Your program currently operates with a 5-Star license issued March 26, 2024. Due to travel time, I arrived at your facility at 9:15am. The last annual compliance was conducted on September 18, 2024. The sanitation inspection was completed April 14, 2025, with a “Superior” classification. The last fire inspection was conducted on October 23, 2024. Your facility was approved for 1st shift care, the facility meets enhanced reduced ratios, enhanced space, and outdoor play is limited to fenced Pre-K playground and field area. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-seven (87) percent as of September 9, 2025. Your facility is owned by Jones County Schools. The Administrator, Elizabeth Thompson, was present and available for consultation. Fourteen (14) children, age four years old, are enrolled and fourteen (14) were present today. There are two (2) approved indoor spaces and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. Upon arrival in your classroom, preschool children were in the gym area. The children transitioned from gym, washed their hands, and transitioned to group time. After group time the children were engaged in a variety of free play activities which included play dough, coloring with markers, super hero dress-up, easel drawing, bristle blocks, measuring tapes, dominoes, magnetic shapes, wooden blocks, legos, and trucks. Routine toileting and handwashing procedures were monitored, and requirements were met. All children were adequately supervised, and staff/child ratios were being maintained. Health and safety trainings were monitored, and requirements were met. Capacity Change: On August 27, 2025, I received an email from you, Elizabeth Thompson, Administrator of Comfort Elementary Pre-K , ID# 52000027, requesting the termination of one (1) NC Pre-K classroom (# 207) due to the classroom no longer being used for NC Pre-K. According to space calculation sheet, classroom #206 has a capacity of thirty (30) children at thirty (30) square feet per child. The capacity of this classroom was limited to eighteen (18), the maximum group size for a NC PreK classroom. However, the maximum group size for NC PreK has increased to twenty (20) and the administrator has requested to increase the capacity from eighteen (18) to twenty (20). Completed paperwork to decrease the capacity at your facility will be submitted to my supervisor for review and then forwarded to the Raleigh office for processing and printing. You will receive a new License indicating the change in capacity from the Raleigh office. When you receive your new License, you must post the new one immediately in a prominent place. Please remember to mail your old License to me, as it is property of the State of North Carolina. You have my contact information. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A criminal Background Check for one (1) staff member was not completed. G.S. 110-90.2(b) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. According to the facility’s Emergency Drill Log, the last shelter-in-place or lockdown drill was conducted on February 27, 2025. .0604(u);.0302(d)(8) Comments: Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. I shared with Ms. Thompson, at the end of today she cannot be around/oversee the NC Pre-K program until a criminal background check is completed and qualification letter received. A compliance letter regarding criminal background checks and all qualification letters must be submitted to me on or before September 24, 2025, verifying correction. Today, we discussed that for any vital changes to your NC Pre-K program, you must reach out to your Childcare Consultant for guidance to remain in compliance with childcare rules. A continued pattern of non-compliance could result in an administrative action. Pre-Service Administrator Form: A Preservice Administrator form for the Principal, E. Thompson, had been completed and submitted to me for review. Once your education information has been uploaded in the Workforce Education System, the Preservice Administrator Form will be processed. NC Pre-K Monitoring: Due to school just starting, NC Pre-K monitoring will take place during your facility’s next routine unannounced visit. Technical Assistance: Emergency Drills: Emergency drills shall be practiced every three (3) months, and records shall be maintained as required. Records of emergency drills documenting the date and time of each drill, the length of time taken, and the signature of the person who conducted the drill shall be available for review. Today I suggest marking a date on your calendar so that you can ensure that an emergency drill is performed every three (3) months. It may also help to plan these drills for different times of the day so children and staff can be prepared no matter the type of activity or routine is taking place. Clean Classrooms for Carolina Kids: Your facility’s status, according to the Clean Classrooms for Carolina Kids website, shows the following: • Water Testing-Completed on April 13, 2021. Your program currently operates with a 5-Star license effective March 26, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was eighty-eight (88) percent as of February 26, 2025. The license was posted, and the restrictions were in compliance. The most recent sanitation inspection was conducted on December 13, 2024, with a superior rating. The most recent fire inspection was conducted on October 23, 2024. There are two (2) approved indoor spaces and one (1) approved outdoor space. Sixteen (16) children between the ages of four (4) and five (5) are enrolled and fourteen (14) children were present today. Age-appropriate materials and activities were provided for all children. A walk-through of the facility was completed today. All indoor and outdoor areas were monitored. Upon arrival at your facility, the preschool children were in the cafeteria eating lunch which consisted of chicken breast sandwiches, corn, sweet potato French fries, peaches, and milk. The children transitioned from lunch to rest time. Each child had an individual cot with individual linens. Soft music was played in the background and appropriate lighting was observed. Routine food service, toileting, handwashing, and cot spacing were monitored, and requirements were met. The program’s compliance history was eighty-eight (88) percent as of February 26, 2025. The license was posted, and the restrictions were in compliance. The most recent sanitation inspection was conducted on December 13, 2024, with a superior rating. The most recent fire inspection was conducted on October 23, 2024. There are two (2) approved indoor spaces and one (1) approved outdoor space. Sixteen (16) children between the ages of four (4) and five (5) are enrolled and fourteen (14) children were present today. The sanitation inspection was completed March 4, 2024, with a “Superior” classification. The last fire inspection was conducted on October 17, 2023. Your facility was approved for 1st shift care, the facility meets enhanced ratios, enhanced space, reduced ratios, and outdoor play is limited to the fenced Pre-K playground and field area. Age-appropriate materials and activities were provided for all children. Upon arrival to you classroom, you were reading a book, “Snowmen at Work” to the children as they sat on the carpet. You explained what an author and illustrator are. The children transitioned to free play activities which included connecting faucet shapes, magnetic colored shapes, building with large hallow wooden blocks and cardboard brick blocks, magnetic letters, gluing, dress up play, and coloring with colored pencils. The outdoor space had a variety of gross motor equipment including three (3) playhouses, sand toys, a caterpillar tunnel, push dump trucks, hula hoops, large legos, a covered sand box, picnic tables, balls, fire truck and school bus tricycles, scooters, three (3) bench toys chests, a large covered dramatic play area, and a large climbing structure with two (2) slides. First Aid/CPR-Renewal: CPR and First Aid training shall be renewed on or before the expiration of the certification for all staff in direct care of children. We discussed tracking tools such as an Outlook calendar to ensure that trainings are obtained in a timely manner and do not expire before new certification is obtained. Health and Safety Trainings: Technical assistance was provided today regarding completion of on-going training hours and the required repetition of Health and Safety topics, most of which are now available for FREE on DCDEE's training platform called Moodle. As discussed, you may use Moodle to start the repetition of topics, and it will also count towards your on-going hours due for this year. Natural and man-made disasters include but are not limited to the following: fire, tornado, flood, power failures, chemical spills, bomb threats, earthquakes, blizzards, nuclear disasters, or a dangerous person(s) in the vicinity. Emergency Preparedness and Response Plan Template In collaboration with NC Child Care Health and Safety Resource Center, and DCDEE, the NC Emergency Management developed a standard EPR template for use (statewide) by licensed child care programs. The content for this template aligns with the EPR in Child Care training and the EPR rules. Shaken Baby and Abusive Head Trauma Policy: As we discussed today, two (2) of the four (4) children records that I reviewed, did not have a signed statement for the review of you facility’s Shaken Baby and Abusive Head Trauma Policy. Child care rules require that all facilities serving children under the age of five years old develop and adopt a Prevention of Shaken Baby and Abusive Head Trauma policy. All staff and parents of children under the age of 5 must sign a statement acknowledging that the policy has been reviewed with them, and the statements must be kept on file. Water, Lead, and Asbestos Testing: As we discussed today, your facility should be enrolled in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids, which previously tested all child care centers in the state for lead in water at drinking and food preparation taps. As this is a legislatively mandated effort, facilities are required to meet rule requirements to have tap water tested every three years and to have inspections for lead paint and asbestos if applicable. The sanitation inspection was completed March 4, 2024, with a “Superior” classification. The last fire inspection was conducted on October 17, 2023. Your facility was approved for 1st shift care, the facility meets enhanced ratios, enhanced space, reduced ratios, and outdoor play is limited to the fenced Pre-K playground and field area. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six (96) percent. Your facility is owned by Jones County Schools. Seventeen (17) children, four (4) years of age are enrolled and fifteen (15) children, were present today. There are two (2) approved indoor spaces and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in the classroom. Age-appropriate materials and activities were provided for all children. Upon arrival to you classroom, you were reading a book, “Snowmen at Work” to the children as they sat on the carpet. You explained what an author and illustrator are. The children transitioned to free play activities which included connecting faucet shapes, magnetic colored shapes, building with large hallow wooden blocks and cardboard brick blocks, magnetic letters, gluing, dress up play, and coloring with colored pencils. The outdoor space had a variety of gross motor equipment including three (3) playhouses, sand toys, a caterpillar tunnel, push dump trucks, hula hoops, large legos, a covered sand box, picnic tables, balls, fire truck and school bus tricycles, scooters, three (3) bench toys chests, a large covered dramatic play area, and a large climbing structure with two (2) slides. Health and Safety Trainings: Technical assistance was provided today regarding completion of on-going training hours and the required repetition of Health and Safety topics, most of which are now available for FREE on DCDEE's training platform called Moodle. As discussed, you may use Moodle to start the repetition of topics, and it will also count towards your on-going hours due for this year. Water, Lead, and Asbestos Testing: As we discussed today, your facility should be enrolled in the Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in building infrastructure. This program is an expansion of Clean Water for Carolina Kids, which previously tested all child care centers in the state for lead in water at drinking and food preparation taps. As this is a legislatively mandated effort, facilities are required to meet rule requirements to have tap water tested every three years and to have inspections for lead paint and asbestos if applicable. Fourteen (14) children, all four (4) years old are enrolled and eleven (11) were present today. There are two (2) approved indoor spaces and one (1) approved outdoor space. At least five (5) activity areas including language, blocks, manipulatives, dramatic play, and art were available in all classrooms. Age-appropriate materials and activities were provided for all children. Upon arrival at your classroom, you and the children were in the outdoor environment. Children were observed, running, riding tricycles and scooters, bouncing on individual bounce balls, and skipping. During the transition from the outdoor space to the indoor space, staff members sang songs and modeled handwashing procedures as the children washed their hands. After handwashing, the children were engaged in a variety of free play activities including playdough, lacing cards, large hallow blocks, farm animals and barns, fruit sorting, and gluing pom-poms. The outdoor spaces had a variety of gross motor equipment including three (3) playhouses, sand toys, a caterpillar tunnel, push dump trucks, hula hoops, large legos, a covered sand box, picnic tables, balls, fire truck and school bus tricycles, scooters, three (3) bench toys chests, a large covered dramatic play area, and a large climbing structure with two (2) slides and a walk climbing wall. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. One (1) out of four (4) children records reviewed did not have a parent signature indicating that a summary of the NC Child Care Law was given to the parent. GS 110-102 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. Due to a thermometer not being in the refrigerator, a temperature of 45 degrees F. or below could not be verified. 15A NCAC 18A .2806(j)(2) 1125 Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. One (1) out of four (4) children records reviewed did not have written permission from a parent that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider before the child was transported. .1003(i)(j) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. Any violations documented must be corrected immediately. Violation 601 was corrected during the visit; therefore it does not have to be included in your compliance letter, On or before Friday, October 6, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 NC Pre-K Monitoring: The NC Pre-Kindergarten Program (NC Pre-K) Site Monitoring Tool for the 2023-2024 school year has not been completed and will be reviewed during the facility’s next routine unannounced visit. The program follows the Jones County school calendar. The length of the day, 7:45am until 3:00pm, meets the instructional day requirement of 6.5-10 ‘hours. Child Assessment and Required Screenings: The assessment tool, GOLD, Teaching Strategies 2010-2011, is in place. Portfolios for each child are maintained electronically. The facility uses the developmental screening tool, DIAL-4 for pre-screening and ongoing assessments. Documentation of pre-screening assessments, health assessments, vision, hearing, and dental screenings were monitored, and requirements were met. Curriculum: The Creative Curriculum for Preschool, 4th Edition, Teaching Strategies, 2002 has been implemented and the classroom has the required resources available for use. Materials to support the current activity plan were displayed throughout the classroom. Family Engagement: Open house was held in August 2023. Parent Engagement Workshops/Activities were scheduled for September 20, 2023, September 27, 2023, and October 4, 2023. Daily parent involvement includes “Keep at Home” and “Return to School” folders and the Class Dojo online application. Parent-Teacher conferences are held twice a year. One hundred (100) percent of parents attended the open house. Staff Requirements: The Administrator and Lead Teacher met NC Pre-K education requirements. Technical Assistance: Rated License: Your facility’s three-year rated license reassessment was due to be completed in February 2021 but was not completed due to reassessments being voluntary during the COVID-19 pandemic. As we discussed today, reassessments will not become mandatory again until July 1, 2024, and your facility has been placed into “Cohort 1.” Placement in this cohort means you will have from July 1, 2023, to June 30, 2024, to plan and prepare for your reassessment, with your reassessment being due between July 1, 2024, and June 30, 2025. In your planning year, you may review resources available at www.ncrlap.org and contact Jones County Partnership for Children for assistance. I am also available to assist you upon request. This is an opportunity to focus on your Star Rated License and consider strategies to help maintain or increase your star rating. Review topics related to the Environment Rating Scales or other quality initiatives. For example, if transitions are challenging, ask yourself what strategies help. Engage to brainstorm, offer support and clarification, and set goals. Allocate resources so you can focus on self-study as part of the workday to help you become more familiar and confident with the assessment process. Also, be reminded that you may voluntarily request a reassessment at any time by contacting me, and that NCRLAP outreach assessments are still being offered if you wish to “practice.” You may also wish to contact technical assistance specialists with Jones County Partnership to help you with mock assessments. Children Records The health and safety of individual children requires that information regarding each child in care be kept and available when needed. Children's records consist of various documentation such as a child's medical and immunization history, emergency medical care information and parental permission to participate in specific activities. This information is the basis for meeting each child's physical, emotional, cognitive, and social needs. Maintaining accurate documentation at the childcare center to determine compliance with the childcare rules and to ensure the health and safety of each child. As we discussed today, please complete any missing documents from children files and remember to verify that an administrator signature is on all required documents. Child Care Immunization Report 2023-2024 This year's Child Care Immunization Reports are due by midnight on Wednesday, November 1, 2023, and will not be accepted after that date. Failure to submit an annual child care immunization report per G.S. 130A-155 will be reported to the Local Health Director for your county. Submit a report even if no children are attending your child care center or your child care center has closed. This will ensure your center meets the reporting requirement and is not considered delinquent. Download instructions and a worksheet to assist in determining the status of each child (Word document).
