The visit was conducted with you, Alicia Harkey and Melanie Rambarran, Lead Teachers. You provided me with applicable program, staff, and children’s records for review. This program currently operates with a Five star rated license effective April 22, 2022. The program’s compliance history was 98 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements monitored today. Children and staff were observed during free play activities indoors and handwashing routines. NC Pre-K requirements were monitored during today’s visit. The outdoor learning environment was unable to be monitored due to active precipitation. The most recent sanitation inspection for your facility was conducted on January 28, 2025. A superior sanitation classification was issued with 0 demerits noted on the grade card. The most recent lead water test results were completed on April 9, 2024. This website also indicated “Survey Review by RTI” for required asbestos and lead paint testing. The most recent approved fire inspection for your facility was conducted on October 30, 2024. The violations observed today were discussed with you and documented on the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. In space #2, one child was not signed out on 3/12/2025, one child was not signed out on 3/14/2025, and one child was not signed out on 3/25/2025. 10A NCAC 09 .0302(d)(4) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff member employed on 8/1/2012 completed health and safety trainings on 10/18/2019. One staff member employed on 8/1/2019 completed health and safety trainings on 11/14/2019. .1103(b) The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance statement by April 16, 2025. Failure to correct the violation and send the written statement by the due date may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Technical Assistance was provided on the following: -Arrival and Departure Records: All children must be signed in and out each day including the time of the child’s arrival and departure. I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. Before leaving for the day, double check your records to ensure each child has been signed out. -Health and Safety Trainings: C. Charlet and B. Schafer must complete all required health and safety trainings again by April 16, 2025. This includes Recognizing and Responding to Suspicions of Child Maltreatment training available at https://preventchildabusenc.org/online-trainings/. I suggested that you record health and safety trainings for each staff on the Health and Safety Training Record available on the DCDEE website under provider documents and forms. Keeping track of completed training will assist you with ensuring all trainings required have been completed within the first year and again within five years. Health and safety training shall be completed as part of on-going training so that every five years, all of the topic areas set forth in 10A NCAC 09 .1102(b) will have been covered. This includes Medication in Child Care training. Consultation: The following items discussed were in compliance during the visit: -Per Senate Bill 425, facilities are currently held harmless from star rated license assessments. -The posted document under provider documents titled Safe Procedures for Pick Up and Delivery is a tool to construct policies, it is not a sample policy. Child Care Rule .1003(b) of the Child Care Requirements states that child care programs must establish safe procedures for pickup and delivery of children to ensure children are protected from danger and not exposed to risk of harm. This includes safe pick-up and delivery procedures for parents, as well as, safe pick-up and delivery procedures for the transportation of children to and/or from the center, to and/or from public/private school programs, to and/or from the child’s home, to and/or from off-premise activities, such as, field trips and other education outings. The safety procedures must be communicated to parents, and a copy must be posted in the center where the procedures can be seen by parents. - If a child’s vision or hearing screening is marked as “uncooperative” or “unable to complete” due to the child’s age or being uncooperative then this requirement is not in compliance. However, if the health care professional provides additional information such as “due to developmental delays the hearing screening was not completed” then compliance has been met. We discussed having a conversation with the school nurse to relay the expectation of this requirement for children enrolled in NC Pre-K. 10A NCAC 09 .3005 CHILD HEALTH ASSESSMENTS (a) A health assessment shall be on file at the NC Pre-K site within 30 days after a child enters the NC Pre-K program and the assessment may be no more than 12 months old at the time of program entry. The health assessment shall include the following: (1) physical examination; (2) updated immunizations; (3) vision screening; (4) hearing screening; and (5) dental screening. (b) Site-level administrators shall review all health assessment results at the time of the child’s entry into the program to determine whether the assessment includes specific instructions for identified health needs that may require physical or occupational or other therapies to support the child’s development and learning goals. If a screening is marked as “uncooperative” or “unable to complete” due to the child being uncooperative then this requirement is not in compliance. However, if the health care professional provides additional information such as “due to developmental delays the hearing screening was not completed” then compliance has been met. -Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The visit was conducted with you, Alicia Harkey and Melanie Rambarran, Lead Teachers. You provided me with applicable program, staff, and children’s records for review. This program currently operates with a Five star rated license effective April 22, 2022. The program’s compliance history was 98 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements monitored today. Children and staff were observed during free play activities indoors and handwashing routines. NC Pre-K requirements were monitored during today’s visit. The outdoor learning environment was unable to be monitored due to active precipitation. The most recent sanitation inspection for your facility was conducted on January 28, 2025. A superior sanitation classification was issued with 0 demerits noted on the grade card. The most recent lead water test results were completed on April 9, 2024. This website also indicated “Survey Review by RTI” for required asbestos and lead paint testing. The most recent approved fire inspection for your facility was conducted on October 30, 2024. The violations observed today were discussed with you and documented on the Visit Summary left with you at the conclusion of this visit. The following violations were observed: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. In space #2, one child was not signed out on 3/12/2025, one child was not signed out on 3/14/2025, and one child was not signed out on 3/25/2025. 10A NCAC 09 .0302(d)(4) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff member employed on 8/1/2012 completed health and safety trainings on 10/18/2019. One staff member employed on 8/1/2019 completed health and safety trainings on 11/14/2019. .1103(b) The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Technical Assistance was provided on the following: -Arrival and Departure Records: All children must be signed in and out each day including the time of the child’s arrival and departure. I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. Before leaving for the day, double check your records to ensure each child has been signed out. -Health and Safety Trainings: C. Charlet and B. Schafer must complete all required health and safety trainings again by April 16, 2025. This includes Recognizing and Responding to Suspicions of Child Maltreatment training available at https://preventchildabusenc.org/online-trainings/. I suggested that you record health and safety trainings for each staff on the Health and Safety Training Record available on the DCDEE website under provider documents and forms. Keeping track of completed training will assist you with ensuring all trainings required have been completed within the first year and again within five years. Health and safety training shall be completed as part of on-going training so that every five years, all of the topic areas set forth in 10A NCAC 09 .1102(b) will have been covered. This includes Medication in Child Care training. Consultation: The following items discussed were in compliance during the visit: -Per Senate Bill 425, facilities are currently held harmless from star rated license assessments. -The posted document under provider documents titled Safe Procedures for Pick Up and Delivery is a tool to construct policies, it is not a sample policy. Child Care Rule .1003(b) of the Child Care Requirements states that child care programs must establish safe procedures for pickup and delivery of children to ensure children are protected from danger and not exposed to risk of harm. This includes safe pick-up and delivery procedures for parents, as well as, safe pick-up and delivery procedures for the transportation of children to and/or from the center, to and/or from public/private school programs, to and/or from the child’s home, to and/or from off-premise activities, such as, field trips and other education outings. The safety procedures must be communicated to parents, and a copy must be posted in the center where the procedures can be seen by parents. - If a child’s vision or hearing screening is marked as “uncooperative” or “unable to complete” due to the child’s age or being uncooperative then this requirement is not in compliance. However, if the health care professional provides additional information such as “due to developmental delays the hearing screening was not completed” then compliance has been met. We discussed having a conversation with the school nurse to relay the expectation of this requirement for children enrolled in NC Pre-K. 10A NCAC 09 .3005 CHILD HEALTH ASSESSMENTS (a) A health assessment shall be on file at the NC Pre-K site within 30 days after a child enters the NC Pre-K program and the assessment may be no more than 12 months old at the time of program entry. The health assessment shall include the following: (1) physical examination; (2) updated immunizations; (3) vision screening; (4) hearing screening; and (5) dental screening. (b) Site-level administrators shall review all health assessment results at the time of the child’s entry into the program to determine whether the assessment includes specific instructions for identified health needs that may require physical or occupational or other therapies to support the child’s development and learning goals. If a screening is marked as “uncooperative” or “unable to complete” due to the child being uncooperative then this requirement is not in compliance. However, if the health care professional provides additional information such as “due to developmental delays the hearing screening was not completed” then compliance has been met. -Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days.
