Jennifer Tallant, principal, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Tallant and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. Three staff members did not have a qualifying criminal background check prior to employment. G.S. 110-90.2(b) 1043 All staff records, except financial records, were not made available for review. During a complaint visit on 1/22/26, staff member’s records were not available for review when requested. Additionally, on 2/17/26, two staff member’s employment applications, three staff member signed personnel and operational policy statements, and three staff member signed acknowledgements of the facility discipline policy were requested for review from the off-site location, listed on the Public-School Off-Site Staff Records Verification form, and were not available for review. G.S. 110-91( 9) 1328 Children's records were not made available for review. On 1/22/26, a child’s record was not available for review upon consultant request. G.S. 110-91(9) Violations must be corrected immediately. The facility operates with a Five (5) Star License, which was issued on June 5, 2018. The last Annual Compliance Visit was conducted on September 6, 2023. Ms. Turkessha Casterlow accompanied on today’s visit Upon arrival to the school, we reported to the office and stated the purpose of our visit and proceeded to the classrooms. This facility has five classrooms. The lead teachers were present in each of the classrooms. Ms. Leslie Clark , Pre-K Coordinator assisted us during today’s visit. The children were observed participating teacher directed circle time, routines and preparing for lunch. The last fire inspection was conducted on April 24, 2024 and approved for day time use only. The last sanitation inspection was conducted on March 8, 2024 with a Superior rating. A partial assessment of child care requirements was conducted this date. The child care license was posted, permit restrictions, supervision, staff/child ratio, staff training and general safety. All staff qualifying letters were current. All required staff trainings, including CPR/First Aid were up to date. The following violations were cited, and technical assistance was offered. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. This was observed in a classroom with children who are four and five years old. 10A NCAC 09 .0604(c) 813 Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. An electrical fan was observe not mounted and out of reach of children and it did not contain a mesh covering. 10A NCAC 09 .0604(d) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Items observed out of compliance with this rule are: Batteries were observed stored in an unlocked drawer, disinfectant wipes, hand sanitizer, were observed placed in unlocked storage. .2820(b) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. Shelter in place drill was not conducted quarterly. .0604(u);.0302(d)(8) The above violations must be corrected immediately. A Corrective Action Letter must be drafted describing how and when the violations were corrected. (c) When not in use, electrical outlets and power strips located in space used by children shall have safety outlets or be covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. (d) Electric fans shall be mounted out of the reach of children or shall be fitted with a mesh guard to prevent access by children. The last Annual Compliance Visit was conducted on September 14, 2022. The facility currently operates with a Five (5) Star License which was issued on June 5, 2018. Upon arrival to the school, I checked in at the front office and proceeded to the classrooms. Ms. Clark, NC PreK Coordinator was present and assisted me during the visit. Attendance was taken and observations made. The children were observed participating in morning routines, indoor free play and having lunch. The center's last Fire Inspection was conducted on September 23, 2022 and was approved for daytime care only. The center's last Sanitation Inspection was conducted on January 31, 2023 and a ‘Superior' rating was achieved. During today's visit, a full assessment of applicable child care requirements; license restrictions maintained, staff/child ratio, supervision, and adequate approved space. Program records were monitored as were criminal background checks. I reviewed staff files and a sampling of children’s records. Emergency drills and playground inspections were current. The following violation was observed, cited and technical assistance was provided. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The mulch beneath the equipment measured 3 inches in depth and the grass is growing up through the mulch. .0605(j) The above violation must be corrected immediately. A Corrective Action Letter must be drafted describing how and when the violation was corrected. Please include the name and identification number of your facility, as well as your electronic signature. The letter must be received by my office on or before September 20, 2023. We discussed the violation cited this date regarding the surfacing around the dome climbing structure on the playground. Please remember the following according to rule; 10A NCAC 09 .0605 OUTDOOR LEARNING ENVIRONMENT IN CHILD CARE CENTERS (j) All stationary outdoor equipment more than 18 inches high shall be installed over protective surfacing. Footings which anchor equipment shall not be exposed. Protective surfacing shall be either: 1) loose surfacing material, including wood mulch, double shredded bark mulch, uniform wood chips, fine sand, coarse sand, and pea gravel, except that pea gravel shall not be used if the area will be used by children under three years of age. (k) The depth of the loose surfacing material shall be based on the critical height of the equipment, which is defined as the maximum height that a child may sit, or stand, as follows: (1) equipment with a critical height of 5 feet or less shall have 6 inches of loose surface materials; (2) for stationary outdoor equipment used by children two years of age or older, the protective surfacing shall extend beyond the external limits of the equipment for six feet; (m) Exceptions to Paragraph (l) of this Rule are as follows: (1)Fall zones may overlap around spring rockers, and around equipment that is more than 18, but less than 30 inches in height. If there are two adjacent structures and one is more than 18, but less than 30 inches in height, the protective surfacing shall extend a minimum of nine feet between the two structures. Comments: Rated License Assessments. All programs continue to be in a “hold harmless” status until February 2024. Cohorts have been established to resume Rated License Assessments. Your program falls in Cohort 2. Therefore, your planning year begins June 30, 2024-July 1, 2025. Therefore, your assessment year will be 2025.
