The visit was conducted with you, Lori Byars, Preschool Director. Dr. John Mauldin, Administrator was present to review the Rated License paperwork during the visit. This program currently operates with a Five-star issued on April 7, 2021. The permit restrictions include 1st shift, enhanced space, meets reduced ratios, Certified Developmental Day, may care for 13 year olds, and school age playground does not meet childcare safety standards. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year -old children are enrolled. Your program uses Creative Curriculum Teaching Strategies Gold. The last annual compliance visit was conducted November 7, 2025. The most recent sanitation inspection for your facility was conducted on November 21, 2025 A superior sanitation classification was issued with seven demerits noted on the grade card. The most recent approved fire inspection for your facility was conducted on August 25, 2025 for daytime care only. The most recent monthly fire drill was conducted on February 12, 2026. The most recent quarterly lockdown drill was conducted on February 11, 2026. The most recent monthly playground inspection was completed on February 16, 2026. The center's compliance history was reviewed with the operator. The program’s compliance history was 98 percent as of March 2, 2026. A walk-through of the facility was completed today. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play center activities, outdoor play time, and lunch. The caregivers were interacting and meeting the developmental needs for each of the children. A limited monitoring of staff files was conducted for the purpose of reviewing CPR/First Aid and special training. Supervision, staff/child ratio, storage of hazardous products, storage of medication, adequate/approved space, license posted, permit restrictions and program records were also monitored. You provided me with applicable program and staff records for review. Rated License Information: We completed the applicable paperwork for your rated license re-assessment choosing the Accreditation and Head Start Pathway #3 option during today’s visit. Your new Five Star License is based on the following: The license is based on the Head Start educational and programmatic requirements that are comparable or exceed NC QRIS requirements. Your program meets the Head Start Ratio. You completed a Classroom Staff to Child Ratio for Accreditation or Head Start form. This form is to be posted on each classroom space. The Request to use Accreditation and Head Start Licensure Pathway to Earn a Star Rated License form completed. An award letter for the current budget period of July 1, 2025-June 30, 2026 was provided. We also completed and signed the Pathway 3 Accreditation and Head Start Star Level Assessment for Child Care Centers form. The program met the requirements for a Five Star License. A new license will be mailed directly to you and must be posted upon receipt. At that time, return the old license to me as it remains the property of the State of North Carolina. The following violations were observed today. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member who was employed on 9/25/25 did not have verification of First Aid training on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member who was employed on 9/25/25 had verification of CPR training on file that did not indicate pediatric training from an organization that was not DCDEE approved. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before March 16, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. We discussed that certification must be from an approved organization. The list of approved organizations may be found on the DCDEE website. Certification must reflect that pediatric CPR and First Aid skills were successfully demonstrated in person. Consultation: The ABCMS provider portal facility roster should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children throughout the facility were participating in outdoor play time, group story time, free play in activity areas, and lunch time. The center's compliance history was at 90% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A sample of five children’s files were reviewed. Five new staff files were reviewed. A sample of one existing staff file was reviewed. The last monthly fire drill was practiced on 10/22/24. The last lockdown drill was practiced on 11/13/24. The last monthly playground inspection was completed on 10/23/24. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. NC Pre K requirements were monitored. Developmental Day requirements were monitored. The most recent lead water testing results were completed on June 27, 2024. Lead testing must be completed every three years. The most recent sanitation inspection for your facility was conducted on August 16, 2024. A superior sanitation classification was issued with six demerits. The most recent fire inspection for your facility was conducted on July 29, 2024. The facility was approved for daytime care. The following violation was observed and discussed with you today: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the classroom for school age developmental age students, there was one aerosol bottle of Stentsy Disney Princess True Love Awaits spray on an unlocked shelf in the bathroom. .2820(b) The violation observed was corrected during the visit. No correction letter is needed. Technical assistance on violations was reviewed with you. Areas discussed included: Per 15A NCAC 18A.2820(b), All hazardous items with multiple warnings must be kept in locked storage, including all aerosol products. We reviewed the hazardous product rule requirements. I encouraged the Developmental Day staff to complete a daily check of all classroom space to assure items are properly stored. The aerosol Stentsy spray bottle was moved into locked storage. This violation was corrected during the visit. Consultation or other topics discussed today: Rated License Reassessments: Hold harmless has been extended until the new QRIS system is implemented. Programs still have the option to choose to voluntarily complete the star rated license reassessment if they so desire. Kathy Harding, Preschool Director for the Polk County Schools, indicated that the program may be interested in volunteering for the rated license reassessment. We discussed that the rated license reassessment request form should be completed and returned to me when the program is ready to proceed. Lead & Asbestos Testing: Facilities are required to enroll in The Clean Classrooms for Carolina Kids Program for lead water testing, lead paint testing, and asbestos testing. Lead water testing must be completed every three years. You complete this process by visiting https://www.cleanwaterforuskids.org/en/carolina/ Testing for lead paint and asbestos requires programs to enroll at www.cleanwaterforuskids.org/carolina. Facilities should have completed the online process by November 1, 2024. Due to the volume of testing and inspections, completion of the online enrollment process in the program is considered temporary compliance with the testing and inspection requirement. You must enroll in both sections including lead based paint and asbestos testing to comply with the rule requirements. You, Jessie Roush, Lead Teacher, assisted me with the visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective April 7, 2021 . The permit restrictions were in compliance including 1st, meets enhanced space, meets reduced ratios, may also care for 13yr olds, playground for school-age children does not meet playground safety rules and Certified Development Day. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor learning environments and found supervision and staff/child ratios in compliance. Children throughout the facility were participating in large group screen time, free play in activity areas, transitions, personal care routines and rest time. The center's compliance history was at 97% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A sample of six children’s files were reviewed. Five new staff’s files were reviewed. A sample of three existing staff files were reviewed. The last monthly fire drill was practiced on 11/30/23. The last lockdown drill was practiced on 11/9/23. The last monthly playground inspection was completed on 11/27/23. The most recent sanitation inspection for your facility was conducted on November 29, 2023. A superior sanitation classification was issued with ten demerits. The most recent fire inspection for your facility was conducted on August 10, 2023. The facility was approved for day time care. The following violation(s) were observed and discussed with you today: Violation Number Comment Rule 539 When screen time was provided to school-aged children, it was not offered as a free-choice activity; not used to meet a developmental goal; was not limited to 30 minutes per day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. In the classroom for school age Developmental Day children, the children were using screen time and no screen time log was completed. .2508(e)(1-5) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One employee hired 8/18/21, had verification of First Aid training that expired 10/2023. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One employee hired 8/18/21, had verification of CPR training that expired 10/2023. .1102(d) The violation(s) documented must be corrected immediately. On or before December 18, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Qualifying letters are now valid for five (5) years from the date of issue. Please begin the criminal background re-check at least two (2) months prior to the expiration of the current qualifying letter. Resuming Star Rated License Reassessment Information: As a reminder your facility is in cohort 2 for resuming the star rated license reassessment. Your child care program’s prep year will begin on July 1, 2024, through June 30, 2025. Your child care program’s reassessment year will begin on July 1, 2025, through June 30, 2026.
