Paris Ham, Child Development Manager, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Eric Faulkner, Executive Director and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On October 30, 2025, a three-year-old child was able to open a gate, exit the fenced outdoor play space and run into the nearby street. 10A NCAC 09 .0601(a) Violations must be corrected immediately. Within one week, November 24, 2025, you must submit documentation of the corrections you made and your plan to maintain compliance with the identified child care requirements to me at Leigh Broome, Investigations Consultant, leigh.broome@dhhs.nc.gov. Upon arrival I rang the doorbell and was greeted by the receptionist. I stated the purpose of my visit and was made aware that the administrator left for the day. I was lead to the conference room where I was introduced to Ms. Tilmon who is in charge in the Directors absence. I greeted her and restated my purpose, and I proceeded with the walk through of the facility, unassisted. Children were observed actively engaged in center play, eating afternoon snack which consisted of milk and animal crackers. Children were also observed transitioning to the bus and departure I conducted a walk-through of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. One new staff file was monitored during this visit. Violation Number Comment Rule 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. New TA file had the medical information with the traditional personnel file. .0701(d) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. Technical assistance: Rated License: There is a new environmental rating scale, Early Childhood Environmental Rating Scales 3rd edition, (ECERS-3). Paris Ham, Child Development Manager, accompanied me during a walk-through of the facility. During the visit, I spoke with Ms. Ham and additional staff members. No children were present because it was a planned teacher workday. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 903 Corporal punishment was used. On March 14, 2024, a staff member hit a child on the buttocks when attempting to address an undesired behavior. GS 110-91(10); .1803(a) 904 Child was handled roughly. On March 14, 2024, a staff member was observed pulling a three-year-old by the wrist. Additionally, the staff member pushed the child to place the child in a different position on a small sofa. .1803(a)(1) Violations must be corrected immediately. Two children were left unattended during a fire drill. Prior to this visit the administrator notified me that she would be doing a self-report concerning the incident. I was greeted by the administrator, Jeannette Smith, and I stated the nature of the visit, and the administrators and teachers were given an opportunity to respond to the concerns. During the visit the classrooms involved in the incident were briefly monitored. Children were observed actively engaged in activity center play and transitioning to lunch. Lunch consisted of pasta and a tomato meat sauce, salad, rolls, apples, and milk. Capacity, Supervision, use of licensed space, space capacity, license restrictions were monitored. The license and emergency care plan were posted, and in compliance. Full enrollment was not captured during this visit. Complaint Visit: Allegation section There is a concern of inadequate supervision. Two children were left unattended during a fire drill. Finding: After teacher and administrators interview. Two children from two different classrooms were left unattended during a fire drill. The fire drill was conducted on September 29th, 2023, during nap time. One child was discovered by the administrator during her walk-through to ensure all staff and children had exited the building. The second child, from space three was discovered by his teacher who stated that she immediately went back in to retrieve the child as her co-teacher let the door close. Both teachers s=from space three stated that they did a headcount prior to heading out of the building. Teachers from space seven were unable to be interviewed as they were still on suspension due to the incident. Based on the interviews and report this incident has been substantiated. Violation Number Comment Rule 303 Children were not adequately supervised at all times. Two children were left inside during a fire drill. .1801(a)(1-5) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during todays visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to follow all the applicable laws and rules. REQUIRED RESPONSE: You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address below by October 25, 2023, The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter, -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE REQUIRED RESPONSE: If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations.
