You, Amber Sain, Lead Teacher, assisted me with the visit. You provided me with applicable program, staff, and children’s records for review. The most recent annual compliance visit was conducted on March 6, 2025. This facility currently operates with a Five star rated license effective December 15, 2017, meeting daytime care only, enhanced ratios, enhanced space, and enhanced ratios minus one child per group. This facility has earned seven points in program standards, seven points in staff education, and one quality point for 75% of lead teachers having a BA/BS or higher in ECE/CD. The license was posted, and the restrictions were in compliance. This facility currently uses Creative Curriculum. The program’s compliance history was 98 percent as of February 24, 2026. A checklist was used to note the requirements monitored today. I observed children in the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children were participating in free play activities, art activities, gross motor play activities, handwashing routines, lunch, and nap. The caregivers were interacting and meeting the developmental needs for each of the children. NC Pre-K requirements were also monitored. The most recent sanitation inspection for your facility was conducted on September 11, 2025. A superior sanitation classification was issued with eight demerits noted on the grade card. The analysis date for the most recent lead water test was November 27, 2024. Lead water testing must be completed every three years. You may review your facilities results by visiting https://www.cleanwaterforuskids.org/en/carolina/. This facility is exempt from required lead-based paint testing. This website also indicated “survey review by RTI” for required asbestos testing. I suggested you or the individual responsible contact RTI to request an update on this facilities status with required asbestos testing. The most recent approved fire inspection for your facility was conducted on August 26, 2025. Records for one new administrator, two new substitutes, one existing staff member and applicable records for all staff were reviewed. All staff members were listed on the facilities ABCMS Provider Portal roster. Two children’s records were reviewed. You stated children currently enrolled do not have any medications. The most recent monthly fire drill was conducted on January 30, 2026, at 11:00am. The most recent quarterly lockdown/shelter-in-place drill was conducted on November 24, 2025, at 9:20am. The most recent monthly playground inspection was completed on February 19, 2026, by Amber Sain. The violation observed today was discussed with you and documented in the Visit Summary left with you at the conclusion of this visit. The following violation was observed: Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The playground inspection for October 2025 was not completed. The date on the playground inspection report was October 21, 2025, however, the results of the inspection were not recorded on the report. .0605(q) The violation documented above must be corrected immediately. On or before March 10, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violation was corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Technical Assistance for the violation observed was provided on the following: Playground inspections- -Playground inspections must be completed and documented monthly. I suggested that you conduct a playground inspection on the same day you complete the monthly fire drill to ensure both are completed each month. I also suggested adding reminder alerts on your computer or phone to complete these inspections at various times each month. Consultation: -I suggested that the individual responsible contact RTI to request an update on this facilities status with required asbestos testing. -Staff Records: I suggested that you create a new substitute packet to ensure all required documentation is received and on file for each staff member. We discussed adding all substitute files to a Google drive for all NC Pre-K programs to utilize. -I reminded you that the next quarterly lockdown or shelter-in-place drill must be completed before the end of the day today, February 24, 2026. -The mulch in the fall zone areas of the composite structure on the playground for preschool children contained the required six inches of mulch. Due to freezing temperatures, some areas of the mulch were frozen. I suggested that you rake and till the mulch in the fall zone areas, adding mulch as needed. -We discussed handwashing routines and traveling from the classroom to the cafeteria. QRIS Pathways to the Stars Rated License Reassessment: A technical assistance visit to review QRIS Pathways to the Stars and your rated license reassessment was conducted on January 6, 2026. You decided to move forward with Pathway #1, Program Assessment Pathway. You began working on your three-month self-study on January 7, 2026. You stated you intend to order the Environment Rating Scale, ECERS-3, on April 2, 2026, prior to Spring Break. I provided you with copies of the application documents that must be completed and submitted to me prior to ordering the Environment Rating Scale. I suggested that you email these documents to me by March 26, 2026. Reminders and Resources: -Providers can speak to a Behavior Support Advisor for advice and resources specific to the challenging behaviors in their classroom by calling 1-888-600-1685 Option 1. Providers may also submit questions to a Behavior Support Advisor online and receive a call or email in response. In addition, providers may post questions in the ‘Talk to the Expert’ Group on an online network, Social Emotional Connections, for early childhood educators. There are also opportunities to join free webinars on challenging topics or classroom activities. Technical Assistance was provided on the following: Medical Action Plan: For children with chronic health care needs, a medical action plan must be attached to the child’s application. A child with a chronic shortness of breath condition did not have a medical action plan on file. You reported that the parent has been given a medical action plan to complete, but the form has not yet been returned. We discussed that all needed paperwork including a medical action plan should be completed and on file before the medication is kept in the classroom. I reminded you that the medical action plan has necessary information about how to respond to the child’s chronic needs, including both medication and non-medication interventions. The medical action plan will need to be completed and placed in the child’s file within the next 2 weeks. Consultation: The following items discussed were in compliance during the visit: -Per Senate Bill 425, facilities are currently held harmless from star rated license assessments. -Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The following violations were observed: Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One child enrolled on August 22, 2023, did not have a signed acknowledgement from the parent for the Summary of the Law until November 14, 2023. The acknowledgement form given to parents at the beginning of the school year did not include a statement of receipt for the NC Summary of the Law and the information contained in the NC Summary of the Law was not included in the parent handbook packet of information. This program currently operates with a Five star rated license effective December 15, 2017. The program’s compliance history was 94 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements monitored today. Children and staff were observed during nap, gross motor activities outdoors, handwashing routines, and a teacher directed group activity. NC Pre-K requirements were monitored during today’s visit. The most recent sanitation inspection for your facility was conducted on March 19, 2024. A superior sanitation classification was issued with 4 demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on September 17, 2023. If a screening is marked as “uncooperative” or “unable to complete” due to the child being uncooperative then this requirement is not in compliance.
