Your program currently operates with a three-star license, issued August 28, 2024, earning 4 points in the education component, 2 points in the program standards component (meeting enhanced space) and 1 quality point for meeting the programmatic option. Restrictions are first shift only, meets enhanced space, capacity of 48 children, ages 0 to 5 years of age. The last annual compliance visit was conducted August 29, 2024. The sanitation inspection was completed August 9, 2025 with a “Superior” classification. The last fire inspection was conducted August 12, 2025 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight percent as of August 18, 2025. All indoor and outdoor spaces were monitored today. Children were observed playing in centers, toileting, having group time and preparing for lunch. All meals are provided by parents. Opt out forms are completed and in each child’s file. There were two violations documented today. Violation Number Comment Rule 504 Perishable foods were not stored to protect against spoilage. Lunch boxes for the children in the classroom for preschoolers were sitting on a table and not stored in the kitchen refrigerator upon my arrival. The staff stated they are normally placed in the refrigerator daily but they had not had time. 15A NCAC 18A .2806 807 A safe indoor and outdoor environment was not provided for the children. A hole was observed under the fence of the playground used by preschoolers. It was located beside the air conditioning unit fence. 10A NCAC 09 .0601(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Staff expressed concern regarding the main entrance door. The door will unlock by itself at times and staff are concerned about someone walking in the building unknown to staff. Child Care Rules .0205 (a)(b)(c) refer to allowing access of persons into the building without knowledge. All doors and alarms should be working properly for the safety of the children. The allegations are as follows: There are concerns that staff/child ratios are not being followed. Today I monitored the attendance and sign in/out documents for the classroom for preschool children. On two days in September, there were twenty-three children, ages two to five, enrolled and present with two staff. Today, there were twenty-two children, ages two to five, enrolled and nineteen present with two staff. The children were observed having group time, preparing for and eating lunch and infant/toddlers were observed having tummy time, free choice in centers and diapering. Allegations were substantiated based on observations made by a collaborating agency. One violation was documented and discussed today with Ms. Diggs, Lead Teacher and Dr. Bennett, Principal. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. On September 4th and 6th, there were twenty three children, ages two to five years of age, enrolled and present with two staff. GS 110-91(7);.0713(a-d) Technical Assistance- To prevent the staff/child ratio issues from occurring in the future, at least one of the following options are recommended- 1. Disenroll three children from the classroom for preschool children. Only twenty children in a group with two staff can be present when the youngest child is two years old. 2. Move three of the younger two year old children to the classroom for toddlers. 3. Divide the classroom for preschool children into two separate areas. One side would have the younger two year old children- up to ten in a group- with one staff. The other side would have the three, four and five year old children and older two year old children with one staff. I recommended contacting Moore County Partners for Children and Families to assist with classroom division and arrangement. Contact them at (910) 949-4045. Ms. Diggs stated staff had been working with the facility earlier and she would contact them for assistance. The allegations are as follows: There are concerns that staff/child ratios are not being followed. Today I monitored the attendance and sign in/out documents for the classroom for preschool children. On two days in September, there were twenty-three children, ages two to five, enrolled and present with two staff. Today, there were twenty-two children, ages two to five, enrolled and nineteen present with two staff. The children were observed having group time, preparing for and eating lunch and infant/toddlers were observed having tummy time, free choice in centers and diapering. Allegations were substantiated based on observations made by a collaborating agency. One violation was documented and discussed today with Ms. Diggs, Lead Teacher and Dr. Bennett, Principal. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. On September 4th and 6th, there were twenty three children, ages two to five years of age, enrolled and present with two staff. GS 110-91(7);.0713(a-d) Technical Assistance- To prevent the staff/child ratio issues from occurring in the future, at least one of the following options are recommended- 1. Disenroll three children from the classroom for preschool children. Only twenty children in a group with two staff can be present when the youngest child is two years old. 2. Move three of the younger two year old children to the classroom for toddlers. 3. Divide the classroom for preschool children into two separate areas. One side would have the younger two year old children- up to ten in a group- with one staff. The other side would have the three, four and five year old children and older two year old children with one staff. I recommended contacting Moore County Partners for Children and Families to assist with classroom division and arrangement. Contact them at (910) 949-4045. Ms. Diggs stated staff had been working with the facility earlier and she would contact them for assistance. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. On or before October 1, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Ms. Diggs assisted me with the visit. Your program currently operates with a three star license, issued November 15, 2019. The facility received 4 points in education, 2 points in Program and one Quality point for the Programmatic option. Restrictions are capacity of 48 children, ages 0 to 5, first shift only, meets enhanced space. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety five percent as of April 17, 2024. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility eating lunch toileting and napping. Infants were napping, finishing lunch diapering. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. One violation was observed today. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. The grassy area outside the classroom exit door for preschool children had high grass and weeds where the children walk to the playground. 15A NCAC 18A .2832(a) On or before May 2, 2024 I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected.
