The last Annual Compliance visit was conducted on April 1, 2025. The facility currently operates with a Five (5) Star License which was issued on February 6, 2020. Upon arrival, I was greeted by the Administrator, Ms. Antoinette Burton. I stated the purpose of my visit, and together we conducted a walk-through of the facility and attendance was taken and observations made. The children were observed participating in teacher directed group time routines and outdoor free play. The center's last Fire Inspection was conducted on July 11, 2025, and was approved for daytime care only. The center's last Sanitation Inspection was conducted on December 18, 2025, and a “Superior” rating was achieved and zero (0) demerits. The Department of the Secretary’s website was used to check the status of the corporation “SALISBURY ROWAN COMMUNITY ACTION AGENCY, INC” on March 12, 2026. The Salisbury Rowan Community Action Agency, Inc. is current/active. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records and Criminal Background Checks were also monitored. Staff and Children’s files were monitored. During today’s visit, three children’s files were monitored and found to be in compliance. During today’s visit all medication was monitored. Transportation is provided by children’s parents. The parents receive a monthly newsletter, receive updates through class DOJO, and they can volunteer throughout the school year. The teachers also have parent meetings and conferences three times a year. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seven five percent. As of 3/13/26 your facility is at 95 % compliance. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space 1 a child did not have a signed permission form to administer medication for "Ventolin Inhaler" . In space 2 - a child did not have a signed permission form from the parent to administer "Symbicort" Inhaler. 10A NCAC 09 .0803(1)(a & b) During today's visit the following violations were cited and technical assistance was offered to maintain compliance. The above violations must be corrected immediately. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature. The letter must be received to my office on, or before 5:00 pm, March 26, 2026. A Corrective Action Letter must include the following items: • The name of your center • Facility ID number • Each violation numbers • How you corrected each violation • Title of the person who signs the letter • Date Please include accompanying documentation of compliance. The letter can be emailed to rena.mccaffrey@dhhs.nc.gov . If a compliance letter is not received at my office by 5:00 pm, March 26, 2026, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Technical Assistance: Today we discussed that all children on medications must have a permission form signed and dated by the parent giving permission to administer the medication. Two children did not have this today for emergency medications. This violation will need to be corrected as soon as possible so as not to hold up your renewal of your license. You were provided an opportunity to ask questions regarding Child Care Requirements. CONSULTATION: Today we discussed the renewal of rated license assessments today. Currently this program operates with a five (5) Star License, issued 2/6/2020. The facility earned seven (7) points in staff education, six (6) points in program standards, and one (1) quality point for meeting enhanced policies, staff benefits package and infrastructure of parent involvement. Rated License Information-QRIS The center has chosen Pathway 3. This facility meets all required standards based on the Head Start Educational and programmatic requirements that are comparable to or exceed NC QRIS requirements. This facility has submitted the “Request for Accreditation and Head Strat Licensure Pathway” on 2/24/26. The center will continue to maintain as a 5-Star license. Congratulations on maintaining a 5- Star license. You should receive your new license by mail. At the completion of the visit a summary was printed, reviewed, and a copy was left with you. On May 7, 2025, the Raleigh office received a report of alleged non-compliance with childcare requirements. Upon arrival at the facility, I was greeted by Ms. Burton, the Center Manager, who assisted me on the walkthrough of the facility. Attendance was taken and observations made. There are twenty-four (24) children present this date, ranging in age from three (3) years to five (5) years old. ALLEGATION: There are allegations of violations of child care requirements. OBSERVATIONS: Today, I discussed the concerns with the Center Manager. I reviewed relevant facility policies. Received written statements from the facility Manager and two staff members. I viewed the video recording of the alleged incident which occurred in a classroom with children who were two and three years old. CONCLUSION: Based on my conversation with the Center Manager and written statements from the staff, and video footage viewed, the allegation regarding a violation of child care requirements is SUBSTANTIATED. The following violation was observed, cited and technical assistance, and resource links were offered. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. . **A staff member in a classroom with children who are three (3) years old was observed on camera displaying aggressive behavior/interaction with a child. While sitting in a circle on the carpet the staff member was observed pulling a child by the arm, placing their face extremely close to the child's face, appearing to be speaking to the child. Once the staff member releases the child’s arm, as the child walked away, the staff member was observed throwing, what appears to be a book in the direction of the child .** G.S. 110-91(10) The above violation must be corrected immediately. Please submit a letter stating when and how you corrected the violation. When drafting the letter please include the item number cited, and any documentation to support the corrective actions taken. Your letter should be received by my office by 5:00 pm, on or before May 22, 2025. The corrective action may be emailed to me at anita.c.graham@dhhs.nc.gov. You were provided with an opportunity to ask questions regarding childcare requirements. We discussed each of the areas of non-compliance this date. TECHNICAL ASSISTANCE: Programs may use specific strategies for positive behavior support, for example, cultivating nurturing, trusting, and positive relationships with the children, as well as developing those relationships through intentional, high-quality interactions and care. This may also include positive verbal interactions. Child care rules and guidelines are for the purpose of maintaining a safe and healthy environment for children. Please be aware that any information obtained during these investigations, and any violations determined, may be included in future Administrative Action. Completion of Immediate Corrective Action does not prohibit the Division from taking further action. However, successful completion of the items will be considered when determining appropriate case closure. The last Annual Compliance visit was conducted on April 9, 2024. The facility currently operates with a Five (5) Star License that was issued on February 6, 2020. Upon arrival to the facility, I was greeted by a staff member, Ms. Scarlett, assistant administrator/lead teacher who accompanied on a walkthrough of the facility. Attendance was taken and observations made. Children were observed participating in the outdoor gross motor activities, indoor free play and routines. The last Fire Inspection was conducted on July 7, 2024 and was approved satisfactory for day time care only. The last Sanitation Inspection was conducted on December 20, 2023, and the program was assigned a superior classification. During today's visit, a partial assessment of applicable Child Care Requirements was conducted. Supervision, staff/child ratio, permit posted, program records, including playground inspections, emergency drills etc. I monitored one new staff members file and the balance of the staff's emergency trainings. This program does not provide transportation. The following violations were observed, cited and technical assistance was provided. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. I observed some "raised sharp" areas along the fence on the playground used by children who are two(2) and three(3) years old. These areas span the scope of the fence and are accessible to the children. G.S. 110-91(6); .0601(b) 802 Safe pick-up and delivery procedures were not communicated to parents and/or were not posted where they can be seen by the parents. I did not observe the safe-pick-up procedures posted at the facility. .1003(b) The above violations must be corrected immediately, and a compliance letter drafted listing the violations cited and stating how each has been corrected. Please submit with supporting documentation of the corrections made. The letter must reach my office on or before November 5, 2024. If the letter is not received by November 5, 2024 a return visit may be required to ensure compliance with North Carolina childcare rules. You were provided an opportunity to ask questions regarding child care requirements. We discussed the items of non-compliance TECHNICAL ASSISTANCE: The North Carolina Division of Child Development and Early Education (DCDEE) requires child care programs to establish safe procedures for picking up and dropping off children. These procedures must, not only be communicated to parents, placed in the child’s file, but must also be posted in the center. A review of a sampling of children’s file, the pickup procedure was present. Please ensure that this requirement is also posted in a prominent area in the school. I have left a sample of the form with the staff today. While reviewing the playground inspection reports, I noticed a reference to the fence's areas of concern. I did inspect the areas and there are a few sharp areas along the fence surrounding the playground used by children who are two(2) and three(3) years old. Maintenance staff was present today and we discussed appropriate materials that may work to offer a buffer to the sharp areas. We discussed placing a breathable mesh to prevent children from possible injury while playing around the fence. Maintenance reached out during the visit to a company to request the specs of the product and timeframes for receiving it. Records are checked when children enter school or childcare. In response to Hurricane Helene, Governor Cooper issued Executive Order 319, which allowed for an extension for immunization and reporting deadlines. Read the details in the State Health Director’s Memo. (see link below) Starting November 1, 2024, children will have a 30-day grace period to show compliance with immunization requirements. The new deadline for Annual Immunization Reports is January 15, 2025. Finally, the Annual Immunization Reports for the 2024-25 school year are now open and can be completed. Upon arrival to the facility, I was greeted by Ms. Burton, school administrator and accompanied on a walkthrough of the facility. Attendance was taken and observations made. Children were observed participating in the outdoor gross motor activities, indoor free play and routines. The last Fire Inspection was conducted on April 24, 2023 and was approved satisfactory for day time care only. The last Sanitation Inspection was conducted on September 22, 2022, and the program achieved a superior classification. During today's visit, a partial assessment of applicable Child Care Requirements was conducted. Supervision, staff/child ratio, permit posted, program records, all staff trainings were reviewed and general safety. The following violations were observed, cited and technical assistance provided. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The last approved sanitation inspection expired on September 22, 2022. The provider did not contact Environmental Service prior to the inspection expiration date. 10A NCAC 09 .0304(b) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Documentation of the completion of the training was not available for Division Representative review; for one of the staff members working in a classroom with children. .1102(g) The above violations must be corrected immediately, and a compliance letter drafted listing the violations cited and stating how each has been corrected.
