Upon arrival I was greeted by the Director F. Colson, when I walked in I greeted everyone and stated the purpose of my visit. The program operates with a four-star license, issued April 24, 2021, earning four (4) points for Staff Education, seven (7) points form Program Standards, and a quality point. License restrictions include First Shift, meeting enhanced ratios and space and enhanced ratios reduced by one in each group. All food prep must be done in kitchen excluding warming of bottles. The North Carolina Secretary of State website was reviewed and ownership information was confirmed as the Non-Profit Corporation The Tabernacle of the Congregation Interdenominational Fellowship, Inc. The address, telephone number, and email were verified during today’s visit as being correct in the Division of Child Development and Early Education (DCDEE) database. Children were observed, eating a breakfast that consisted of Kits cereal, pears, and milk. After snack, children ate cupcakes for an school aged child birthday. Children were observed transitions from handwashing, toileting, and indoor to outdoors. The following centers were present: blocks, science, dramatic play, art, and fine motor to name a few. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last sanitation inspection was conducted 06/08/2026 The last fire inspection was conducted 07/07/2025 The last documented fire drill was conducted 06/24/2026 The last documented shelter-in-place or lockdown drill was conducted 06/18/2026 The last documented playground inspection was conducted 6/22/2026. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Staff and training worksheet was completed onsite and Children’s records were also monitored. This facility does not currently provide transportation. Rated License: This facility has decided to pursue Pathway two: classroom and instructional practices. We went over things you have completed and are working on, the curriculum the facility will use and ways to organized documents needed. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Sleep charts are stored in a blastic bin. Several sleep charts were missing for a child. .0606(g) 1031 Documentation of staff's education, training, and experience was not on file. We opened the documents, reviewed, and practice workling in Excel. It is imperative that you create a system to store and organize documents needed for you facility. I have given you the retention sheet as well as went over the pathway you chose and suggestions on how to retain those documents along with other required documents. Suggestions: purchase/ create lable file boxes (bins/folder/notebook),to place sleep charts, lesson/activity plans., and other documentation needed for the Pathways. Delegate staff to document their own trainings on the appropriate log. Type, instead of handwriting the staff and training worksheet, to ensure that you are always prepared for a visit and it will be easier to locate. Show staff where important files are for days that you are absence. Once you have completed your curriculum training, please email your application. This should all be completed before September 1, 2026 in order to have your stars processed. *Contact your local Childcare resource and referral agency to get assistance with preparing for Pathway two. The last documented fire drill was conducted 6/3/25 The last documented shelter-in-place or lockdown drill was conducted 6/5/25 The last documented playground inspection was conducted 6/12/25. The last documented compliance score was 92% I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Staff and training worksheet was completed onsite and Children’s records were also monitored. This facility does not currently provide transportation. The facility has enlarged their outdoor play space, the space was measured during this visit. The current measurements equal 2137.50 square feet, 29 children at 75 sq feet per child and 21 for 100 square feet. You stated that the facility is still interested in licensing the downstairs level for older children. This space was measured in 2023. Once you receive the required building/zoning, fire and sanitation approvals, email or mail a copy and notify me once the room has been set up for use. Once set up, and required approvals, I will make a visit to approve the space. I proceeded with the walk through the facility. I was assisted by Mrs. S and Mrs. Colson. School Aged children were observed participating in outdoor gross motor activity, playing with balls and riding toys. Inside two- to five-year-olds were playing in dramatic play and a child was in the block center building a standing cross and pulpit. There was one infant present, a teacher was reading to her while the other staff cleaned the room. The following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure. There were no new staff. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. a bottle of Create Basic white glue, and clear adhesive glue, that stated "Keep out of reach of children under three", was stored on a shelf accessible to two year old and younger children. .2820(b) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. There were several plastic bags in the cubbies, and in a laundry basket with art materials. .0604(q) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Upon arrival I was greeted by Ms. S Pauling and I shared the reason for the visit. Ms. F. Massey assisted me with today’s visit. Your program currently operates with a Four Star Center License effective 4/24/2021. The license was posted, with restrictions to first shift care, meets enhanced ratios and meets enhanced space, enhanced ratios reduced by one in each age group. All food prep must be done in the kitchen excluding warming of bottles. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety two (92%) percent as of today’s visit. Ownership: The facility owner The Tabernacle of the Congregation Interdenominational Fellowship, Inc. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The Fire Inspection has not occurred since the last visit and remains current. Sanitation visit occurred on 12/5/2023 with a Superior rating. The following items above were found in compliance and are listed below. •Last fire drill was conducted on 1/5/2024. •Last Shelter in Place/Lockdown Drill was conducted 1/9/2024. •Last Playground Inspection was conducted on 1/9/2024. The children were observed transitioning from outside free play to handwashing to a teacher director circle time with a themed book. The Preschool age children then played in self-directed centers with teacher involvement. The Preschool age teacher was attentive to the children’s needs, demonstrated nurturing and interactive play with her class. The infants and toddlers were involved in self-care routines including bottle feeding and floor time interactive play. The infant and toddler teacher was on the children’s level and addressing their needs in a timely manner. Program Records: We reviewed all the required records and all were in compliance Staff Records: The staff-training worksheet was completed by the Consultant during the visit and there was one (1) new staff files to monitor. Medication: Medication was monitored and while the emergency medication had all required permissions and storage requirements, the medication was not in its original container. The parent was on site and was able to label the medication and was asked to bring in the original container. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is provided and Ms. Massey stated that all appropriate DCDEE signage, insurance, and inspections are current. There were two (2) violations were observed today. The violations are as follows: Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. An epi-pen was not in the original container. The parent was on site and was able to label the medication for identification. .0803(2)(a) 1534 Personnel records were not maintained on file in the timeframes given by Rule .2318(7). New staff member S.B. did not have the following in her personal file. Operations and personal policy, training worksheet to calculate hours within the first 2 (two) weeks and 6 (six) weeks and review of emergency medical care plan. S.B. has been out with several illnesses since her start date and has been out. .2318 (7) On or before 2/21/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License •Contact your Child Care Consultant for the rated license cohort preparation year and assessment year. Please let the Child Care Consultant aware of when and if you are ready to begin the Star Rated License process within the cohort timeline. Technical Assistance •There is a new Summary of the Law on the DCDEE website that should be posted on the information board. •There are new NC Sanitation Laws that have been implemented, please reach out if you have any questions. •Reach out to your local CCRI and Partnership for Children for assistance with the Rated License process. •Please keep in mind that EPR plans must be reviewed annually or if any information changes at a minimum. •In the infant and toddler area some of the popcorn ceiling is peeling and hanging, it is not to the point where it is out of compliance but it should be removed and repaired in a timely manner. •I left with you an updated medical permission form to use with emergency medications. •I also reminded you that if changes are made to the menu, it must be noted before the food is served.
