Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your compliance history was 99% prior to today’s visit. I was greeted by a teacher who opened the door. Once inside I greeted the children and staff and stated the purpose of my visit. Today’s visit was conducted with Karen Gerald, interm director. The facilities is a 5 star facility meeting the requirements of the 7 points in the Education Component, 7 points level in the Program Component and 1 Quality point which was verified at today's visit. The facility has a permit with a capacity of 36, age range of 3 years old to 6 years old and the permit restrictions of daytime care only, no children under 3 years old, and meets all enhanced standards including reduced ratios and Developmental Day. The license was effective March 14, 2025. The facility is composed of 1 NC Pre-K classrooms. The facility currently does not provide care for Developmental Day children and it was stated that they are not planning on enrolling anymore children who fall under these guidelines. Ownership information was confirmed as sole proprietorship and the owner being First United Methodist Church of Wadesboro NC Inc. The address and telephone number were verified during today’s visit. The owner was located on the Secretary of State website as an active corporation SOS id 0710476. Children were observed actively engaged in Circle time, reading books, some children played in dramatic play. Children transitioned from diapering/toileting and handwashing. For lunch, Children were served, tortilla, fish nuggets, baked beans, pears; and milk. Naptime was also observed. I monitored the following items: Supervision, Staff/child ratio, health, safety, program and management. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. INSPECTIONS: The last sanitation inspection was conducted 11-18-2024. The last fire inspection was conducted 1-24-2024. The last documented fire drill was conducted 9-17-2025. The last documented shelter-in-place or lockdown drill was conducted 9-22-2025. The last documented playground inspection was conducted 9-22-2025. The last compliance was at 92 %. NC PreK Developmental Day / Requirements: The official NC Pre-K day and developmental Day facility requirements, nutrition, curriculum including developmental screenings and instructional assessments, staff to child ratio and class sizes, family engagement, activity plans, teacher education and schedules were reviewed and found in compliance. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An electrical outlet had covers but the plastic outlet was broken and children could possibly insert an object into the hole. 10A NCAC 09 .0604(c) 1329 Application for enrollment did not include all required information. The facility has a permit with a capacity of 36, age range of 3 years old to 6 years old and the permit restrictions of daytime care only, no children under 3 years old, and meets all enhanced standards including reduced ratios. The license was effective December 4, 2019. The facility is composed of 2 NC Pre-K classrooms, and one is being converted into a developmental day. Children were observed actively engaged in dramatic play, blocks, fine motor, science, reading, and interacting with sand play indoors. Outside gross motor active play was observed, and the area was clean. Teacher did not bring out portable gross motor equipment, however the items are stored outside in a shed on the playground. Quality Option: The facility has earned a quality point by having enhanced policies that were approved by a pervious Child Care Consultant. The facility will be a five (5) star facility, with Developmental Day. Violation Number Comment Rule 102 The license was not posted in a prominent place at all times. There was no evidence of the license in a place were parents can view. Upon arrival I was greeted by Lyndsey Myers, administrator. I stated the purpose of my visit and proceeded with the walk-through of the facility. The children and teachers were on a field trip to the library. When they returned, I observed them transition to handwashing to eating lunch. Lunch consisted of a cheeseburger with a bun, potato rounds, peaches, and milk. After lunch children were actively engaged in pretend play. Departure was observed as several parents picked up their child early. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. Ms. Myers and I discussed her ideas for preparing for the next school year. She is expecting to have the same teachers in the NC PreK classroom and wants to reopen space one to add children with differing abilities. We discussed the importance of teacher education and experience to maintain quality and five stars. I offered technical assistance on the repairs needed in the room, room arrangement and other needs to run a successful mixed ability classroom. Prior to this visit an environmental rating scale was completed, now that they are fully staffed the facility will have their Rated license reassessed. Rated License Summary: Program Standards The Rated License Assessment was conducted on December 5, 2022, and the facility received a 6.36, which will give them seven (7) points in program standards. Education All teachers and administrators were in works and reviewed while onsite. The facility will receive seven (7) points in Education. Quality Option: The facility has earned a quality point by having enhanced policies that were approved by a pervious Child Care Consultant. The facility will remain a five (5) star facility. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There was two outlet uncovered near the Television and the further down on the same side of the wall. 10A NCAC 09 .0604(c) You are required to always maintain compliance with all applicable childcare rules and regulations. NC GS 110-90(4)(d) requires all childcare facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Technical Assistance: *Paint the areas of the wall that have chipped paint. *If you are going to enroll children in diapers, purchase the proper equipment to meet the needs, changing table with stars, or a changing mat depending on the age of the children. *You can currently enroll children aged three to six years of age. If you are interested in enrolling younger children please contact your consultant. *Contact Anson Partnership for children for more technical assistance on room set-up. *Contact Anson's Children Center and ask for a tour of their facility and feedback on how they provide care for children with various abilities in on classroom. *If you choose to enroll exceptional children please be proactive with setting up classroom management and using tools like "Conscience Discipline", "Flip-it" to train teachers and utilize in the classroom.
