The last annual compliance visit was conducted on April 10, 2025. This facility operates with a four-star license which was issued on November 8, 2016. Upon arrival to the facility, I reported to and signed in at the school administrative office and proceeded to the class, which is held in the cafeteria. Upon arrival to the classroom, I was greeted by Ms. Jacobs, Group Leader, and the Teacher Assistant, Ms. Dawn Long. Each of them assisted in some way during the visit. Attendance was taken and observations made. The children were observed having snack, indoor free play, transition to outdoor learning and departing for the day. A full assessment of childcare requirements was conducted on this date. The license was posted, and all required postings were observed, including the summary of laws, emergency numbers, and emergency procedures. Staff files were reviewed, along with a sample of children’s records. The following violations were observed, cited, and technical assistance was provided. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. *The posted Fire Inspection expired in March 2026. There is no documentation that contact with the Fire Department to schedule the inspection was made.** 10A NCAC 09 .0304(a) 1041 Prior to employment a Criminal Background Check was not completed. **Documentation on file provided the a copy of fingerprint attained and forms filled out. However, the qualifying letter was not available for review. A check of the ABCMS portal showed the status as "in process". G.S. 110-90.2(b) The above violations must be corrected immediately, and a letter must be submitted to my office stating how and when the violations were corrected. The letter may be emailed to anita.c.graham@dhhs.nc.gov and must be received by April 15, 2026. If the letter is not received by the due date, a follow-up visit may be necessary to ensure compliance with childcare requirements and to collect the compliance letter. Additional violations may be cited if observed during the follow-up unannounced visit. Technical Assistance: Technical assistance was provided regarding staff qualification requirements. When staff are hired to work with children under DCDEE, each provider must obtain a clear qualifying letter prior to working with children. A qualifying letter may not be in “in-process” status while the staff member is serving children. The qualifying letter must be complete, with all required information submitted through the DCDEE /ABCMS portal and must be maintained on-site for review by a division representative. At the time of the visit, the Teacher Assistant had copies of documentation she stated as submitted; however, the ABCMS portal indicated the status was still “in process.” This staff member must not serve children until a qualifying letter is completed and on file at the facility. The fire inspection observed posted and/or available for review had expired. A new fire inspection must be obtained prior to expiration or within 12 months of the previous inspection. If the fire department has not completed the inspection within 10 to 14 days prior to expiration, it is recommended that the facility contact the fire department in writing to request the inspection and maintain documentation of that inspection was requested. You new staff member who was hired in February 2026 must complete BSAC, Recognizing and Responding to Suspicions of Child Maltreatment, CPR and First Aid trainings by May 2026. This is within three months of her hire date. All health and safety trainings and on-going trainings must be completed by February 2026.
