Shamecca Burch (Mosely), administrator, assisted me with the visit. Currently this program operates with a Five Star license, issued October 19, 2017, earning seven (7) points in education standards, six (6) points in program standards, and one (1) quality point. Restrictions include: first and second shift, meets enhanced ratios and enhanced space. The program does not currently offer transportation. The last annual compliance visit was conducted March 7, 2025. The last sanitation inspection was conducted on December 10, 2025, earning a superior rating. The last fire inspection was conducted on May 14, 2025. The program's compliance history was one hundred percent (100) as of 2/12/2026. The NC Secretary of State's website was checked on 3/7/2025 and LITTLE MIRACLES DAY CARE VASS, N.C. was listed as current-active. I visited each indoor and outdoor space. Children enrolled were observed in free play in the centers, outdoor play, participating in teacher directed activities and personal care routines. Infants and toddlers enrolled were observed during personal care routines, free play, teacher directed activities and outdoor play. Teachers were interacting with the children and attending to the needs of all the children. The program does not currently serve school aged children or offer/provide transportation. Lunch consisted of beans and franks, French fries, whole wheat grain bread, orange slices and milk. The following violations were observed during the visit: Violation Number Comment Rule 542 The written feeding plan was not modified as the child's needs changed. Teacher reported that a seven (7) month old in the space used by the infants received baby food from home, however the feeding plan had not been modified to document the change. 10 NCAC 09 .0902(a) 871 Center staff did not comply with the safe sleep policy. Sleep charts for two infants ages five (5) and seven (7) months old do not document the positions that the children were observed asleep in. 10A NCAC 09 .0606(a) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The facility does not have any current employees listed on the ABCMS portal. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before February 27, 2026, (an extra day was given because the center is closed on the 16th in observance of the Holiday) I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. The purpose of a safe sleep policy is to reduce the risk of Sudden Infant Death Syndrome or SIDS and other sleep related accidents. Infants under the age of one must be visually checked every fifteen (15) minutes and it must be documented properly on the individual sleep charts. The Center for Disease Control (CDC) and the U.S. DHHS recommends that infants be given their own sleep space that is blanket free of soft bedding, pillows, bumper pads and toys. The American Association Pediatrics or AAP recommends that devices such as strollers, swings, infant carrier, car seats and inclined seats not be used for regular sleep routines. The safe sleep policy states that infants be placed in their cribs on their back to sleep. For children who can rollover on their own, I recommend placing a statement over their cribs stating that they can do so. Technical assistance was also offered on completing the moodle training on the ABCMS portal. Once you have completed that training, you must follow the steps to ensure that your facility and yourself are listed in the ABCMS portal. Technical assistance was also offered on updating documents such as the infant feeding plan to ensure that parents requests and instructions are being followed. Updating this document as required when changes occur can ensure that the child is receiving a continuity of care in the absence of the parent or regular caregiver who may have prior knowledge of the recent changes. During this visit we discussed the QRIS Modernization Pathways to the Stars. Mrs. Burch has decided to follow the option #2 Program Assessment for Child Care Centers and Centers Located in a Residence (CLIAR). This pathway includes the following: • Enhanced Ratio(required) and/or Enhanced Space (optional) • 50% of all Lead Teachers and 50% other educators meet 5 Star Education Standards • Family and Community Engagement Foundational Practices AND 3 additional choices • CQI Plan for Facility and Individuals • Approved curriculum for all ages served • Child Observations and assessments • Coaching/training options for administrators and lead teachers. • NCRLAP Self Study I also left copies of the Continuous Quality Improvement and Professional Development Plan, work experience form, Pathway #2 -Classroom and Instructional Quality for Child Care Centers and Centers Located in a Residence (keep those copies and continue working on completing them prior to your next visit.) You stated that you will review the approved curricula and assessment tools at a later date and decide if you are going to continue using your current curriculum or go with another option. Mrs. Burch stated that they have already begun reviewing on the Facility CQI plan for both the center and the staff. Shamecca Burch, administrator, assisted me with the visit. Currently this program operates with a Five Star license, issued October 19, 2017, earning seven (7) points in education standards, six (6) points in program standards, and one (1) quality point. Restrictions include: first and second shift, meets enhanced ratios and enhanced space. The center's compliance history was reviewed with the administrator. The program’s compliance history ninety-eight (98) percent as of January 2, 2024. The NC Secretary of State website was reviewed on January 2, 2024 and LITTLE MIRACLES DAY CARE VASS, NC. was listed as current-active. The license was posted and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Children throughout the facility were observed participating in group time, in free play activity areas, transitions and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. Supervision and staff/child ratios were in compliance. No new staff have been employed since my last visit. The last sanitation inspection was conducted on 11/28/2023 and earned a superior rating. The last fire inspection was conducted on 4/27/2023. Breakfast consisted of cheese toast, orange slices and milk. The following violations were observed during today’s visit: Violation Number Comment Rule 530 Each infant was not held for bottle feeding until he/she can hold his own bottle. A five month old in the space used by the infants was placed in a bouncey seat during feeding. 10A NCAC 09 .0902(b) 531 Bottles were propped. A bottle was propped for an infant seated in a bouncey seat using a blanket. 10A NCAC 09 .0902(b) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A shelter in place was practiced on 6/30/2023 and again on 10/2/2023. .0604(u);.0302(d)(8) Because the violations today were corrected during the visit, a compliance letter is not required at this time. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance was offered on playing music during free play or other group activities without an intended purpose. Sometimes when music is played in the background as a filler, it can take away the opportunity for meaningful conversations and interactions to take place. At other times it can cause the noise level in the classroom to be higher than normal because individuals have to speak over the music in order to be heard. Try limiting times when music is played to music/movement time, as a transitional prompt or during rest periods. Technical assistance was also offered on propping bottles. Per the childcare rule regarding nutrition for infants, each child must be held or placed in age-appropriate seating devices to be fed. Each infant must be held for bottle-feeding until able to hold his or her own bottle. Once a child is able to hold his/her own bottle, it becomes a matter of choice whether the child is held or placed in an appropriate feeding device. Bottles can never be propped. Infants must never be allowed to lie down in a crib with a bottle. The dangers of bottle propping can include, choking risks, tooth decay and ear infections. It also interrupts the bonding time between the caregiver and the child. Shamecca Burch, administrator, assisted me with the visit. Currently this program operates with a Five Star license, issued October 19, 2017, earning seven (7) points in education standards, six (6) points in program standards, and one (1) quality point. Restrictions include: first and second shift, meets enhanced ratios and enhanced space. The center's compliance history was reviewed with the administrator. The program’s compliance history ninety-eight (98) percent as of January 2, 2024. The NC Secretary of State website was reviewed on January 2, 2024 and LITTLE MIRACLES DAY CARE VASS, NC. was listed as current-active. The license was posted and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. Children throughout the facility were observed participating in group time, in free play activity areas, transitions and personal care routines. Infants were engaged in tummy time, napping, and diapering routines. Sleep checks were completed, recorded, and maintained as required. The caregivers were interacting and meeting the developmental needs for each of the children. Supervision and staff/child ratios were in compliance. No new staff have been employed since my last visit. The last sanitation inspection was conducted on 11/28/2023 and earned a superior rating. The last fire inspection was conducted on 4/27/2023. Breakfast consisted of cheese toast, orange slices and milk. The following violations were observed during today’s visit: Violation Number Comment Rule 530 Each infant was not held for bottle feeding until he/she can hold his own bottle. A five month old in the space used by the infants was placed in a bouncey seat during feeding. 10A NCAC 09 .0902(b) 531 Bottles were propped. A bottle was propped for an infant seated in a bouncey seat using a blanket. 10A NCAC 09 .0902(b) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A shelter in place was practiced on 6/30/2023 and again on 10/2/2023. .0604(u);.0302(d)(8) Because the violations today were corrected during the visit, a compliance letter is not required at this time. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical assistance was offered on playing music during free play or other group activities without an intended purpose. Sometimes when music is played in the background as a filler, it can take away the opportunity for meaningful conversations and interactions to take place. At other times it can cause the noise level in the classroom to be higher than normal because individuals have to speak over the music in order to be heard. Try limiting times when music is played to music/movement time, as a transitional prompt or during rest periods. Technical assistance was also offered on propping bottles. Per the childcare rule regarding nutrition for infants, each child must be held or placed in age-appropriate seating devices to be fed. Each infant must be held for bottle-feeding until able to hold his or her own bottle. Once a child is able to hold his/her own bottle, it becomes a matter of choice whether the child is held or placed in an appropriate feeding device. Bottles can never be propped. Infants must never be allowed to lie down in a crib with a bottle. The dangers of bottle propping can include, choking risks, tooth decay and ear infections. It also interrupts the bonding time between the caregiver and the child.
