A checklist was used to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 2/17/25. Currently this program operates with a four (4) star license on 8/19/17. The facility earned six (6) points in program standards, five (5) points in staff education, and one (1) quality point for education options met. Center director Deborah Medford assisted me today. I conducted an assisted walkthrough of the center. The Secretary of State website was used to check the status of the corporation. The status is current and active. The last sanitation inspection was completed 7/22/25 with a ‘Superior’ classification and 4 demerits. The last fire inspection was conducted on 9/9/25. Children were observed playing in activity areas and involved in routine care activities. A full assessment of applicable childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The letter for C. Saunders was due by 7/16/25 and was received 7/22/25. G.S. 110-90.2(b) & .2703(n)&(o) 1898 Staff did not complete the health and safety training within one year of employment. W. Frye, T. Fields, and A. O'Neal did not receive the medication administration training. A copy of the training certificate must accompany the corrective action letter. .1102(a) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Medford. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 2/23/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. CONSULTATION AND RESOURCES: A technical assistance visit was conducted on 2/4/26 to discuss the pathways. Today you indicated you are choosing pathway one and have reached out to schedule a practice assessment with NCRLAP. You have emailed your staff to have the education reviewed for their star rating. I will evaluate these and follow-up with you. Once you receive your scores for the practice assessment, we can arrange a time to review and being your self-study for your center. Staff should conduct the self-assessment tool in the back of the ITERS-3 and ECERS-3 books. Every classroom should complete this. They will use this information for their three-month self-study. We discussed visiting the QRIS Modernization page on the NC Childcare website, https://ncchildcare.ncdhhs.gov, under the ‘What’s New’ tab. In addition, the NCRLAP website, https://ncrlap.org, has many resources and trainings for the ITERS3 and ECERS3 assessments. M. Causey will need the medication administration training by 4/30/2026. S. Tucker will need the training by 9/30/2028. S. Wintz will need the training by 9/16/26. S. Pilkenton will need the training by 12/31/26. B. Lewis will need the training by 9/20/27. We reviewed DCDEE Moodle today and how to locate the training. The health and safety trainings for S. Forrester by 6/11/26, for B. Lemonds by 7/31/26, and for S. Wintz by 9/16/26. D. Creel are due 10/20/26, The medical action plan for a child in room 3 must be renewed by 3/12/26. If the plan is completed by the parent, it is valid for six months. Upon my arrival I was greeted by the Administrator, Deborah Medford , she assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment and attendance was documented, and observations were made. The children were observed participating in free play in their classrooms. The children in the classroom for infants were observed interacting with the teacher. The last annual compliance visit was conducted March 4, 2024. The center is operating four star license that was issued August 9, 2017. The license was earned with 6 points in program standards, 5 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted September 16, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted August 19, 2024. All new staff files and six children’s files were monitored. Your compliance history is 87%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. The arrival times were not documented for two classrooms. 10A NCAC 09 .0302(d)(4) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff hired 8/2024, did not complete this training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff hired 8/2024, did not complete this training. .1102(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff hired 8/2024, did not complete this training. .1102(g) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics.One staff did not complete the health and safety training. .1103(b) The above violations must be corrected immediately, and a letter must be sent to me no later than March 3, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. Christy Saunders, Business Owner, was present and assisted me during today’s visit. I completed a walk-through of the facility, attendance was documented, and observations were made. The children were observed participating in free play in their classrooms and outside. The last annual compliance visit was conducted April 4, 2023. The center is operating under a Four Star License that was issued August 9, 2017. The last sanitation inspection was conducted October 8, 2023 the center received an Approved rating. The last fire inspection was conducted October 5, 2023. Fire drills and playground inspections were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. The music is too loud during nap time. UPDATES The Division of Child Development has announced their plan to resume the Star Rated License process. Facilities have been divided into 3 Cohorts. Each cohort will have a preparation year and an assessment year. Your facility is in Cohort 1. Your preparation year began July 1, 2023 and will end on June 30, 2024. Your assessment year begins July 1, 2024. The preparation year is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. It could be helpful to consider areas where refresher information might be helpful. These ideas can then be incorporated into ongoing training and/or professional development plans as you prepare for your assessment year when the regular star rated license reassessment process is required. Some preparation year ideas to consider are: 1. Review your previous ECERS-R and ITERS-R assessments. 2. Look for assessment related resources at www.ncrlap.org 3. Seek out webinars and/or in-person trainings related to topics of interest. 4. Start a self-study process to become more familiar with the ERS or priority areas relevant to your program 5. Schedule staff meetings to discuss topics related to the Environment Rating Scales 6. Support and encourage staff during a self-study process. Connect them with resources at ncrlap.org 7. Ensure all staff’s education is current in WORKS by having them log in to access their information at https://dcdee.works.nc.gov/ The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. To take the exam, go to DCDEE Moodle and select Early Childhood Professional Development/Child Care and Development Fund (CCDF)/North Carolina Early Childhood Equivalency Exam. Christy Saunders, Business Owner, was present and assisted me during today’s visit. I completed a walk-through of the facility, attendance was documented, and observations were made. The children were observed participating in free play in their classrooms and outside. The last annual compliance visit was conducted April 4, 2023. The center is operating under a Four Star License that was issued August 9, 2017. The last sanitation inspection was conducted October 8, 2023 the center received an Approved rating. The last fire inspection was conducted October 5, 2023. Fire drills and playground inspections were monitored. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. In the classroom for two year olds there was no schedule posted. GS 110-91(12);.0508(a) 428 A current activity plan was not posted for each group of children for reference. In the classroom for toddlers there was no activity plan posted. GS 110-91(12); .0508(a) 847 Parent's medication authorization did not include required information. Several diaper creams in the classroom for two year olds did not have current authorizations on file and one diaper cream did not have an authorization on file. 10A NCAC 09 .0803(4)(6-9) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not have first aid training within 90 days of employment .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not have CPR training within 90 days of employment .1102(d) 1054 Documentation of staff's on-going training was not on file and/or was not current . Three staff did not have current on going training on file. 10A NCAC 09 .1106(a) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. None of the staff had yearly evaluations or staff development plans on file. 10A NCAC 09 .0514(f) 1898 Staff did not complete the health and safety training within one year of employment. One staff did not complete heath and safety training with the required time frame. .1102(a) 9999 A violation was found for which there is no item number.15A NCAC 18A .2819 DIAPERING AND DIAPER CHANGING FACILITIES Diaper changing procedures were not followed in the classroom for toddlers. The above violation must be corrected immediately, and a letter must be sent to me no later than March 15, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. If I do not receive your letter by March 15, 2024 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. The music is too loud during nap time. UPDATES The Division of Child Development has announced their plan to resume the Star Rated License process. Facilities have been divided into 3 Cohorts. Each cohort will have a preparation year and an assessment year. Your facility is in Cohort 1. Your preparation year began July 1, 2023 and will end on June 30, 2024. Your assessment year begins July 1, 2024. The preparation year is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. It could be helpful to consider areas where refresher information might be helpful. These ideas can then be incorporated into ongoing training and/or professional development plans as you prepare for your assessment year when the regular star rated license reassessment process is required. Some preparation year ideas to consider are: 1. Review your previous ECERS-R and ITERS-R assessments. 2. Look for assessment related resources at www.ncrlap.org 3. Seek out webinars and/or in-person trainings related to topics of interest. 4. Start a self-study process to become more familiar with the ERS or priority areas relevant to your program 5. Schedule staff meetings to discuss topics related to the Environment Rating Scales 6. Support and encourage staff during a self-study process. Connect them with resources at ncrlap.org 7. Ensure all staff’s education is current in WORKS by having them log in to access their information at https://dcdee.works.nc.gov/ The North Carolina Early Childhood Equivalency Exam is a new pathway option comparable to a NC Early Childhood Credential. Applicants who achieve a score of 80% or higher on the NC Early Childhood Equivalency Exam earn the new North Carolina Early Childhood Equivalency Certificate. This certificate is equivalent to a North Carolina Early Childhood Credential and will qualify the individual to be a lead teacher in an early childhood classroom. To take the exam, go to DCDEE Moodle and select Early Childhood Professional Development/Child Care and Development Fund (CCDF)/North Carolina Early Childhood Equivalency Exam. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These trainings are free, and participants will receive training certificates when completed.
