The previous Annual Compliance visit was completed on December 12, 2024, and prior to today’s visit, the eighteen-month compliance history score was 97% which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a 5-Star License issued November 15, 2024. The license was based on the program earning Cognia accreditation. The information on record with DCDEE was verified as correct by Melissa Karikas, Lead Teacher. Current Inspections are as follows: Sanitation Classification of Superior was completed on January 16, 2025. The most recent fire inspection was completed on September 05, 2024. There are allegations of violations of child care requirements, including: • Supervision • Incident Reporting M. Karikas, Lead Teacher, and Antoinette Williams, Teacher Assistant, were present while I observed the indoor and outdoor areas. Upon my arrival, twelve (12) children were present. The children were observed participating in free-choice activities. The group cleaned up their toys and materials before transitioning to the carpeted area for a dance activity. When the dance activity ended, the teacher told the children to sit on their individual spaces on the carpet. Children were called, one by one, to go to their cubbies to put on a jacket and stand in line at the door. While sitting on the carpet, a child was observed spitting on another child. The teacher responded by saying, “We don’t spit at each other. Remember what we said about personal space? Let’s sit on your space on the carpet.” When all of the children were in line with their jackets on, they transitioned to the outdoor learning environment for outdoor play. While outside, children explored with the age-appropriate stationary equipment, balls, and tricycles. Three children were observed rough-housing on the stairs of the stationary equipment. The teacher called them to the fence to have a discussion about keeping their hands to themselves. Two children were observed having a disagreement about sharing a tricycle. The teacher responded by saying, “You two need to separate since you can’t play nice together.” Children were also observed watching butterflies being released from a butterfly habitat. The teacher stated that the children witnessed the butterfly life cycle. They watched the caterpillars make chrysalises and hatch from cocoons. When children returned inside, they used the bathroom, washed their hands, and sat at tables for lunch. Active supervision was observed. Both teachers positioned themselves to be able to see or hear all children at all times. Routine care and handwashing practices were observed. Lunch consisted of ham and cheese sandwich on whole wheat bread, broccoli, strawberry applesauce, mayonnaise, mustard, and milk. One staff file was reviewed, and all documentation was in compliance with applicable child care rules. The allegations were discussed with the administrator/principal, the NC Pre-K Coordinator, and two additional teachers. The administrator/principal stated that he was aware of the incidents in the NC Pre-K classroom, but was not aware that the child received follow-up medical care. The NC Pre-K Coordinator stated that he was not aware of any incidents regarding a child being injured in the classroom. Both teachers stated that they follow Cumberland County Schools’ policy for reporting incidents by completing an accident report, giving the parent/guardian a courtesy call to inform them what happened, and turning the accident report into the front office to be reviewed and signed by the principal. They also stated that parents are not required to sign the accident report and are given a copy per request only. NC Wise Student Accident Reports for incidents that occurred on March 17, 2025, and January 30, 2025, were available for review. The accident reports did not contain the following information: witness to incident, time parent was notified, where the child received medical treatment, steps to prevent reoccurrence, and a parent signature line. Copies of the incident reports were not given to the parent. An incident report log was not available for review. An incident report was not sent to the child care consultant within 7 days, when follow-up medical care was provided. Based on the information gathered and observations made, there is not enough sufficient information to confirm the allegation of violations of child care requirements pertaining to supervision. Based on the information gathered and observations made, the allegation of violations of child care requirements pertaining to incident reporting is substantiated. At the completion of the visit, the Visit Summary was reviewed with M. Karikas. An incident report for an incident that occurred on March 17, 2025, and required a child to get follow-up medical treatment, was not mailed or emailed to the child care consultant within 7 days. .0802(f) 1952 A copy of the incident report was not given to the parent. Copies of incident reports, that were completed on January 30, 2025, and March 17, 2025, were not given to the parent/guardian. .0802(e) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violation observed and documented today must be corrected immediately. Additionally, by 5:00pm on April 15, 2025, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. This report shall be signed by the person completing it and by the parent, a copy given to the parent or the parent declining a copy and the report maintained in the child's file. When medical treatment is required by a health care professional, community clinic, or local health department as a result of an incident occurring while the child is in care, a copy of the incident report shall be mailed to a representative of the Division within seven calendar days after the incident. The previous Annual Compliance visit was completed on January 11, 2024, and prior to today’s visit, the eighteen-month compliance history score was 97% which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). During a whole group activity, the teachers and children reviewed the letter M. The lead teacher read a book to the children titled, “I’ve Seen Santa”, then the group had a discussion about what they would do if they saw Santa. The group transitioned to free-choice activities. Children were observed participating in math, science, block building, and creative art activities. The group used the bathroom, washed their hands, and put on their coats before transitioning to the outdoor learning environment for outdoor play. While outside, children were observed playing with balls, tricycles, and the stationary equipment. When children returned inside, they used the bathroom, washed their hands, and sat on the carpet to listen to a book about a tractor named Otis. While the lead teacher read the book, the teacher assistant cleaned and sanitized tables to prepare for lunch. Routine care and handwashing practices were observed. Lunch consisted of chicken tenders, ketchup, whole wheat roll, corn, banana, and milk. Two staff files were reviewed, and all documentation was in compliance with applicable child care rules. Two children’s records were reviewed, and all documentation was in compliance with applicable child care rules. I verified compliance by reviewing the physical files as well as the off-site verification forms for the staff and children. Program files were reviewed. The most recent fire drill was completed on November 22, 2024. The most recent shelter-in-place drill was completed on October 24, 2024. The most recent playground inspection was completed on December 02, 2024. The most recent fire inspection report was received during today’s visit. NC Pre-K requirements were reviewed. The Creative Curriculum was being implemented in the activity plan and classroom activities. The Ages and Stages Questionnaires (ASQ-3) were used for developmental screenings and were placed in each child’s file. Hearing and Vision screenings were documented on the children’s health assessment form. Dental Screenings were placed in each child’s file. At the completion of the visit, the Visit Summary was reviewed with M. Karikas. The following violation was observed: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Teaching the Whole Child: Supporting the Social-Emotional Wellness of Preschool and Elementary School-Aged Children This free course explores the connection between students’ social-emotional development, learning, and wellness with their success in Pre-K and elementary school classrooms. The utility of attending to social-emotional wellness and ranges of typical development is discussed, as well as the impact of trauma on students and how school staff can support children in concrete ways to promote their social-emotional learning (SEL). Melissa Karikas, teacher, and Antoinette Williams, teacher assistant, did not complete the required Recognizing and Responding to Suspicions of Child Maltreatment training within the past five years. .1103(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Violations observed and documented today may impact the compliance history score. The violations observed and documented today must be corrected immediately. Additionally, by 5:00pm on January 25, 2024, you must submit a written, dated, and signed statement to me. The statement must describe accurately and in detail, how and when the violations were corrected. If the letter states that corrections have been made when they have not, this may be considered falsification of information. According to Child Care Rule .0304(b), each center shall be inspected annually by an Environmental Health Specialist and receive an approved or superior rating in accordance with applicable sanitation requirements adopted by the Commission for Public Health as described in 15A NCAC 18A .2800. Fire Inspection A violation was cited today for not submitting your most recent fire inspection to DCDEE within 7 days of the inspection visit. According to Child Care Rule .0304(a), the operator shall submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division. The most recent fire inspection was conducted on September 06, 2023. A copy of the inspection was received during today’s visit. Health and Safety Training A violation was cited today for a staff members not having documentation of completing the required health and safety training topics within the past five years. According to Child Care Rule .1103(b), Health and safety training shall be completed as part of on-going training so that every five years, all of the topic areas set forth in 10A NCAC 09 .1102(b) will have been covered. Health and Safety topics include: prevention and control of infectious diseases, including immunization; administration of medication, with standards for parental consent; prevention of and response to emergencies due to food and allergic reactions; building and physical premises safety, including identification of and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; emergency preparedness and response planning for emergencies resulting from a natural disaster, or a man-caused event; handling and storage of hazardous materials and the appropriate disposal of bio-contaminants; precautions in transporting children, if applicable; prevention of shaken baby syndrome, abusive head trauma, and child maltreatment; CPR and First Aid training; recognizing and reporting child abuse, child neglect, and child maltreatment; and prevention of sudden infant death syndrome and use of safe sleeping practices. July 2023 Child Care Rule Changes The DCDEE is excited to share information about the July 2023 child care rule changes. The NC Child Care Commission revised rules related to staff/child ratios for multi-age groups; cooperative arrangements; and enhanced space requirements. These rule changes may or may not impact your facility. The multi-age group staff/child ratio changes support a previous rule change submitted by Early Head Start and the new rule for defining cooperative arrangements provides clarification to a previous change in statute.
