I was sitting in the driveway writing the second attempted visit . The provider drove up in her van, got out of the car and greeted me. After stating the purpose of my visit l proceeded with the with Ms. Janice Smith. There was two present today. After transitioning from the van into the home children were offered books to read while they waited. Children were also observed playing in activity centers. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. See technical assistance for topics discussed during this visit. Violation Number Comment Rule 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Fire drill was don on April 3, 2026, there was no attendance taken on that day. .1721(e)(6) 1101 Operator or other transportation provider did not have written permission from each child's parents or guardian to transport the child. The provider was not approved for transportation, therefore it was no form completed. .1723(5) 1103 For each child being transported, the operator or other transportation provider did not have identifying information, including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form required by Rule .1721(a )(3) in the vehicle whenever children were transported. The provider was not approved for transportation and there was no evidence of this information. .1723(13) 1715 The written plan of care did not specify how the operator will maintain compliance with transportation requirements specified in Rule .1723 when children are transported. The provider was not approved for transportation, therefore it was not on the form. .1712(e )(3) 1716 The written plan of care did not specify how parents will be notified when children accompany the operator off-premises for routine tasks not specified on the written schedule. The provider was not approved for transportation, therefore it was not on the form. .1712(e )(4) 1997 Vehicle used for transportation did not have a fire extinguisher. No evidence of this requirement. .1723(11) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. REQUIRED RESPONSE: You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. You shall correct the violation(s) immediately. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. Technical Assistance: To provide transportation for routines or to take children to the park a consultant must approve the vehicle. I have given you the rules to have the vehicle approved. I am unable to approve it today due to no fire extinguisher. Also your tire tread is getting low, if you decide on transporting I would suggest getting new tires before approval. See resources for the rule. I also gave you form to have parents sign for off premises activities. We also discussed, if you provide transportation or transport children you must update your Written Plan of Care document. We discussed the Clean Water requirements. The provider was assisted with navigating the system in attempt to complete the required survey. Provider could not remember her password or which email she used and she sent an email to retrieve a new one. We were unable to complete this task. All the testing is complete, a Mitigation plan and survey must be completed as soon as possible. Rated License: During this visit we discussed the different pathways, you stated that you are interested in Pathway two, but do not want to continue as a four star facility. Subsidy does not offer funds to one or two star facilities, please take this in consideration when you decide. A follow up email will be sent with the next steps for applying. You will not be required to re-apply until three years from your current license date. The options are Three Pathways to earn a 2-5 Star Rated License; I will send you an email with links to learn more and next steps. Pathway #1 – Program Assessment using the Environment Rating Scales (ITERS-3, ECERS-3, FCCERS-3 and SACERSU). This pathway is similar to what you have done in the past. Pathway #2 – Classroom & Instructional Quality focusing on curriculum and child assessment for demonstration of quality practices: Approved curriculum and assessments must be purchased, used, and you must obtain training to achieve stars over three. Today’s visit was with you, Ms. Janice Smith, owner. This facility currently operates with a four (4) star rated license effective October 30 2018. Upon arrival I was greeted by Ms. Janice, who let me into the facility ad assisted me with the visit. The children were observed napping when I arrived, eating, reading books, and later during the visit they played in activity centers and the provider read a book to them. Children were served backed beans hotdog, bun, apple sauce milk and juice. One child napped in the living room, and two children napped in the activity center area. For snack children had grapes and water. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last documented fire drill was conducted 8-1-25 The last documented shelter-in-place or lockdown drill was conducted 7-2-25 The last documented playground inspection was conducted 8/1/25 I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility does not provide transportation. The provider stated when asked that there are no firearms on the premises. Due to inclement weather the outdoor environment was not monitored today. Consultant was unable to log in to regulatory due to internet issues. A handwritten summary was left with the provider. Violation Number Comment Rule 801 Bacteriological analysis indicating the water is free of bacteria had not been performed on a sample of water from each well used for the children's water supply every two years. Results were not kept on file in the home. Upon arrival I was greeted by Ms. Janice, who let me into the facility ad assisted me with the visit. The children were observed napping when I arrived, eating, reading books, and later during the visit they played in activity centers and the provider read a book to them. Children were served backed beans hotdog, bun, apple sauce milk and juice. One child napped in the living room, and two children napped in the activity center area. For snack children had grapes and water. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last documented fire drill was conducted 8-1-25 The last documented shelter-in-place or lockdown drill was conducted 7-2-25 The last documented playground inspection was conducted 8/1/25 I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility does not provide transportation. The provider stated when asked that there are no firearms on the premises. Due to inclement weather the outdoor environment was not monitored today. Consultant was unable to log in to regulatory due to internet issues. A handwritten summary was left with the provider. During the visit two children arrived from the Head Start bus. Children walked into the living room washed hands and headed to the kitchen for snack. Ms. Janice, woke the other child and all three children sat at the table in the kitchen. Snack consisted of peanut butter crackers, milk and chips. Children were offered books to read while they waited for all children to finish eating. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. During this visit we discussed the grant that you received that will fix your broken closet door in the activity center area. The provider painted the activity space and received a printer, computer and other items to assist with her business. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. I was greeted by Ms. Janice Smith, who opened the door. After stating the purpose of my visit l proceeded with the walk-through of the facility. Upon arrival there were three children in the living room with a plate of food on the couch. Two other children were sitting in the kitchen eating pasta with tomato sauce, oranges, and milk. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. Violation Number Comment Rule 705 All medicines, including refrigerated and unrefrigerated, were not kept in locked storage. There were three prescription medication and one cold and allergy medication in the living room where the children were sitting. .1719(a)(5) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. aerosol air fresheners were on the kitchen table, and in the living room. the kitchen table has additional cleaning supplies. .1719 (a)(7) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and Styrofoam, were accessible to children under the age of three. Plastic bags were in the living room where the children napped and in the front living room on the left of the room near the television. In the dramatic play area, there were several pretend food toys that were still in the plastic the was on it when it was purchased. .1719(a)(18) 1948 Sleeping infants, ages 12 months and younger were not visually checked every 15 minutes. There was no evidence of visually check available for review. .1724(a)(7) 1962 Activity plans did not include activities intended to stimulate the developmental domains, in accordance with NC Foundations for Early Learning and Development. There were no activity plan available for review. Provider stated she was still working on it with her son. 10A NCAC 09 .1718(a)(8)(A)(i-iv) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. REQUIRED RESPONSE: You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Technical Assistance: Rated License: As outlined in Session Law 2021-127 - Senate Bill (SB); facilities who have been unable to hire/retain staff with the same educational levels in the current star rating are in a hold harmless state. Your facility meets these criteria and therefore will retain your current star rating until six months after Governor Cooper issues an order to rescind Executive Order No. 116 Declaration of a State of Emergency to Coordinate Response and Protective Actions to Prevent the Spread of COVID-19. The state of emergency ended August 15, 2022. Your facility is in Cohort two. Today’s visit was with you, Ms. Janice Smith, owner. This facility currently operates with a four (4) star rated license effective October 30 2018. Upon arrival I was greeted by Ms. Janice, who let me into the facility ad assisted me with the visit. At arrival two children were napping. Later during the visit two additional children arrived for a total of four children today. The children were observed napping, reading books, and later during the visit they played in activity centers and the provider read a book to them. Children were served green beans , pasta, meatballs and tomato sauce, with milk and Jello. INSPECTIONS: The last documented fire drill was conducted 8-04-2023. The last documented shelter-in-place or lockdown drill was conducted 3-08-2023. The last documented playground inspection was conducted 08-04-2023. The last compliance history was 91%. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility does not provide transportation. The provider stated when asked that there are no firearms on the premises. The attached checklist was used to note the requirements we monitored today. During this visit we discussed furthering your education and I checked WORKS on your behalf. Raid flying bug spray was left on the kitchen counter. WD 40 was outside on the picnic table. .1719 (a)(7) 716 Electrical outlets not in use were not covered. Outlets were uncovered behind a chair were children were sleeping and in the living room. 10A NCAC .1719(a)(27) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. last documented was March 8, 2023, should have been conducted in June. .1719(a )(16) & .1721(e )(7) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Plastic bags were stored under the bathroom sink, under the changing tables, the dramatic play food was still in plastic and accessible to children. .1719(a)(18) 1953 Operator did not ensure child's hands were washed upon arrival at the home each day. Two children arrived during the visit and they went straight to the kitchen to finish the meal they arrived with. .1725(a)(4) 2042 Swimming pool on the premises was not enclosed by a fence at least four feet high, or the fence was not locked, to prevent access by children. There was no fence, lock or cover. .1730(i) Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. You are required to always maintain compliance with all applicable child care rules and regulations.
